  • 癫痫发作的报警与预警

    癫痫是一种最常见的神经系统疾病,特点为多数发作无诱因且难以预测。发作可导致合并症,包括外伤及癫痫猝死(Sudden unexpected death in epilepsy,SUDEP),并致生活质量下降。过去20年广泛研究了发作的预警和报警,开发很多方法及设备,如头皮脑电图、颅内脑电图、肌电图、皮肤电变化、心率和心率变异性(Heart rate variability,HRV)。其中HRV是最有前景的方法。发作发放通过网络导致交感神经和副交感神经间不平衡并且改变了自主神经发放合并心率异常。过去20年用计算机方法开发了HRV的谱分析。HRV的变化早于脑电图发作和临床发作的开始。HRV可能是癫痫发作的预警和报警的指标。现在虽有很多关于癫痫的HRV算法,但是缺少标准的对于癫痫患者的方案,并且没有固定的监测模式,使之难以转化为临床实用,解决这个问题是十分重要的。总结出一个HRV评估的最低方案可用于所有癫痫患者的研究十分必要,可使HRV成为预警癫痫发作的有用工具。

  • Predictive value of systolic pulmonary artery pressure on autonomic nerve excitation in 186 patients with valvular disease: A prospective study

    ObjectiveTo explore the predictive value of systolic pulmonary artery pressure (SPAP) on autonomic nerve excitation in patients with valvular disease, so as to provide reference for the formulation of clinical intervention plans. Methods The clinical data of patients with valvular disease who received surgical treatment in the General Hospital of Northern Theater Command from August 28, 2020 to February 3, 2021 were prospectively collected. According to the standard deviation of normal-to-normal R-R intervals (SDNN) of the heart rate variability (HRV) of the long-range dynamic electrocardiogram (ECG) 7 days before the operation, the patients were divided into three groups: a sympathetic dominant (SE) group (SDNN≤50 ms), a balance group (50 ms<SDNN<100 ms) and a parasympathetic dominant (PSE) group (SDNN≥100 ms). The correlation between the changes of echocardiographic indexes and autonomic nerve excitation among the groups and the predictive values were analyzed. Results A total of 186 patients were enrolled, including 108 males and 78 females aged 55.92±11.99 years. There were 26 patients in the SE group, 104 patients in the balance group, and 56 patients in the PSE group. The left anteroposterior diameter (LAD), left ventricular end diastolic inner diameter, ratio of peak E to peak A of mitral valve (Em/Am), left ventricular end diastolic volume, left ventricular end systolic volume and SPAP in the SE group were higher than those in the balance group (P<0.05), while peak A of tricuspid valve (At) and left ventricular ejection fraction (LVEF) were lower than those in the balance group (P<0.05). The LAD and Em/Am in the balance group were significantly higher than those in the PSE group (P<0.05). Multivariate analysis showed that patients in the SE group had lower At (right atrial systolic function declines), lower LVEF and higher SPAP than those in the balance group (P=0.04, 0.04 and 0.00). When HRV increased and parasympathetic nerve was excited in patients with valvular disease, Em/Am decreased (left atrial function and/or left ventricular diastolic function declined) with a normal LAD. Pearson analysis showed that there was a linear negative correlation between SPAP and SDNN, with a coefficient of −0.348, indicating that the higher SPAP, the lower HRV and the more excited sympathetic nerve. Receiver operating characteristic curve showed that when SPAP≥45.50 mm Hg (1 mm Hg=0.133 kPa), the sensitivity and specificity of sympathetic excitation in patients with valvular disease were 84.60% and 63.70%, respectively. ConclusionParasympathetic excitation is an early manifestation of the disease, often accompanied by decreased left atrial function and/or left ventricular diastolic function. Sympathetic nerve excitation can be accompanied by the increase of SPAP and the decrease of left ventricular and right atrial systolic function. SPAP has a unique predictive value for the prediction of autonomic nerve excitation in patients with valvular disease.

  • The Association of BIS and HRV in TargetControlled Infusion of Etomidate Used in Fasttracking Anesthesia of the Elderly

    目的:研究老年患者依托咪酯靶控输注时不同BIS值(脑电双频指数)的HRV(心率变异性)的变化情况,探讨不同镇静深度与HRV之间的关系。方法:选择65岁以上行门诊胃镜检查患者30例,随机分为3组,A组BIS45~55,B组55~65,C组65~75,各组均在麻醉前、麻醉诱导后,术中、术毕监测BIS、HRV及血液动力学指标。结果:A组各监测HRV明显降低(Plt;0.05),B组仅有轻度下降(Pgt;0.05),C组明显升高(Plt;0.05)。结论:患者镇静深度BIS55~65时,即可明显抑制内镜操作刺激所致的HRV变化,是临床较为合适的镇静深度,可显著降低老年患者交感神经活性、交感/迷走神经均衡性和自主神经总张力,利于机体血液动力学稳定。

  • Study on the prediction of cardiovascular disease based on sleep heart rate variability analysis

    The peak period of cardiovascular disease (CVD) is around the time of awakening in the morning, which may be related to the surge of sympathetic activity at the end of nocturnal sleep. This paper chose 140 participants as study object, 70 of which had occurred CVD events while the rest hadn’t during a two-year follow-up period. A two-layer model was proposed to investigate whether hypnopompic heart rate variability (HRV) was informative to distinguish these two types of participants. In the proposed model, the extreme gradient boosting algorithm (XGBoost) was used to construct a classifier in the first layer. By evaluating the feature importance of the classifier, those features with larger importance were fed into the second layer to construct the final classifier. Three machine learning algorithms, i.e., XGBoost, random forest and support vector machine were employed and compared in the second layer to find out which one can achieve the highest performance. The results showed that, with the analysis of hypnopompic HRV, the XGBoost+XGBoost model achieved the best performance with an accuracy of 84.3%. Compared with conventional time-domain and frequency-domain features, those features derived from nonlinear dynamic analysis were more important to the model. Especially, modified permutation entropy at scale 1 and sample entropy at scale 3 were relatively important. This study might have significance for the prevention and diagnosis of CVD, as well as for the design of CVD-risk assessment system.

  • Study on the Optimum Order of Autoregressive Models for Heart Rate Variability Analysis

    Heart rate variability (HRV) analysis technology based on an autoregressive (AR) model is widely used in the assessment of autonomic nervous system function. The order of AR models has important influence on the accuracy of HRV analysis. This article presents a method to determine the optimum order of AR models. After acquiring the ECG signal of 46 healthy adults in their natural breathing state and extracting the beat-to-beat intervals (RRI) in the ECG, we used two criteria, i.e. final prediction error (FPE ) criterion to estimate the optimum model order for AR models, and prediction error whiteness test to decide the reliability of the model. We compared the frequency domain parameters including total power, power in high frequency (HF), power in low frequency (LF), LF power in normalized units and ratio of LF/HF of our HRV analysis to the results of Kubios-HRV. The results showed that the correlation coefficients of the five parameters between our methods and Kubios-HRV were greater than 0.95, and the Bland-Altman plot of the parameters was in the consistent band. The results indicate that the optimization algorithm of HRV analysis based on AR models proposed in this paper can obtain accurate results, and the results of this algorithm has good coherence with those of the Kubios-HRV software in HRV analysis.

  • An Analysis of the Relationship between Resting Heart Rate Controlling and Pulse Wave Velocity in Patients with Long-standing Persistent Atrial Fibrillation

    目的 研究长期持续性心房颤动患者静息心率控制与脉搏波传导速度(PWV)的关系。 方法 序贯收集于2011年12月-2012年3月在四川大学华西医院心脏内科门诊就诊的长期持续性心房颤动患者84例,将人群按静息心率是否低于80次/min,分为标准组(心率<80次/min)和对照组(心率≥80次/min),采用Pearson相关及多元线性回归分析方法,研究静息心率控制与PWV的关系。 结果 ①标准组人群的PWV显著低于对照组,而饮酒率显著高于对照组,差异皆有统计学意义(P<0.05)。②Pearson相关分析显示静息心率与PWV存在线性相关关系(r=0.355,P=0.001);多元线性回归分析显示,在调整了年龄、性别、BMI、收缩压、舒张压、吸烟、饮酒、空腹血糖、总胆固醇、甘油三酯、高密度脂蛋白、低密度脂蛋白等混杂因素影响后,心率与PWV仍独立相关。 结论 长期持续性心房颤动患者的静息心率控制不良与PWV升高关系密切。

  • Progress in the study of mechanisms of cardiac injury induced by infrasound

    At present, the potential hazards of infrasound on heart health have been identified in previous studies, but a comprehensive review of its mechanisms is still lacking. Therefore, this paper reviews the direct and indirect effects of infrasound on cardiac function and explores the mechanisms by which it may induce cardiac abnormalities. Additionally, in order to further study infrasound waves and take effective preventive measures, this paper reviews the mechanisms of cardiac cell damage caused by infrasound exposure, including alterations in cell membrane structure, modulation of electrophysiological properties, and the biological effects triggered by neuroendocrine pathways, and assesses the impact of infrasound exposure on public health.

  • Effect of Experimental Cerebral Infarction on Heart Rate Variability in Rats

    【摘要】 目的 探讨急性脑梗死对心脏自主神经活性的影响。 方法 Wistar大鼠32只随机分为正常组、假手术组和脑梗死组,脑梗死组用线栓法行右侧大脑中动脉阻塞。脑梗死组和假手术组于术前及术后24 h作心率变异性(HRV)检测,同时检测正常组HRV,将3组的HRV指标进行比较。实验终点取各组心肌组织检测儿茶酚胺和神经肽Y(NPY),进行组间比较。 结果 术后24 h脑梗死组和正常组、假手术组相比,窦性心搏间期标准差、均方根,总功率谱、高频功率谱(HF)、低频功率谱(LF)降低,差异有统计学意义。3组比较LF/HF和分数维无明显差异。脑梗死组心肌组织去甲肾上腺素(NA)和NPY高于正常组和假手术组。 结论 脑梗死引起心脏自主神经总活性降低、自主神经功能受损,自主神经末梢去甲肾上腺素和NPY的异常分泌可能是重要的原因。【Abstract】 Objective To investigate the effect of acute cerebral infarction on cardiac autonomic nervous activity. Methods A total of 32 Wistar rats were divided into normal group, sham operation group and infarction group by random. Experimental cerebral infarction in Wistar rats was induced by intraluminal occlusion of middle cerebral artery. About 24 hours after the occlusion or 24 hours after sham operation, the heart rate variability (HRV) sequences were measured, and the HRV values in the three groups were compared. The levels of catecholamine and neuropeptide (NPY) in myocardium were measured. Results At the 24th hour after the occlusion, the standard deviation and root mean square standard deviation of R-R interval, the total power, high frequency (HF) and low frequency (LF) in infarction group were lower than those in normal and sham operation group. LF/HF and fractal dimension did not differ much among the three groups. The levels of noradrenaline and NPY in myocardium in infarction group were higher than those in the other groups. Conclusion It is suggested that acute cerebral infarction may cause the decrease of autonomic nervous activity and damage of the autonomic nervous function; the abnormal secretion of noradrenalin in autonomic nerve ending and NPY may be the important reasons.

  • Non-contact Heart Rate Estimation Based on Joint Approximate Diagonalization of Eigenmatrices Algorithm

    Based on the imaging photoplethysmography (iPPG) and blind source separation (BSS) theory the author put forward a method for non-contact heartbeat frequency estimation. Using the recorded video images of the human face in the ambient light with Webcam, we detected the human face through software, separated the detected facial image into three channels RGB components. And then preprocesses i.e. normalization, whitening, etc. were carried out to a certain number of RGB data. After the independent component analysis (ICA) theory and joint approximate diagonalization of eigenmatrices (JADE) algorithm were applied, we estimated the frequency of heart rate through spectrum analysis. Taking advantage of the consistency of Bland-Altman theory analysis and the commercial Pulse Oximetry Sensor test results, the root mean square error of the algorithm result was calculated as 2.06 beat/min. It indicated that the algorithm could realize the non-contact measurement of heart rate and lay the foundation for the remote and non-contact measurement of multi-parameter physiological measurements.

  • Heart Rate Variability Study Based on a Novel RdR RR Intervals Scatter Plot

    On the basis of Poincare scatter plot and first order difference scatter plot, a novel heart rate variability (HRV) analysis method based on scatter plots of RR intervals and first order difference of RR intervals (namely, RdR) was proposed. The abscissa of the RdR scatter plot, the x-axis, is RR intervals and the ordinate, y-axis, is the difference between successive RR intervals. The RdR scatter plot includes the information of RR intervals and the difference between successive RR intervals, which captures more HRV information. By RdR scatter plot analysis of some records of MIT-BIH arrhythmias database, we found that the scatter plot of uncoupled premature ventricular contraction (PVC), coupled ventricular bigeminy and ventricular trigeminy PVC had specific graphic characteristics. The RdR scatter plot method has higher detecting performance than the Poincare scatter plot method, and simpler and more intuitive than the first order difference method.
