  • The effects of intervention with Tanakan on anterior ocular segment in diabetic retinopathy after retinal photocoagulation

    Purpose To investigate the effects of intervention with Tanakan on anterior ocular segment in diabetic retinopathy (DR) after retinal photocoagulation. Methods Prospective random controlled study was performed on 72 patients (72 eyes) with ultrasound biomicroscopy (UBM),by obtaining and quantitatively analyzing the changes of anterior ocular segment including anterior chamber, anterior chamber angle, ciliary body and choroids before and the 3rd day and the 7th day after retinal photocoagulation. Results Three days after photocoagulation, significant elev ated IOP and narrowed chamber angle were observed in control group and 4 eyes (1 1.11%) in Tanakan group (Plt;0.01). Choroidal detachment in 32 eyes (88.89%) in control group and in 2 eyes (5.56%) in Tanakan group and the severity of ciliochoroidal detachment in tanakan group was significantly lower than that in control group. Conclusion Tanakan is effective to prevent the complications of anterior segment, such as ciliochoroidal detachment, elevation of IOP, narrowing of chamber angle occurring early after retinal photocoagulation and reduce the severity of ciliochoroidal detachment. (Chin J Ocul Fundus Dis, 2001,17:187-189)

  • 251例玻璃体切除术后激光治疗的临床分析

    目的:探讨玻璃体切除手术后激光治疗的临床规律。 方法;回顾分析1990~1996年在我院施行玻璃体切除手术后接受激光治疗患者的临床贤料.结果:251例患者255只眼接受激光治疗,占同期玻璃体切除手术患者的22.9%.其中因眼 底血管性疾病需要针对原发病补充激光治疗者占57.3%;因视网膜裂孔、可疑裂孔或高度危险的视网膜变性等原因术后需要激光治疗者占37.6%;需施行YAG激光虹膜造孔术的患者约占4.3%。全部激光治疗患者中术后视力提高者209只眼,不变者34只眼,下降者12只眼。 结论:在开展玻璃体视网膜手术的同时应该配备可靠的激光治疗手段和技术.激光治疗可提高视网膜复位手术的成功率,减少和防治血管性疾病再出血的危险性。 (中华眼底病杂志,1997,13:202-203)

  • Early influence of laser photocoagulation on macular retinal thickness in diabetic retinopathy

    Objective To investigate the early influences of laser photocoagulation on macular retinal thickness in diabetic retinopathy(DR). Methods Optic coherence tomography examination was performed in 30 eyes with DR(phase Ⅲ~Ⅳ) before, and on the 3rd day and the 7th day after photocoagulation respectively. The thickness of neuroretina and pigment epithelium were measured in the areas of fovea macula and 750 μm from fovea macula. Results Three days after photocoagulation, significant thickening of neuroretina was observed in the fovea macula, which is positively related with age, fasting blood sugar and duration of DR. There was no significant changes in the thickness of pigment epithelium in macula and in the thickness of neuroretina 750 μm from fovea macula. Conclusion Significant thickening of neuroretina in fovea macula in DR early after photocoagulation reveals progressed macular edema induced by photocoagulation which is positively related with age, fasting blood sugar and duration of DR.   • Quantitative investigation of early influences of laser photocoagulation on retinal function in diabetic retinopathy

    Objective To investigate the early influences of laser photocoagulation on retinal function in diabetic retinopathy(DR). Methods The multifocal electroretinograms (MERG) of 30 eyes with DR (phase Ⅲ~Ⅳ) were tested with visual evoked response image system IV b efore,and the 3rd day and the 7th day after laser photocoagulation. Results Three days after photocoagulation, the latency of N1 prolonged in the central macula 5deg; area and superionasal quadrant.Th e response densities of N1,P1 and N2 markedly reduced, and most significant changes occurred in the central macula 5deg; area and then in the central 10deg;area. There were also differences in the changes of the amplitude of N1 and P1 in diff erent quadrants .The changes of visual acuity were positively related to the de crease of amplitudes of N1,P1 and N2 in the macula. Conclusion The reduction of response densities in MERG reveals functional damage in diabetic retina occurring early after photocoagulation.The functional damage in macula induced indirectly by photocoagulation may explain the reduction of visual acuity after panretinal photocoagulation in some degree.   • Krypton laser photocoagulation of macular branch retinal vein occlusion

    Objective The present study focus on the macular branch retinal vein occlusion (MBRVO) with and without laser treatment,and evaluate prospectively whether laser treatment is useful in improving the visual acuity and reducing the macular edema. At the same time to learn the difference of macular light sensitivity before and after laser treatment.Methods Forty-five eyes of forty-five patients with MBRVO and macular edema were randomized to laser treatment and no laser treatment. Follow up examinations were performed every 3 months. We compared the difference of visual improvement, resolution of macular edema between the two groups. The central 30°visual field of 20 patients with MBRVO were examined at pretreatment and 3 months after laser treatment. A comparison of light sensitivity at fovea ,central 10°and 11~30°eccentricity were done between before and after laser treatment. Results Comparison of visual improvement and resolution of macular edema showed a statistical difference between laser treatment and no laser treatment. The mean reduction of macular light sensitivity at the fovea and 11~30°3 months after the treatment has no statistical difference between before and after treatment (Pgt;0.05). But the mean light sensitivity at central 10°eccentricity were significantly decreased 3 months after photocoagulation (Plt;0.05).Conclusion The laser treatment might promote or accelerate visual acuity recovery and reduce the macular edema. There is no significant difference about the efficiency on macular function after laser treatment.(Chin J Ocul Fundus Dis,2003,19:201-268)

  • ARGON LASER PHOTOCOAGULATION FOR CHOROIDAL OSTEOMA

    PURPOSE:To evaluate the therapeutic effect of argon laser photocoagulation on choroidal osteoma. METHODS:Six cases (7 eyes)of choroidal osleoma were treated with argon laser photoeoagulation. The pre and postoperative visual aeuity,fundus appearance and fundus fluorescein anglography(FFA)were observed. The average period of follow up was 23 months. RESULT:The visual acuity of all patients was unchanged. Choroidal osteomas in 3 patlents ( 3 eyes)were flattened. The EFA showed that choroidal vaseulature disappeared in one eye. CONCLUSION :Multiple sessions of photocoagulation may cause decalcification of choroidal ostema and ]imitation of growth of the tumors awing to destroying the choroid blood vessel by pholoeoagulation in the area of tumor and around it.   • Laser photocoagulation or endolaser combined with vitrectomy for congenital optic disc pit maculopathy: “dam sign” of optical coherence tomography is a sign to indicate the anatomic success

    ObjectiveTo investigate the successful signs of laser photocoagulation or endolaser combined vitrectomy for congenital optic disc pit (ODP) with serous macular detachment.MethodsA retrospective case analysis. Twelve eyes of 12 patients with congenital ODP complicated with serous macular detachment diagnosed in Zhongshan Ophthalmic Center from 2003 to 2018 were included in this study. There were 2 males (2 eyes) and 8 females (8 eyes). The average age was 30.17 years old. Retinal laser photocoagulation and/or vitrectomy were performed in all the eyes. The optic disc and macular area were scanned using the tracking mode of the Germany Heidelberg Spectralis OCT instrument. The " dam-sign” change was a retinal choroidal scar on the channel between the ODP and the macula on the OCT image after treatment. The eyes were divided into a " dam-sign” change group (group A, 10 eyes) and no " dam-sign” change group (group B, 2 eyes). The BCVA of eyes in group A was 0.03-0.6. In group A, 1 eye was treated with laser photocoagulation alone, 9 eyes were treated with vitrectomy combined with laser photocoagulation. The BCVA of eyes in group B was 0.1. All the eyes in group B underwent vitrectomy combined with laser photocoagulation. The follow-up was ranged from 6 to 174 months. The same equipment and method before treatment were used for 1, 3, 6, 12, 18, and 24 months after treatment. The BCVA, absorption of subretinal fluid (SRF) and the formation of " dam-sign” change were observed.ResultsAt the last follow-up, the BCVA of eyes in group A was 0.4-1.0 (0.4 in 1 eye, 0.5 in 3 eyes, 0.6 in 2 eyes, 0.8 in 3 eyes, 1.0 in 1 eye). In 9 eyes treated with vitrectomy combined with laser photocoagulation, SRF was completely absorbed. In 1 eye treated with laser photocoagulation alone, SRF remained in small amount. The BCVA of the two eyes in group B was 0.03 and 0.3, respectively; and SRF was not absorbed in both of them.ConclusionsThe " dam-sign” change near optic disc after laser photocoagulation can promote SRF absorption and improve BCVA. It can be used as a success indicator after treatment.
