  • Vitreous surgery for severe ocular trauma

    Objective To summarize the visual outcome of patients with severe ocular trauma treated with vitreous surgery. Methods Clinical data of 188(191 eyes) with severe ocular trauma treated with vitreous surgery in a period from November 1996 to April 1998 were analysed retrospectively. Results The study included penetrating injury in 56 eyes, foreign bodies in the posterior segment in 70 eyes, blunt injury in 41 eyes , and globe rupture in 24 eyes. Main complications included endophthalmitis in 35 eyes, choroidal bleeding in 20 eyes, retinal detachment in 60 eyes, and vitreous hemorrhage in 97 eyes. Post-opera-tively, out of 188 eyes, except for 3 of patients too young to examine, visual acuity improved in 133(70.7%), including 85(45.2%) with visal acuity 0.02-1.0, 46(24.5%) remained unchanged; and 9(4.8%) had worse vision. Among 34 with no-light-perception, 12 had light-perception or over. Conclusion A majority of severe trauma eyes can be salvaged with considerable visual recovery after adequate and timely vitreous surgery. (Chin J Ocul Fundus Dis,1999,15:4-6)

  • 急性脑梗死尿激酶溶栓治疗后血清尿酸水平变化与梗死体积及神经功能缺损关系

    目的 分析急性脑梗死患者溶栓治疗后尿酸水平变化与梗死体积及神经功能缺损的关系。 方法 选取2006年2月-2011年3月急性脑梗死患者59例,按梗死体积分为小梗死组(≤5 cm3),大梗死组(>5 cm3);根据神经功能缺损程度评分分为轻度损伤组(0~12分),重度损伤组(≥13分),比较不同梗死体积和损伤程度时溶栓前后尿酸水平变化。 结果 尿激酶溶栓治疗后重度损伤组患者较轻度损伤组发病第2天尿酸水平下降显著,差异有统计学意义(P<0.05);不同梗死体积尿酸水平变化不同,大梗死组尿激酶溶栓治疗后较小梗死组发病第2天尿酸水平明显下降,两组间溶栓第2天尿酸水平,差异具有统计学意义(P<0.05)。 结论 急性脑梗死溶栓后尿酸水平下降,梗死体积越大者尿酸水平下降越显著;同时神经功能缺损程度越高者尿酸水平下降越明显。尿酸在缺血再灌注过程中发挥一定作用,尿酸水平变化与脑梗死体积及神经功能缺损程度有关。

  • Value of combining albumin-bilirubin score with standardized future liver remnant in predicting post-hepatectomy liver failure of patient with hepatocellular carcinoma

    ObjectiveTo analyze factors affecting post-hepatectomy liver failure (PHLF) of patients with hepatocellular carcinoma (HCC) and explore value of combining albumin-bilirubin (ALBI) score with standardized future liver remnant (sFLR) in prediction of PHLF.MethodsThe clinical data of patients with HCC underwent curative hepatectomy in the Second and the Fifth Departments of General Surgery of the First Hospital of Lanzhou University from January 2016 to June 2018 were retrospectively reviewed. The risk factors of PHLF were identified through the logistic regression, the area under the receiver operating characteristic curve (AUC) was used to analyze the predictive value of the ALBI score, sFLR, or ALBI score×sFLR.ResultsA total of 72 patients with HCC were enrolled, all of them were the Child-Pugh A grade. The incidence of PHLF was 27.78% (20/72) in these 72 patients with HCC, which was 12.96% (7/54) and 72.22% (13/18) in the 54 patients with ALBI- Ⅰ grade and 18 patients with ALBI- Ⅱ grade respectively, the difference was statistically significant (P<0.001). The results of multivariable analysis of PHLF showed that the PLT (OR=0.030, P=0.018), ALBI grade (OR=11.758, P=0.020), and sFLR (OR=0.835, P=0.003) were identified as the independent predictors of PHLF. The AUC for the ALBI score×sFLR in predicting the PHLF was 0.892, it was greater than that of the ALBI score (AUC=0.799) or the sFLR (AUC=0.773).ConclusionCompared with Child-pugh grade, ALBI grade is more accurate in predicting PHLF of HCC patients, and combining ALBI score with sFLR is better than sFLR or ALBI score alone in predicting PHLF of patients with HCC.

  • Effect of lenvatinib+transarterial chemoembolization+PD-1 antibody (LEN-TAP) on residual liver volume in patients with intermediate and advanced hepatocellular carcinoma

    ObjectiveTo investigate the effects of lenvatinib combined with transarterial chemoembolization (TACE) and programmed death protein-1 (PD-1) monoclonal antibody (Abbreviated as LEN-TAP regimen) on residual liver volume and surgical safety in intermediate and advanced hepatocellular carcinoma (HCC). MethodsThe clinicopathologic data of patients with intermediate and advanced HCC were collected retrospectively, who underwent the LEN-TAP conversion therapy and surgical resection in the Department of Liver Surgery, West China Hospital, Sichuan University from October 2020 to December 2021. The total liver volume, tumor volume, and residual liver volume of the patients before and after conversion therapy were analyzed. ResultsA total of 48 patients were included, 26 of whom had partial remission and 22 had stable disease, the objective response rate was 54.2% (26/48) according to the Response Evaluation Criteria in Solid Tumours 1.1 after conversion therapy. Before and after conversion therapy, the total liver volumes including tumor were (1 607.15±712.22) mL and (1 558.03±573.89) mL [mean difference (MD) and 95% confidence interval (CI)=–57.42(–134.30, 19.46), t=–1.503, P=0.140], the total liver volumes excluding tumor tissue were (1 095.28±227.60) mL and (1 260.31±270.71) mL [MD(95%CI)=165.03(128.13, 201.93), t=8.997, P<0.001], the tumor volumes were 260.25(107.75, 699.50) mL and 121.73 (33.00, 332.88) mL [MD(95%CI)=–222.45(–296.46, –148.44), Z=–5.641, P<0.001], and the residual liver volumes were (493.62±154.51) mL and (567.83±172.23) mL [MD(95%CI)=74.21(54.64, 93.79), t=7.627, P<0.001], respectively. The increase rates of tumor volume and residual liver volume after conversion therapy were (–53.34±33.05)% and (16.34±15.16)%, respectively. The conversional resections were successfully completed in all patients, with 13 (27.1%) cases experiencing postoperative complications and without occurrence of postoperative liver failure. ConclusionThe data analysis results of this study indicate that the LEN-TAP conversion therapy can shrink tumor volume and increase the residual liver volume for patients with intermediate and advanced HCC, which helps to improve the safety of conversion resection.

  • Effects of large amount of vitreous hemorrhage on electroretinogram in rabbits

    Objective To test the effects of large amount of blood in the vitreous on electrophysiological examination. Methods The reductions of transmission of flash light through a serial dilution and depth of whole blood were measured.An experimental model of vitreous hemorrhage in rabbits was established by injecting 0.5ml autologous uncoagulated whole blood into the vitreous cavities after compression with an expanding perfluoropropane gas bubble.Pars plana vitrectomy was performed to clear the blood clots 2 weeks after blood injection.Ganzfeld and bright-flash electroretinography were performed through six-week observation period. Results Blood reduced remarkably the transmission of reduced remarkably the transmission of bright-flash light.Massive vitreous hemorrhage had a dense filtering effect and extinguished the Ganzfeld but not the bright-flash electrotetinogram.About 3.5log units higher of the intensities of bright- flash light than that of conventional method could stimulate the responses of ERG-B waves in blood injected eyes.Slow recovery of Ganzfeld ERG-b waves after vitrectomy were noted within 2 weeks (Plt;0.05),AND ERG-b wave reached at 80-90% of normal level during the third week. Conclusion The ERG-b waves,which become undetectable because of absorption of the dense opacities of the absorption of the dense opacities of the vitreous in eyes with a large amount of vitreous hemorrhage,can be recorded in bright-flash light conditions with nearly nearly normal amplitudes.This result indicates that functions of retina were not severely damaged by the large amount of vitreous hemorrhage. The injection of large amount of blood into vitreous cavities may cause a transient reduction of the amplitudes of ERG-b waves. (Chin J Ocul Fundus Dis,1998,14:104-107)

  • Effect of fiber volume and material property and nucleus pulposus area on intervertebral disc mechanical behavior

    The material properties and volume proportion of the fibers as well as the cross-sectional area proportion of nucleus pulposus vary greatly in different studies. The effect of these factors on the mechanical behavior of intervertebral discs (IVDs) are uncertain. The IVDs finite element models with different parameters were created to investigate the pressure, height, rotation, stress, and strain of the IVDs under loads: pure compression, rotation after compression or axial moment after compression. The results showed that the material properties of fibers had great impact on the mechanical behavior of IVDs, especially on the rotation angle. When the fiber volume ratio was small, its changes had a significant impact on the rotation angle of the IVDs. The area proportions of nucleus pulposus had relatively little effect on the mechanical behavior of IVDs. The IVDs rotation should be observed when validating the model. By adjusting the elastic modulus or volume ratio of fibers within a reasonable range, a model that could simulate the mechanical behavior of normal IVDs could be obtained. It was reasonable to make the area proportion of nucleus pulposus within 25%–50% for the IVDs finite element model. This study provides guidance and reference for finite element modeling of the IVDs and the investigation of the IVDs degeneration mechanism.

  • Preliminary exploration on the application of hydrogel from acellular porcine adipose tissue to assist lipofilling

    ObjectiveTo investigate the effect of hydrogel from acellular porcine adipose tissue (HAPA) on the survival of transplanted adipose tissue.MethodsFor in vitro study, adipose tissue and HAPA-adipose tissue complex were cultured in normoxia and hypoxia atmospheres for 24 and 72 hours. TUNEL and Perilipin immunofluorescence staining were performed to observe the effect of HAPA on apoptosis and survival of adipocities. For in vivo study, 42 healthy male nude mice (4-6 weeks old) weighing 15-18 g were randomly divided into adipose group (group A), 10%HAPA group (group B), 20%HAPA group (group C), 30%HAPA group (group D), 40%HAPA group (group E), and 50%HAPA group (group F) according to different HAPA/adipose tissue volume ratio (n=7). For each group, 1 mL adipose tissue or HAPA-adipose tissue complex was injected subcutaneously into the dorsum of the nude mice. At 4 weeks after transplantation, 7 nude mice in each group were sacrificed and grafts were harvested, gross observation, volume measurement, ultrasound examination, and histologic staining (HE staining, CD31 and Perilipin immunofluorescence stainings) were applied.ResultsHypoxia showed a tendency of promoting adipose tissue necrosis and apoptosis, while HAPA exhibited an obvious effect of inhibiting cell apoptosis in vitro study (P<0.05). For in vivo study, grafts of all groups had intact fibrocapsule. No obvious signs of infection and necrosis were observed at 4 weeks. Volume shrinkage was observed in all groups, however, the groups A-D had significantly higher volume retention rate than groups E and F (P<0.05). Ultrasound examination showed that there were no significant difference in the number and volume of liquify area of the grafts in each group (P>0.05). With the increase of HAPA’s volume ratio, HE staining proved an improved fat integrity while a gradually decreased vacuoles and fibrosis. CD31 immunohistochemical staining showed that the number of neo-vascularisation in groups E and F were significantly higher than those in groups A-D (P<0.05). Perilipin immunofluorescence staining showed that with the increase of HAPA volume ratio, the number of living adipocytes increased gradually, and more new adipocytes could be seen in the field of vision.ConclusionAs the volume ratio of HAPA gradually increased, the survival of transplanted adipose tissue also increased, but the volume retention rate decreased gradually. 30%HAPA was considered the relative optimal volume ratio for its superior adipose tissue survival and volume retation rate.

  • The role of amygdala volume index in surgical evaluation in patients with meisal temporal lobe epilepsy

    ObjectiveTo investigate the role of amygdala volume index(AVI) in surgcial evaluation in patients with mesial temporal lobe epilepsy (mTLE), including clinical features, etiologies and surgical outcome. MethodsThirty six patients were diagnosed as mTLE after surgical evaluation including clinical manifestations, video-electroencephalogram (VEEG) and magnetic resonance imaging (MRI) at the Second Affiliated Hospital of Zhejiang University between March 2013 and March 2016. Bilateral amygdala AVI was then calculated from amygdala volumes on MRI, which were measured with region of interest (ROI) analysis. All patients were treated surgically. Etiologies of mTLE were further confirmed by the histopathology of the resected tissue. ResultsAmong the 35 patients, there is a strong correlation between AVI on the lesion side and age of onset (R =-0.389, P = 0.019) as well as age of surgery (R =-0.357, P = 0.032). No obvious relation can be seen between AVI and gender, history of febrile convulsion, duration of epilepsy, secondary generalized seizure, side of lesion, presurgical seizure frequency and electrode implantation. There is no significant difference in AVI among the five etiologies. At follow-up, thirty patients (80.5%) reached seizure-free, AVI on the lesion side is nota predictor of surgical failure (P > 0.05). ConclusionAVI plays a role in etiology evaluation in patients with mesial temporal lobe epilepsy. Moreover, a larger AVI on the lesion side is correlated with an earlier age of onset. There is limited value of amygdala volume insurgical outcome prediction of patients with mTLE.

  • REDUCED SIZED LIVER TRANSPLANTATION (REPORT OF 1 CASE)

    Objective To investigate the surgical technique of reduced sized liver transplantation. Methods A reduced size liver transplantation was successfully performed on a 11-year old girl with incurable caroli′s disease. Results The recovery of liver graft function was good after the operation in this patients without complications. Conclusion Reduced size liver transplantation is a safe and effective technique for pediatric liver transplantation to provide liver graft. Authors introduced their experiences of surgical technique in this patient.

  • An improved method for calculating liver volume in partial hepatectomy in rats

    ObjectiveTo improve the calculation method of the sum of residual hepatic lobe volume and total liver volume after partial hepatectomy in rats.MethodsOne hundred and thirty-five SD rats of different body sizes were divided into five groups by completely random design. The body length, tail length, chest circumference, body weight and length of hepatic triangle lobe of the rats were measured before surgery. Then, according to the classic Higgins and Anderson methods, different lobectomies of liver were performed for each group: middle lobe + left inner lobe, left outer lobe, bilateral papillary lobe, triangular lobe, and right lateral lobe were removed; the proportion of theoretical liver resection in each group was 38.1%, 30.1%, 7.9%, 7.8% and 15.3%, respectively. The actual liver resection volume and residual liver volume were measured after surgery. we finally analyzed statistical differences of liver volume calculated by different indirect methods. In addition, the correlation analysis and regression analysis were conducted between the preoperative measured rat body surface parameters and the measured whole liver volume, so as to explore a more simple and accurate volume measurement method.ResultsThe actual proportion of liver resection in each group was 35.0%, 29.2%, 7.1%, 4.9% and 12.0%, respectively. Compared with the residual liver lobe volume actually measured, that calculated by using the indirect method of substitution of the theoretical liver resection proportion was statistically different in all the other four groups except the left outer lobe group. However, there was no statistical difference between the residual liver lobe volume actually measured and that calculated by the actual liver resection proportion in the 5 groups. In addition, in the preoperative measurement of 5 kinds of basic parameters of rats, the body length has the best correlation with whole liver volume, and the regression equation is \begin{document}$\hat Y = - 27.667 + 0.899X$\end{document}.ConclusionsThe liver volume calculated by indirect method using the actual liver resection proportion, compared with the theoretical liver resection proportion, is more accurate. Compared with the body weight, the body length has the better correlation with whole liver volume.
