  • Clinical Application of Pedicle Screw System in Thoracic and Lumbar Fractures of Patients for Wenchun Earthquake

    目的:探讨后路椎弓根螺钉固定在地震伤胸腰椎骨折中的应用及优点。方法:对19例胸腰椎骨折的地震伤患者行后路椎弓根螺钉内固定术。结果:本组病例的手术时间70~115分钟,平均出血量约280mL,两例病员术中出血超过400mL进行输血,复位椎体前缘高度由术前平均57.5%恢复到术后平均93.6%,后突角由术前平均21°矫正到术后平均3°,术后3~7天转往外地继续治疗,Frankel分级平均提高0.4。结论:后路椎弓根螺钉固定具有省时、节约医疗资源、提高救治效率、减轻患者痛苦的优点,尤其适用于大批伤病员的紧急救治。

  • APPLICATION OF ATLAS PEDICLE SCREW SYSTEM FIXATION AND FUSION FOR TREATMENT OF UPPER CERVICAL DISEASES

    Objective To explore the feasibilities, methods, outcomes and indications of atlas pedicle screw system fixation and fusion for the treatment of upper cervical diseases. Methods From October 2004 to January 2006, 17 patients with upper cervical diseases were treated with atlas pedicle screw system fixation and fusion. There were 13 males and 4 females, ageing 19 to 52 years. Of 17 cases, there were 14 cases of atlantoaxial dislocation(including 3 cases of congenital odontoid disconnection,4 cases of old odontoid fracture,2 cases of new odontoid fracture(typeⅡC), 3 cases of rupture of the transverse ligament, and 2 cases of atlas fracture; 2 cases of tumor of C2; 1case of giant neurilemoma of C2,3 with instability after the resection oftumors. JOA score before operation was 8.3±3.0. Results The mean operative time and bleeding amount were 2.7 hours (2.1-3.4 hours) and 490 ml (300-750 ml) respectively. No injuries to the vertebral artery and spinal cord were observed. The medial-superior cortex of lateral mass was penetrated by 1 C1 screw approximately 3 mmwithout affecting occipito-atlantal motions. All patients were followed up 3-18 months. The clinical symptoms were improved in some extents and the screws were verified to be in a proper position, no breakage or loosening of screw and rob occurred. All patients achieved a solid bone fusion after 3-6 months. JOA score 3 months after operation was14.6±2.2. JOA improvement rates were 73%-91%(mean 82%). Conclusion The atlas pedicle screw system fixation and fusion is feasible for the treatment of upper cervical diseases and has betteroutcomes, wider indications if conducted properly.

  • DESIGN AND EXPERIMENTAL STUDY OF INDIVIDUAL DRILL TEMPLATES FOR ATLANTOAXIAL PEDICLE SCREW FIXATION

    Objective To explore and evaluate the accuracy and feasibil ity of individual rapid prototype (RP) drill templates for atlantoaxial pedicle screw implantation. Methods Volumetric CT scanning was performed in 8 adult cadaveric atlas and axis to collect Dicom format datas. Then three-dimensional (3D) images of atlas and axis were reconstructed and the parameters of pedicles of 3D model were measured by using software Mimics 10.01. The 3D model was saved by STLformat in Mimics. The scattered point cloud data of 3D model were processed and the 3D coordinate system was located in software Imageware 12.1. The curves and surfaces of 3D model were processed in software Geomagic Studio 10. The optimal trajectory of pedicle screw was designed and a template was constructed which accorded with the anatomical morphology of posterior arch of atlas and lamina of axis by using software Pro/Engineer 4.0. The optimal trajectory of pedicle screw and the template were integrated into a drill template finally. The drill template and physical models of atlas and axis were manufactured by RP (3D print technology). The accuracy of pilot holes of drill templates was assessed by visually inspecting and CT scanning. Results The individual drill template was used conveniently and each template could closely fit the anatomical morphology of posterior arch of atlas and lamina of axis. Template loosening and shifting were not found in the process of screw implantation. Thirty-two pedicle screws were inserted. Imaging and visual inspection revealed that the majority of trajectories did not penetrate the pedicle cortex, only 1 cortical penetration was judged as noncritical and did not injury the adjacent spinal cord, nerve roots, and vertebral arteries. The accuracy of atlas pedicle screw was grade 0 in 15 screws and grade I in 1 screw, and the accuracy of axis pedicle screw was grade 0 in 16 screws. Conclusion The potential of individual drill templates to aid implantation of atlantoaxial pedicle screw is promising because of its high accuracy.

  • Comparison of effectiveness of cortical bone trajectory screw fixation and pedicle screw fixation in posterior lumbar interbody fusion

    Objective To compare the effectiveness of cortical bone trajectory screw (CBTS) and conventional pedicle screw for posterior lumbar interbody fusion (PLIF) in the treatment of single segment lumbar degenerative disease. Methods Between May 2013 and May 2016, a total of 97 patients with single segment lumbar degenerative disease were treated with PLIF. Fifty-one patients were fixed with CBTS in PLIF (trajectory screw group) and 46 with pedicle screw (pedicle screw group). There was no significant difference in age, gender, body mass index, preoperative diagnosis, lesion segment, and preoperative visual analogue scale (VAS) score, Oswestry dysfunction index (ODI) between 2 groups (P>0.05). The operation time, intraoperative blood loss, postoperative drainage, bed rest time, length of hospital stay, serum creatine kinase (CK) concentration, total amount of diclofenac sodium, perioperative complications, ODI, VAS score, and interbody fusion rate were recorded and compared between 2 groups. Results All patients were followed up 12 months. The patients in trajectory screw group had a significantly less operation time, intraoperative blood loss, postoperative drainage, and serum CK concentration when compared with the patients in pedicle screw group (P<0.05). Thirty-five patients (68.6%) in trajectory screw group and 46 patients (100%) in pedicle screw group were given diclofenac sodium within 48 hours after operation, showing significant difference between 2 groups (χ2=89.334, P=0.000). There was no significant difference in the incidence of perioperative complications between trajectory screw group and pedicle screw group (3.9% vs. 8.7%, P=0.418). There was no significant difference in the VAS score, ODI, and interbody fusion rate at 12 months after operation between 2 groups (P>0.05). Conclusion For the single segment degenerative lumbar disease, the use of CBTS or conventional pedicle screw for PLIF can obtain satisfactory clinical function and interbody fusion rate. But the former has the advantages of less blood loss, less intraoperative muscle damage, less perioperative pain, shorter length of hospital stay and bed rest time.

    Objective To elucidate the new development and effects of three-dimensional correction techniques of idiopathic scol iosis (IS). Methods The related home and abroad l iterature concerning three-dimensional correction techniques of IS was extensively reviewed. Results With more and more attention to three-dimensional correction of IS, all kinds of surgery and developed techniques of correction are applied to the correction of IS. The effects of three-dimensional correction of IS are satisfied. Conclusion With more knowledge about IS and more developed theory of correction, more safe and effective techniques of correction is therefore the hot spot for future study.

  • EFFECTIVENESS EVALUATION OF PERCUTANEOUS MONOAXIAL SCREW COMBINED WITH INJURED VERTEBRAE POLYAXIAL PEDICAL SCREW FIXATION FOR TREATMENT OF THORACOLUMBAR FRACTURES

    ObjectiveTo explore the effectiveness percutaneous monoaxial screw combined with polyaxial pedical screw for treating thoracolumbar fracture by comparing with simple polyaxial pedicle screw fixation. MethodsBetween January 2012 and June 2014, 56 cases of thoracolumbar fractures were treated by percutaneous pedicle screw fixation, the clinical data were retrospectively analyzed. Of 56 cases, 30 were treated with percutaneous monoaxial screw combined with percutaneous polyaxial pedical screw fixation (group A), 26 patients with only percutaneous polyaxial pedicle screw fixation (group B). There was no significant difference in gender, age, body mass index, injury causes, time from injury to admission, involved segments, fracture type, and preoperative American Spinal Injury Association (ASIA) stage, visual analogue scale (VAS), the anterior height of the injured vertebrae, Cobb angle, and sagittal index between 2 groups (P>0.05). The operation time, intraoperative blood loss, and complications were recorded and compared between the 2 groups. The VAS score was used to evaluate the improvement of the pain. The sagittal kyphosis Cobb angle, the anterior height of the injured vertebrae, sagittal index, and the average correction (difference between 3 days after oeration and preoperation) and loss degrees (difference between last follow-up and 3 days after operation) were measured on the X-ray films at preoperation, 3 days after operation, and last follow-up. ResultsIncision healing at stage I was obtained, no related complications occurred. The operation time and intraoperative blood loss showed no significant difference between 2 groups (P>0.05). The patients were followed up 20-42 months (mean, 32 months) in group A and 21-44 months (mean, 30 months) in group B. VAS score of group A was significantly lower than that of group B at 3 days after operation (t=-2.277, P=0.027), but no significant difference was found at last follow-up (t=-0.289, P=0.774). X-ray examination showed good position of internal fixation, with no broken nails or exit of nail. There were significant differences in the anterior height of the injured vertebrae, Cobb angle, and sagittal index between at preoperation and at 3 days and last follow-up, and between at 3 days and last follow-up in 2 groups (P<0.05). The anterior height of the injured vertebrae, Cobb angle, and sagittal index of group A were significantly better than those of group B at 3 days and last follow-up (P<0.05), and correction degree were significantly higher than those of group B (P<0.05), but loss degree was not significant between 2 groups (P>0.05). ConclusionPercutaneous monoaxial screw combined with polyaxial pedicle screw fixation is better than simply polyaxial pedicle screw in effects of treating thoracolumbar fracture under the premise of strictly holding indications.

  • SECOND MEASUREMENT OF THREE-DIMENSIONAL CT RECONSTRUCTION DATA FOR SCOLIOSIS ORTHOPAEDIC SURGERY

    【Abstract】 Objective To explore the clinical application and outcomes of preoperative second measurement of three-dimensional (3-D) CT reconstruction data for scoliosis orthopedic surgery. Methods Between August 2006 and March 2008, 11 patients with severe rigid scoliosis received surgery treatment, including 4 males and 7 females with an average age of 17.2 years (range, 15-19 years). Preoperative second measurement of 3-D CT reconstruction data was conducted to guide the surgery, including the angle and width of pedicle, the entry point, and the choice of screws whose lengths and diameters were suitable. A total of 197 pedicle screws were implanted. The operation time, blood loss, postoperative nerve function,and Cobb’s angles at sagittal and coronal view were all recorded, and the postoperative CT scan was performed to assess the accuracy of pedicle screw insertion according to Andrew classification. Results Pedicle screws were implanted within 1-11 minutes (mean, 5.8 minutes), and the blood loss was 450-2 300 mL (mean, 1 520 mL). The postoperative X-ray films showed the correction rates of Cobb’s angle were 68.5% in coronal view and 55.5% in sagittal view. The accuracy of pedicle screw insertion was rated as grade I in 77 screws (39.1%),grade II in 116 screws (58.9%), and grade III in 4 screws (2.0%) according to postoperative CT scan. All 11 cases were followed up 14 months to 2 years without any complications. Conclusion Preoperative second measurement of 3-D CT reconstruction data can make the surgery process easy and accurate in treatment of severe scoliosis.

  • Application of 3D printed lumbar puncture models in orthopedic clinical teaching

    ObjectiveTo explore the feasibility of lumbar puncture models based on 3D printing technology for training junior orthopaedic surgeons to find the optimal pedicle screw insertion points.MethodsMimics software was used to design 3D models of lumbar spine with the optimal channels and alternative channels. Then, the printed lumbar spine models, plasticine, and cloth were used to build lumbar puncture models. From January 2018 to June 2019, 43 orthopedic trainees performed simulated operations to search for the insertion points of pedicle screws base on the models. The operations were performed once a day for 10 consecutive days, and the differences in operation scores and operation durations of the trainees among the 10 days were compared.ResultsAll the trainees completed the surgical training operations successfully, and there were significant differences in the operation scores (13.05±2.45, 14.02±3.96, 17.58±3.46, 21.02±2.04, 23.40±4.08, 25.14±3.72, 27.26±6.09, 33.37±4.23, 35.00±4.15, 38.49±1.70; F=340.604, P<0.001) and operation durations [(22.51±4.28), (19.93±4.28), (18.05±2.89), (17.05±1.76), (16.98±1.97), (15.47±1.74), (13.51±1.42), (12.60±2.17), (12.44±1.71), (11.91±1.87) minutes; F=102.359, P<0.001] among the 10 days.ConclusionThe 3D models of lumbar puncture are feasible and repeatable, which can contribute to surgical training.

  • Treatment for Thoracolumbar Kyphosis by Whole Posterior Spinal Osteotomy Combined with Pedicle Screw

    【摘要】 目的 探讨经后路全脊截骨联合椎弓根螺钉治疗胸腰椎脊柱后凸畸形的手术评估和临床疗效。 方法 2004年4月-2010年6月采用后路脊椎截骨椎弓根螺钉内固定治疗脊柱后凸畸形15例,其中男11例,女4例;年龄16~61岁,平均49岁。陈旧性胸腰椎骨折后凸10例,结核后凸3例,椎体发育不良2例;后凸顶点:胸9椎体1例,胸11椎体3例,胸12椎体4例,腰1椎体5例,腰2椎体2例。脊柱后凸Cobb角35~61°,平均46°。Frankel分级:E级2例,D级8例,C级4例,B级1例。 结果 术中出血800~1 800 mL,平均1 000 mL;术中1例左侧胸神经根损伤;1例术后双下肢肌力感觉减退。15例患者均获随访,随访时间10~30个月,平均12个月。术后6个月Cobb角5~10°,平均矫正率86.5%。术后6~10个月X线片显示截骨平面骨性愈合,术后神经功能恢复情况,除1例B级恢复至D级外,其余为E级。内固定物无松动、断裂和纠正度数丢失等并发症。 结论 经后路脊柱截骨联合椎弓根螺钉内固定具有减压、矫形同时进行,矫正度数大,并发症少,临床效果明显。【Abstract】 Objective To explore the surgery assessment and clinical outcome of the treatment for thoracolumbar kyphosis by whole posterior spinal osteotomy combined with pedicle screw. Methods Fifteen patients including 11 males and 4 females with kyphosis were treated by posterior spinal osteotomy combined with pedicle screw from April 2004 to June 2010. The age of them ranged from 16 to 61 years old averaging at 49. There were 10 cases of old thoracolumbar fracture kyphosis, 3 cases of tuberculosis kyphosis, and 2 cases of poor vertebral growth. As for kyphosis vertices, there were 1 case of T9, 3 cases of T11, 4 cases of T12, 5 cases of L1, and 2 cases of L2. Kyphosis Cobb angle ranged from 35° to 61°, averaging at 46°. Based on Franke Grade, there were 2 grade E cases, 8 grade D cases, 4 grade C cases, and 1 grade B case. Results Intraoperative blood loss was from 800 to 1 800 mL with an average of 1 000 mL; There was 1 case of left thoracic nerve root injury during operation and 1 case of lower extremity muscle strength hypoesthesia after operation. All the 15 patients were followed up for 10 to 30 months with an average time of 12 months. Six months after surgery, Cobb angle ranged from 5° to 10°, with an average correction rate of 86.5%. Six to ten months after osteotomy, X ray showed a good bone healing condition. As for the recovery status of neurological function after surgery, All patients recovered to grade E except 1 patient who returned to grade D from grade B. No such complications as fixation without loosening, fracture or loss of correction degree occurred. Conclusion In posterior spinal osteotomy combined with pedicle screw fixation, decompression and correction can be carried out at the same time to correct a large degree of kyphosis with few complications. The clinical effect is obvious.

  • CLASSIFICATION OF ATLAS PEDICLES AND METHODOLOGICAL STUDY OF PEDICLE SCREW FIXATION

    Objective To investigate the classification of atlas pedicles and the methods of the pedicle screw fixation. Methods To study the classification of atlas pedicles, 48 dry adult atlas specimens were measured. By atlas 3D-CT reconstruction, two transverse sections were establ ished by going through the one third of the lateral atlas pedicle and 2 mmbelow the vertebral artery sulcus. By setting 3.50 mm and 1.75 mm as the standardized diameter and radius for the screwand according to the thickness of bone substance of vertebral artery sulcus that went through the one third of the lateralatlas pedicle, the anatomical morphology of atlas pedicles were classified into three types: general type with 40 specimens (83%), l ight variation type with 6 specimens (13%), and severe variation type with 2 specimens (4%). The entry pathway was confirmed by the intersection l ine of the two transverse sections that went through the lateral one third of the atlas pedicle and 2 mm below the vertebral artery sulcus. The project-point of the entry pathway on the atlas posterior arch was considered to be the entry point. Forty-eight dry atlas specimens were used to measure the following relevant anatomic data with an electronic cal iper: the distance between the entry point and the posterior margin of the lateral mass (L1), the height of atlas pedicle at the entry point (L2), the vertical distance between the entry point and the inferior articular facet of the lateral mass (L3), the mass height at the entry point (L4), the mass width at the entry point (L5), the width of the atlas pedicle at the entry point (L6), the thickness of the pedicle under the vertebral artery sulcus at the entry pathway (H1). To research the method of the pedicle screw fixation, 12 fresh-frozen adult atlas specimens were adopted to simulate the fixation of the pedicle screw. The thickness of the bone substance of vertebral artery sulcus on both the left and the right sides of the pathway was grinded into 3 types: 1.5 mm and 2.5 mm, 1.5 mm and 4.0 mm, 2.5 mm and 4.0 mm, and each type had four specimens. The entry pathway was confirmed by the intersection l ine of two transverse sections that went through the lateral one third of atlas pedicle and 2 mm below the vertebral artery sulcus. Results On the left side, L1 was (5.79 ± 1.24) mm, L2 (4.55 ± 1.29) mm, L3 (5.12 ± 1.06) mm, L4 (12.43 ± 1.01) mm, L5 (12.66 ± 1.37) mm, L6 (7.86 ± 0.77) mm, and H1 (4.11 ± 1.25) mm. On the right side, L1 was (5.81 ± 1.26) mm, L2 (4.49 ± 1.22) mm, L3 (5.15 ± 1.05) mm, L4 (12.49 ± 0.98) mm, L5 (12.65 ± 1.38) mm, L6 (7.84 ± 0.78) mm, and H1 (4.13 ± 1.29) mm. There was no significant difference between the two sides (P gt; 0.05). After simulation of inserting screws, no screw in the specimens was found to break the bone substance in the sulcus of vertebral artery. Conclusion For the pedicle screw fixation of those patients whose atlas posterior arches are not high enough, we might partly drill through or beyond the atlas posterior arch. The entry point should be ascertained by preoperative 3D-CT reconstruction and intra-operative exploration.
