  • 慢性阻塞性肺疾病急性加重引起患者疼痛的病因初步探讨

    目的初步探讨慢性阻塞性肺疾病(简称慢阻肺)急性加重引起患者不同部位疼痛发生的病因。方法回顾性分析自 2017 年 6 月至 2019 年 3 月入住徐州医科大学第二附属医院呼吸内科因慢阻肺急性加重引发不同部位疼痛的 50 例患者的临床资料。入选患者均依据病情需要给予化痰、抗感染、解痉止喘、吸氧改善通气等综合治疗。评估时间点为入院初治疗前、治疗后第 3 天、治疗后 1 周;评估方法采用简式麦吉尔疼痛问卷(SF-MPQ)量表对各时间点患者疼痛情况进行评分。同时评估各时间点患者血细胞分析、降钙素原、超敏 C 反应蛋白、血气分析;酶联免疫吸附试验法测定各时间点患者血清 5-羟色胺及缓激肽水平。结果与治疗前比较,经治疗后第 3 天、治疗后 1 周后,患者 SF-MPQ 疼痛量表评分值明显降低,血清 5-羟色胺及缓激肽水平降低(均 P<0.05)。患者 SF-MPQ 疼痛量表评分与血细胞分析中白细胞计数、超敏 C 反应蛋白、降钙素原、动脉血二氧化碳分压、血清 5-羟色胺及缓激肽水平呈正相关(均 P<0.05),与动脉血氧分压呈负相关(P<0.05)。结论慢阻肺急性加重引起的疼痛症状可能与全身炎症反应、低氧血症及体内二氧化碳潴留致高碳酸血症有一定关系。

  • Impact and Association of the COVID-19 pandemic and respiratory infection prevalence on hospitalization for acute exacerbation of chronic obstructive pulmonary disease

    Objective To understand the changing patterns and characteristics of the number of patients hospitalized with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) before, during, and in the post-epidemic period of the COVID-19 pandemic and the Association between acute respiratory infections and hospitalization of patients with AECOPD. Methods A retrospective analysis was conducted to count the patients hospitalized for AECOPD in the Department of Respiratory Medicine of the Third Affiliated Hospital of Chongqing Medical University from July 2017 to June 2024. The pattern of change in the number of AECOPD hospitalizations and the associations with patients with respiratory tract infections in outpatient emergency departments were analyzed. Results During the COVID-19 epidemic, the number of hospitalizations of patients with AECOPD did not increase compared with the pre-epidemic period. Instead, it significantly decreased, especially in the winter and spring peaks (P<0.05). The only exception was a peak AECOPD hospitalization in the summer of 2022. COPD inpatient mortality and non-medical discharge rates tended to increase during the epidemic compared with the pre-epidemic period. Analysis of the curve of change in the number of patients with respiratory infections in our outpatient emergency departments during the same period revealed a downward trend in the number of patients with respiratory infections during the epidemic and an explosive increase in the number of patients with respiratory infections in the post epidemic period, whose average monthly number was more than twice as high as that during the epidemic. Correlation analysis of the number of patients with respiratory infections between AECOPD hospitalizations and outpatient emergency departments showed that there was a good correlation between the two in the pre-epidemic and post-epidemic periods, and the correlation between the two in the post-epidemic period was more significant in particular (r=0.84-0.91, P<0.001).In contrast, there was no significant correlation in 2021 and 2022 during the epidemic (r=0.24 and 0.50, P>0.05 ). The most common respiratory infection pathogens among AECOPD hospitalized patients during the post-epidemic period were influenza virus, COVID-19 virus, and human rhinovirus, respectively. Conclusions The pandemic period of COVID-19 infection did not show an increase in the number of AECOPD hospitalizations but rather a trend towards fewer hospitalizations. Respiratory infections were strongly associated with the number of AECOPD hospitalizations in the pre- and post-pandemic periods, while the correlation between the two was poorer during the pandemic period. Influenza virus was the most important respiratory infection pathogen for AECOPD during the post-epidemic period.

  • The assessment of clinical characteristics between different LAA phenotype in patients with acute exacerbation in chronic obstructive pulmonary disease

    ObjectiveTo explore the feasibility and clinical application value of low attenuation areas (LAA) scoring system in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).MethodsA total of 380 patients with AECOPD were included. Clinical data including general information, laboratory examinations and treatments during hospitalization were collected. According to the high-resolution computed CT (HRCT) imaging performance, the patients were divided into bronchitis phenotype and emphysema phenotype. The clinical data between these two groups were compared to analyze the differences between different phenotypes and the feasibility of LAA scoring system.ResultsIn patients of bronchitis phenotype, the levels of body mass index, C-reactive protein, interleukin-6, procalcitonin, neutrophil-to-lymphocyte ratio, and eosinophil counts on admission were higher than those of emphysema phenotype (P<0.05). Patients with emphysema phenotype had a higher proportion of male, a higher smoking index, higher cystatin C levels and lower bilirubin levels on admission (P<0.05), the rates of using mechanical ventilation and systemic glucocorticoids were higher as also (P<0.05). LAA scores had a positive correlation with the use of mechanical ventilation and systemic glucocorticoids and cystatin C levels, and a negative correlation with interleukin-6 levels (P<0.05).ConclusionsFor patients with AECOPD, using LAA scoring system to classify different phenotype through HRCT has relevant accuracy and clinical practicability. The LAA scoring system might help to evaluate the patient's condition and prognosis to a certain extent.

  • Physiologic Study of A New Generation of Proportional Assist Ventilation in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease

    ObjectiveTo investigate the physiologic effects of different pressure assist (PA) on ventilatory status,oxygenation and work of breathing (WOB) when a new generation of proportional assist ventilation (PAV) is applied in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). MethodsA prospective,crossover randomized physiologic study was performed.80%PA,60%PA and 40%PA was sequentially randomized to be applied with the duration of 30 minutes,and pressure support ventilation (PSV) with the duration of 30 minutes was applied before each PA.Ventilatory status, oxygenation,dyspnea indexes in PSV and different PA were compared,and WOB of patients and ventilator were compared in different PA. ResultsTwenty-eight patients were recruited into the study.With the decrease of PA,peak inspiratory pressure (PIP),mean airway pressure (Pm),and tidal volume (VT) decreased gradually (P>0.05),respiratory rate (RR) increased gradually (P<0.05),while minute volume (MV),heart rate (HR),systolic blood pressure (SBP),mean arterial pressure (MAP),pH,arterial carbon dioxide tension (PaCO2),and arterial oxygen tension/inspired oxygen fraction (PaO2/FiO2) did not change significantly (P>0.05).Compared with PSV mode,PIP increased significantly in 80%PA (P<0.05),decreased significantly in 60%PA and 40%PA (P<0.05).Pm did not change significantly in 80%PA (P>0.05),decreased significantly in 60%PA and 40%PA (P<0.05).VT increased significantly in 80%PA (P<0.05),decreased significantly in 60%PA and 40%PA (P<0.05).RR did not change significantly in 80%PA (P>0.05),increased significantly in 60%PA and 40%PA (P<0.05).MV did not change significantly in 80%PA (P>0.05),decreased significantly in 60%PA and 40%PA (P<0.05).With the decrease of PA,Borg score and scale for accessory muscle use increased gradually (P<0.05).Compared with PSV mode,Borg score and scale for accessory muscle use did not change significantly in 80%PA (P>0.05),increased significantly in 60%PA and 40%PA (P<0.05).WOB of patients in 40%PA was significantly higher than that in 60%PA (P=0.000) and that in 80%PA (P=0.000),while which in 60%PA was significantly higher than that in 80%PA (P=0.000).On the contrary,WOB of ventilator in 40%PA was significantly lower than that in 60%PA (P=0.004) and that in 80%PA (P=0.000),while which in 60%PA was significantly lower than that in 80%PA (P=0.000). ConclusionThe new generation of PAV can safely and effectively provide respiratory support to patients with AECOPD.Respiratory pattern,levels of dyspnea and accessory muscle use in 80%PA are similar with those in PSV.With the decrease of PA,levels of spontaneous breathing and WOB increase and dyspnea worsens.PAV can promote spontaneous breathing and prevent respiratory muscle disuse atrophy even more, but easily lead to respiratory muscle fatigue with inappropriate use.

  • The clinical phenotype of acute exacerbation of chronic obstructive pulmonary disease by cluster analysis

    ObjectiveTo explore the clinical phenotype of patients with acute exacerbation of chronic obstructive pulmonary disease (COPD) by cluster analysis and provide a basis for individualized treatment.MethodsA total of 515 patients with acute exacerbation of COPD admitted to this department from January 2014 to December 2016 were enrolled. The age, duration, smoking index, number of hospitalizations in the past 1 year, hospitalization days, treatment costs and other information were collected for cluster analysis.ResultsThe patients were divided into three categories of phenotype: " mild-glucocorticoid resistance-antibiotic dependent”," mild-glucocorticoid sensitive”, and " serious complication”. The patients with the first two phenotypes had a milder condition and lower hospitalization costs. There were differences in the time and cumulative dose of glucocorticoids in different pathways, antibiotic use time and usage rate. The third phenotype was the most serious, with the highest cost of hospitalization, and may merge or co-exist with other diseases such as cardiovascular disease and digestive tract disease.ConclusionCluster analysis may identify different phenotypes of acute exacerbation of COPD to provide a reference for clinical individualized treatment.

  • A study on the combination of Wells score and thromboelastography for rapid prediction of acute exacerbation of COPD complicated with pulmonary embolism

    Objective To explore the clinical value of Wells score combined with thromboelastography (TEG) in rapid prediction of pulmonary embolism (PE) in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods A total of 130 AECOPD patients admitted to the Department of Respiratory and Critical Care Medicine of Guangxi University of Science and Technology from January 2022 to March 2023 were selected as the study subjects. According to results of digital subtraction angiography (DSA) of the pulmonary artery, the patients were divided into a PE group (n=37) and a non-PE group (n=93). Both groups underwent Wells score analysis combined with TEG. Thromboelastographic parameters were comparing between the two groups, including Wells score, reaction time (R time), reaction time (K time), coagulation index (CI), and maximum amplitude (MA). The diagnostic value of different prediction models to diagnose AECOPD with PE was compared, including Wells score, thromboelogram and Wells score combined with TEG. Results The Wells score of the patients in the PE group was significantly higher than that in the non-PE group, and R time of the patients in the PE group was significantly lower than that in the non-PE group. The K time, CI, MA and other parameter levels of the patients in the PE group were significantly higher than those in the non-PE group (all P<0.05). The sensitivity, specificity, and area under ROC curve of the combination of Wells score and TEG prediction model in diagnosing AECOPD complicated with PE were greater than those of the Wells score alone prediction model (P<0.05). Conclusion The combination of Wells score and TEG has significant clinical value in quickly predicting AECOPD complicated with PE, and is worthy of clinical application and promotion.

  • The Relevance of the Ratio of Pulmonary Arterial Diameter to Aortic Diameter Exceeding One with Brain Natriuretic Peptide and Inflammatory Factors in Patients with Acute Exacerbation of COPD

    ObjectiveTo explore the relevance of the ratio of pulmonary arterial diameter to aortic diameter exceeding one (PA:A>1) with brain natriuretic peptide (BNP) and inflammatory factor levels in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). MethodsFrom August 2013 to December 2013,95 inpatients with AECOPD in West China Hospital were divided into two groups according to the ratio of pulmonary arterial diameter to aortic diameter. The clinical data of the patients were collected. Meanwhile,arterial blood gas,plasma levels of BNP,C-reactive protein (CRP),and interleukin-6 (IL-6) within 24 hours were measured. ResultsThe plasma BNP level was 2005(483-4582)ng/L in the group with PA:A>1,and 404(137-1224)ng/L in the group with PA:A<1. There was significant difference in plasma BNP level between two groups (P<0.01). There was no significant difference in CRP or IL-6 level between two groups (P>0.05). ConclusionThe ratio of pulmonary arterial diameter to aortic diameter is correlated with BNP level in patients with AECOPD,but is not correlated with CRP or IL-6.

  • Correlation analysis of red blood cell distribution width and disease severity of acute exacerbation of bronchiectasis in the elderly patients

    ObjectiveTo investigate the relationship between the red blood cell distribution width (RDW) and the severity of acute exacerbation of bronchiectasis in elderly patients.MethodsThe clinical data of 216 elderly patients with acute exacerbation of bronchiectasis admitted from January 2015 to October of 2018 were analyzed retrospectively. The severity of acute exacerbation of bronchiectasis in the elderly was evaluated by bronchiectasis severity index (BSI) score. Meanwhile, 50 elderly people receiving qualified medical examination were collected as a healthy control group in the same period. The distributions of BSI score, RDW, procalcitonin (PCT), neutrophil percentage (NEU%) and C-reactive protein (CRP) were described in the patients with different risk degree. The severity of acute exacerbation of bronchiectasis in the elderly was evaluated by BSI. The patients were divided into three groups by BSI score: a low risk group, a middle risk group, and a high risk group. The indexes were described including the distribution of stratified BSI score, RDW, PCT, NEU%, CRP at different risk levels. The correlation of each index was analyzed by Spearman correlation. The threshold value of RDW was calculated by general linear regression, and the influencing factors of BSI score were analyzed by multivariate linear regression analysis.ResultsThe higher the risk stratification, the higher the BSI score, RDW, PCT, NEU% and CRP were. RDW was positively correlated with PCT, NEU% and CRP (r values were 0.425, 0.311, 0.177, respectively, P<0.05). BSI score was positively correlated with RDW, PCT, NEU%, and CRP (r values were 0.425, 0.394, 0.650, 0.578, respectively, P<0.05). RDW was positively correlated with PSI score (r=0.425, P<0.05). The thresholds of RDW were 11.45% and 14.03%. Multiple linear regression showed that RDW, PCT, NEU% and CRP were all influential factors of BSI score and explained 52.3% of the total mutation rate.ConclusionRDW is related to the severity of acute exacerbation of bronchiectasis in the elderly, and can predict the severity of acute exacerbation of bronchiectasis in the elderly.

  • The influence of gastroesophageal reflux on the acidity of airway and pulmonary ventilation function in acute exacerbation of chronic obstructive pulmonary disease patient

    0bjective To investigate the prevalence of gastroesophageal reflux(GER)in acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and to explore the influence of GER on the acidity of airway and pulmonary ventilation function in AECOPD patients.Methods Sixty-five hospitalized AECOPD patients and 20 healthy volunteers were enrolled in this study.According to “GER survey”questionnaire,AECOPD patients were allocated into GER group and non-GER group and expired breath condensate(EBC)pH and pulmonary ventilation function were detected.Breath condensate samples were collected using a specially designed condensing chamber(EcoScreen,Germany).Results About 30.8% (20/65)AECOPD patients were complicated with GER.EBC pH was lowest in GER group,middle in non-GER group and highest in the control group[6.oo±0.75 VS 6.76±0.77 and 7.97±0.12,both Plt;0.05).The pulmonary ventilation function(FEV1、FEV1%pred)in AECOPD patients with GER were lower than those without GER(both Plt;0.05).Conclusions There iS a high prevalence of GER in AECOPD patients.And the airway pH is reduced in AECOPD patients,especially when complicated with GER,which may play an important role in the development of COPD.

  • Evidence-Based Evaluation and Selection of Essential Medicine for Township Health Centre in China: 4. Acute Exacerbation of Chronic Bronchitis

    Objective To evaluate and select essential medicine for acute exacerbation of chronic bronchitis (AECB) using evidence-based methods based on the burden of disease. Methods By means of the approaches, criteria, and workflow set up in the second article of this series, we referred to the recommendations of evidence-based or authority guidelines from inside and outside China, collected relevant evidence from domestic clinical studies, and recommended essential medicine based on evidence-based evaluation. Data were analyzed by Review Manager (RevMan) 5.1 and GRADE profiler 3.6 to evaluate quality of evidence. Results (1) Five guidelines were included (four foreign guidelines, one domestic guideline; three based on evidence, two based on expert consensus). (2) Medicines recommended at least twice by the National Essential Medicine List (NEML, 2009 version) and Chinese National Formulary (CNF) contained: ipratropium bromide (four times), amoxicillin amp; clavulanate potassium (three times), and corticosteroid (three times), cefuroxime (twice), ciprofloxacin (twice), levofloxacin (twice), salbutamol (twice) and dextromethorphan (twice). (3) As for domestic study evidence, a result of one RCT indicated that amoxicillin amp; clavulanate potassium had efficiencies of 92.3% to 94.7% (n=77, low quality). A result of three RCTs (n=275, low quality) indicated that cefuroxime had efficiencies of 67.6% to 90% and an incidence of 5% as to adverse reaction that mainly included skin rashes, diarrhea, etc. A result of two RCTs (n=120, low quality) indicated that ciprofloxacin had efficiencies of 78.3% to 86.6%, bacterial clearance rates of 72.7% to 86.5% and the incidences of 8.7% to 16.2% as to adverse reaction that mainly included gastrointestinal reaction, skin rashes, etc. A result of seven RCTs (n=523, low quality) indicated that levofloxacin had efficiencies of 72.5% to 94.5%, bacterial clearance rates of 82.1% to 95.8% and the incidences of 5% to 7.5% as to adverse reaction. A result of two RCTs (n=239, low quality) indicated that salbutamol had efficiencies of 85.4% to 96.7%. A result of one RCT (n=95, low quality) indicated that ipratropium bromide had efficiencies of 98%. A result of five RCTs (n=466, low quality) indicated that the combined use of budesonide and bronchodilators had efficiencies of 93.4% to 97.8%. Conclusion (1) We offer a b recommendation for cefuroxime, amoxicillin amp; clavulanate potassium, ciprofloxacin and levofloxacin used in the treatment of AECB. (2) We offer a b recommendation for glucocorticoid (budesonide, aerosol) and anticholinergic bronchodilator (ipratropium bromide) and a weak recommendation for short-acting β2-agonist bronchodilator (salbutamol) and antitussive agent (dextromethorphan) for alleviating symptoms due to AECB. (3) We make a recommendation against mucolytic agents and theophylline as routine use. (4) More large-scale, multi-center, double-blinded RCTs are needed in clinical and pharmacoeconomic studies on AECB and outcome indicator should be improved in order to produce high-quality local evidence.
