This study was to observe the result in patients suffering from severe ischemia of legs with arterialization of the great saphenous vein. Eighty nine patients suffering from the disease were treated by bridging the autogenous cephalic vein or Gore-Tex artificial vessel between the great saphenous vein and the femoral or external iliac arteries. The results showed that the blood supply of the affected legs was increased immediately after operation. The ulcers of the toes and the plantar areas were healed gradually and the pain relieved. It was concluded that this method did not interfere with the reflux of the venous blood in the diseased limbs. This method was simple, safe and effective for treating severe ischemia of the lower limbs.
Objective To compare the short-term effect of no-touch technique and routine method for harvesting great saphenous vein in coronary artery bypass grafting. Methods The clinical data of patients receiving the great saphenous vein by no-touch technique (a NT group) or routine method (a routine group) for coronary artery bypass grafting from April 2016 to July 2017 in our hospital were retrospectively analyzed. There were 26 males and 13 females with an average age of 60.1±8.0 years ranging from 40 to 75 years in the routine group, as well as 21 males and 17 females with an average age of 61.3±6.9 years ranging from 38 to 75 years in the NT group. The operation time, postoperative ventilation duration, postoperative myocardial injury index, postoperative complications, postoperative ejection fraction, postoperative hospital stays and other indicators were compared. Results There was no statistical difference between the two groups in operation time, postoperative ventilation time, postoperative ejection fraction or postoperative hospital stays. Incisions of postoperative lower extremity in two groups successfully healed. There was no recurrence of acute myocardial infarction or death. All patients recovered and discharged. One patient was treated with thoracotomy in the routine group. One patient in the NT group had bad healing of chest incision, and one patient with acute kidney injury recovered after treatment. Conclusion The great saphenous vein by no touch technique has a good early effect in coronary artery bypass grafting without increasing the risk of surgery.
OBJECTIVE: Retrospective clinical analysis of primary surgical operation in the treatment of lower limb accompanied by chronic leg ulcer were adopted in this study. METHODS: From September 1990 to June 1998, there were 31 males and 20 females, aged 68 years in average, the area of ulcer varied from 5 cm x 3 cm to 22 cm x 11 cm. The ligation and strip of saphenous vein, debridement and free skin flap grafting were finished in primary operation. RESULTS: The skin flaps were survived completely in 50 cases, only 1 case was necrosis partially and healed after changing dressing. Forty-two cases were followed up for 16 months to 9 years (66 months in average), the varicosis and ulcer were healed in 39 cases and only 3 relapsed in ulceration. CONCLUSION: Primary surgical operation in the treatment of varicosis of lower limb accompanied by chronic ulcer is practicable in clinic. The curative efficacy is satisfactory and the operative manipulation is simple.
周围静脉疾病是临床上的常见病和多发病,其发病率为20%~40%,皮肤营养障碍占2%~5%[1,2] ,发病率比动脉疾病约高10倍,发病机理比动脉疾病复杂,分类方法亦不一致,至今存在着争论。下肢慢性静脉功能不全(chronic venous insufficiency,CVI)按其病因可分为原发性、继发性及先天性; 按解剖范围可分为浅、深和交通静脉3个系统,亦即包括浅静脉曲张、深静脉瓣膜功能不全、深静脉血栓形成后遗症、交通静脉瓣膜功能不全等[3,4]。多年来下肢浅静脉曲张主张施行大隐静脉高位结扎加剥脱术,但术后复发者较多。近年来,随着国内、外学者对CVI的深入研究,尤其是对深静脉瓣膜功能不全的认识,使传统下肢浅静脉曲张的概念发生了根本转变。下肢浅静脉曲张已不再被认为是一个独立的疾病,而是一种可由多种不同病因引起的共同临床表现……
目的 观察高位结扎联合腔内激光治疗大隐静脉曲张的疗效。 方法 回顾性分析2008年4月-2009年4月采用高位结扎联合腔内激光治疗32例大隐静脉曲张患者的临床资料,并与2003年-2008年采用传统手术方法治疗的61例患者进行对照分析。 结果 采用高位结扎联合腔内激光治疗的患者1例伤口感染,1例手术后患肢疼痛,3例手术后3个月局部轻度曲张;其余患者下肢症状减轻或消失,手术后1个月复查时活动均无障碍,无下肢深静脉血栓形成,无下肢深静脉损伤发生。高位结扎联合腔内激光治疗与传统手术方法比较具有切口少、出血量小、手术时间短、恢复快、住院时间短、手术后并发症较少、复发率低等优点。 结论 高位结扎联合腔内激光治疗大隐静脉曲张是一种安全有效的治疗方法,与传统手术比较具有明显优势。
Objective To evaluate improved effect for deep venous valve function after superficial vein surgery of lower extremity in the intermediate stage. Methods Totally 43 patients (55 limbs) with varicose veins of lower extremity were enrolled to accept surgical management of vein systems in our department from March 2006 to October 2006. All patients were respectively followed up after 6 months and 4 years about the changes of deep venous valve function with color Doppler ultrasonography. Results Thirty-nine patients’ deep venous valve function kept well up to now, and there was no significant difference between the two results. Four patients without proximal saphenous vein ligation recurred, and there was reflux in deep venous. Conclusion Endovenous laser treatment and ablation of varicose veins of lower extremity with deep venous insufficiency could improve deep venous valve function effectively. Proximal great saphenous vein ligation is important for successful operation.
Objective To compare the condition of the structure and oxidative stress of great saphenous vein grafts between the patients with and without type 2 diabetes mellitus, and to study the mechanisms for providing the theory evidence ofthe protective way for great saphenous vein graft in patients with type 2 diabetes mellitus. Methods The segments of human great saphenous vein graft were collected from 36 patients undergoing coronary artery bypass graft surgery, who were divided into 2 groups, experimental group (17 patients with type 2 diabetes mellitus) and control group (19 patients without type 2 diabetes mell itus). There was no significant difference in age, gender, hypertension, serum creatinine, hyperl ipidemia, smoking, and the number of pathological coronary arteries between 2 groups (P gt; 0.05). Two cm distal great saphenous vein from each patient was obtained. The structure of great saphenous vein was observed by the microscope, and nicotinamide adenine dinucleotide phosphate (NADPH) oxidase enzymatic activity and superoxide anion level were quantified by lucigenin-enhanced chemilumi nescence. Results The NADPH oxidase activity and superoxide anion levels were significantly higher in experimental group [(308.8 ± 33.7) counts/μg and (1 951.71 ± 355.2) counts/(min.mg)] than in control group [(202.7 ± 29.5) counts/μg and (1 230.73 ± 340.5) counts/(min.mg)] (P lt; 0.05). HE staining showed the damage of ultrastructure of great saphenous vein endothel ium in experimental group, including necrosis and exfol iation of endoepithel ial cells, spl itting of the basement membrane, thickened lower layer of the endothelium with vacuoles and deformed vascular smooth muscle cells; however, integrated vessel intima was observed in control group.
Abstract: Objective To evaluate clinical outcomes of endoscopic vein harvesting (EVH)for coronary artery bypass grafting(CABG) in diabetic patients. Methods In this prospective non-randomized control study, patients with type 2 diabetes who underwent CABG from December 2010 to Febuary 2012 in West China Hospital were enrolled. Based on different vein graft harvesting technique, these patients were divided into two groups: an EVH group and a conventional vein harvesting(CVH)group. Perioperative complications were compared between the two groups. Interventional or CT coronary angiogram was used to evaluate bypass graft patency during follow-up. Results A total of 51 patients with type 2 diabetes were enrolled in this study with 24 patients in the EVH group and 27 patients in the CVH group. There was no statistical difference in age, weight, and comorbidities between the two groups. There was no statistical difference in cardiopulmonary bypass time and aortic cross-clamping time between the two groups (67.2±9.8 min versus 68.3±14.5 min, P>0.05; 62.4±11.3 min versus 65.2±10.3 min, P> 0.05). The vein graft harvesting time (35.6±6.4 min versus 45.2±11.4 min, P< 0.05)and rate of delayed leg wound healing(0.0% with 0/24 versus 18.5% with 5/27, P<0.05) of the EVH group were significantly shorter or lower than those of CVH group.There was no statistical difference in major postoperative complications with respect to venous graft failure rate and chest pain during short term follow-up(9.1 months in the CVH group and 9.4 months in the EVH group) between the two groups. Conclusion EVH is a safe, effective, minimally invasive and quick vein graft harvesting technique for CABG in diabetic patients.
ObjectiveTo explore the feasibility of radiofrequency ablation of great saphenous varicose veins in ambulatory surgery.MethodsPatients who underwent radiofrequency ablation of great saphenous varicose veins between May 2018 and June 2019 in General Hospital of Northen Theater Command were prospectively selected. According to their choices of treatment modes, patients were divided into two groups, day surgery group and routine inpatient group. The preoperative waiting time, length of hospital stay, and hospitalization expenses of the two groups were compared. The postoperative pain, tenderness, and complications were followed up on the first day (the next day after operation), the third day, and the fourteenth day after operation, and the time of return to normal life and work and patients’ satisfaction in two groups were recorded.ResultsA total of 95 patients were enrolled, including 52 in the day surgery group and 43 in the routine inpatient group. There was no significantly difference in age, gender, heart rate, mean arterial pressure, or operation time between the two groups (P>0.05). Compared with the routine inpatient group, the day surgery group had shorter preoperative waiting time, shorter length of hospital stay, and lower hospitalization expenses (P<0.05). In the day surgery group, the time of return to normal life and the time of return to work were shorter and patients’ satisfaction was significantly higher than those in the routine inpatient group (P<0.05). There was no significant difference in pain, tenderness, or paresthesia scores between the two groups after surgery according to the follow-up assessment (P>0.05); the pain, tenderness, and paresthesia scores on the first day and the third day were significantly different from those on the fourteenth day after surgery, indicating that the longer the postoperative time of the two groups, the less pain, tenderness, and paresthesia scores were.ConclusionRadiofrequency ablation of great saphenous varicose veins in ambulatory surgery has a short hospital stay, low medical cost, high patient satisfaction, and can ensure the safety of treatment at the same time, which is worthy of clinical promotion
