  • Femoral Neck Fracture Treated with Close Reduction plus Cannulated Screw Fixation with Quadratus Femoris Muscle Pedicle Bone Grafting

    目的 目前对青壮年有移位新鲜股骨颈骨折患者的治疗仍存在许多争议,被认为尚未完全解决难题之一,股骨颈骨折内固定术后常导致骨折不愈合股骨头缺血性坏死。 方法 2003年1月-2008年6月,收治青壮年新鲜移位股骨颈骨折患者12例,年龄17~55岁,平均36.3 岁。所有患者在受伤后1周内采用牵引下闭合复位空心钉内固定加股方肌骨瓣移植术治疗。术后随访1~5年,平均3.5年。 结果 12例患者均达骨性愈合,随访期内未出现股骨头缺血性坏死;术后采用Harris 髋关节评分标准进行评估,获优良10例,一般1例,差1例。 结论 青壮年新鲜有移位股骨颈骨折患者,采用牵引下闭合复位空心钉内固定加股方肌骨瓣转移术治疗效果良好,均能达到骨折愈合,且无股骨头缺血性坏死发生。

  • TREATMENT OF FRACTURE OF NECK OF FEMUR IN THE AGED WITH HOLLOW COMPRESSION SCREW

    OBJECTIVE To summarize the clinical results of hollow compression screw in treatment of fracture of neck of femur in the aged. METHODS: From November 1993 to October, 1998, 52 aged patients with several types of fracture of neck of femur were treated; among them, there were 25 males and 27 females aged from 60 to 83 years (70 years on average). There were 48 cases of fresh fracture and 4 cases of old fracture. Forty-two cases were performed closed reduction internal fixation, 10 cases with open reduction internal fixation. RESULTS: All the patients could sit by themselves 4 to 6 days after operation, and could walk with a crutch 10 to 15 days after operation. The mobility of hip joint was nearly normal 4 weeks after operation. All the patients were followed 26 to 84 months, 48.5 months on average. Bone union was achieved in 50 cases, nonunion in 2 cases. The average healing time was 4.7 months. There were no other complications, such as ankylosis and muscular atrophy, but ischemic necrosis in 3 cases. CONCLUSION: This method has following advantages, convenient manipulation, less injury, stable fixation, and the short-term recovery, which avoid some common complications. It is a reliable method worthy of popularizing.

  • The safety and efficacy of sequential use of tranexamic acid in perioperative period of half hip replacement for femoral neck fracture in the elderly

    ObjectiveTo investigate the safety and efficacy of sequential use of tranexamic acid (TXA) in the perioperative period of half hip replacement for femoral neck fracture in the elderly.MethodsPatients who underwent hemiarthroplasty due to femoral neck fracture in Meishan Traditional Chinese Medicine Hospital from January 2016 to October 2018 were retrospectively included. According to the use of TXA during the perioperative period, the patients were divided into group A (no use of TXA), group B (TXA use on the day of surgery), and group C (sequential therapy). The perioperative blood loss, blood transfusion rate and incidence of deep vein thrombosis (DVT) in the 3 groups were recorded and counted.ResultsA total of 113 patients were included, and 99 patients were finally included (31 in group A, 36 in group B, and 32 in group C). There were no statistically significant difference in age, sex, body mass index, hemoglobin (Hb) at admission, hematocrit at admission, blood volume, or blood transfusion among the three groups. The differences in total blood loss (F=43.613, P<0.001), preoperative blood loss (F=4.746, P=0.011), preoperative Hb (F=6.220, P=0.003), maximum Hb change (F=18.913, P<0.001), and postoperative length of hospital stay (F=43.511, P<0.001) among the three groups were statistically significant. There was no DVT of the lower extremities or pulmonary embolism found in the three groups. The differences in preoperative blood loss and preoperative Hb were not statistically significant between group A and group B. The difference in postoperative length of hospital stay was not statistically significant between group B and group C. The differences in other indexes between all the pairs of the three groups were statistically significant (P<0.05).ConclusionUsing TXA early after femoral neck fracture and sequentially to the day of surgery can reduce perioperative blood loss, including hidden blood loss before surgery and blood loss during and after the operation, which is beneficial to patients’ postoperative recovery without increasing the risk of DVT.

  • 带旋髂深血管蒂髂骨瓣联合空心加压螺钉内固定治疗青壮年股骨颈骨折的疗效分析

    目的 总结带旋髂深血管蒂髂骨瓣联合松质骨空心加压螺钉内固定治疗青壮年股骨颈骨折的疗效。 方法 2006 年1 月- 2008 年12 月,采用带旋髂深血管蒂髂骨瓣联合松质骨空心加压螺钉内固定治疗23 例外伤致股骨颈骨折患者。其中男17 例,女6 例;年龄19 ~ 47 岁,平均31.4 岁。骨折按Garden 分型:Ⅱ型3 例,Ⅲ型14 例,Ⅳ型6 例。受伤至手术时间1 ~ 9 d,平均5 d。 结果 术后患者切口均Ⅰ期愈合。23 例均获随访,随访时间13 ~ 36个月,平均15.3 个月。骨折愈合时间9 ~ 15 个月。术后15 个月Harris 评分为73 ~ 92 分,其中优5 例,良16 例,中2 例,优良率91.3%。1 例术后33 个月发现股骨头缺血性坏死(avascular necrosis of femoral head,ANFH),其余患者无ANFH及股骨头晚期塌陷等并发症发生。所有患者无神经损害并发症,无术区感染;1 例术后伴供骨区疼痛,12 个月后疼痛自行消失。 结论 带旋髂深血管蒂髂骨瓣联合松质骨空心加压螺钉内固定治疗青壮年股骨颈骨折手术操作简便、安全,可获得较好疗效。

  • Comparison of clinical effects of traditional manual and robot-assisted screw placement in the treatment of femoral neck fracture

    Objective To compare the clinical effects of traditional manual and robot-assisted implantation of cannulated screws in the treatment of femoral neck fracture. Methods The medical records of patients with femoral neck fracture in Department of Orthopaedics, People’s Hospital of Deyang City were analyzed retrospectively. The patients were divided into two groups, including the traditional manual implantation group from January to December 2018 and the robot-assisted implantation group from May 2019 to May 2020. The clinical therapeutic efficacy of the two groups were compared. Harris hip function score was used to evaluate hip function. Results A total of 85 patients were included. All patients had closed fractures. There were 45 cases in the traditional manual implantation group and 40 cases in the robot-assisted implantation group. There was no significant difference between the two groups in preoperative waiting time, operation time, hospitalization time, fracture healing time, complications within one year after operation, or Harris hip function score one year after operation (P>0.05). The placement nail time [(11.1±2.0) vs. (23.8±2.3) min; t=27.142, P<0.001], frequency of guide pin insertion [(4.7±1.2) vs. (11.4±1.7) times; t=20.640, P<0.001], frequency of intraoperative fluoroscopy [(10.8±1.7) vs. (21.0±1.8) times; t=26.990, P<0.001] and intraoperative blood loss [(8.1±2.0) vs. (12.0±1.7) mL; t=9.711, P<0.001] in the robot-assisted implantation group were less than those in the traditional manual implantation group. No wound infection or neurovascular injury was found in the two groups. Conclusion Robot-assisted implantation of cannulated screws in the treatment of femoral neck fracture has the advantages of less fluoroscopy, fewer guide pin insertion, less blood loss, more accurate screw placement than the traditional manual implantation of cannulated screws.

  • Treatment of unstable femoral neck fracture with posteromedial comminutations by cannulated screws and medial bracing plate combined with bone allograft

    Objective To evaluate the effectiveness of unstable femoral neck fracture with posteromedial comminutations treated by cannulated screws and medial bracing plate combined with bone allograft. MethodsThe clinical data of 18 patients with unstable femoral neck fracture with posteromedial comminutations treated by cannulated screws and medial bracing plate combined with bone allograft between July 2016 and March 2020 were retrospectively analyzed. The age ranged from 22 to 64 years, with a median age of 43 years. The causes of injury included 11 cases of falling injury, 3 cases of traffic accident injury, and 4 cases of fall from height injury. According to Garden classification, the femoral neck fracture was classified as type Ⅲ in 3 cases, type Ⅳ in 15 cases, and all patients were type Ⅲ according to Pauwels classification. The time from injury to operation was 1-5 days, with an average of 2.3 days. The fracture healing time and complications were recorded. The quality of fracture reduction was evaluated by Garden index immediately after operation; at last follow-up, the degree of femoral neck shortening was determined by Zlowodzki method, Harris score was used to evaluate hip function. Results The operation time was 62-98 minutes (mean, 75 minutes); intraoperative blood loss was 101-220 mL (mean, 153 mL). Cannulated screws guide wire was inserted 3-5 times (mean, 4 times). Intraoperative fluoroscopy was performed 9-21 times (mean, 15 times). The hospital stay was 5-11 days (mean, 7.2 days). All the patients were followed up 12-40 months with an average of 17.3 months. There was no postoperative complication such as accumulated pneumonia, lower extremity deep venous thrombosis, nail cutting, nail withdrawal, internal fixation fracture, and so on. There was no fracture nonunion and osteonecrosis of the femoral head during the follow-up; the fracture healing time was 7-15 weeks, with an average of 12.1 weeks. The quality of fracture reduction was evaluated immediately after operation, the results were grade Ⅰ in 15 cases and grade Ⅱ in 3 cases. At last follow-up, there were 2 cases with femoral neck shortening less than 5 mm and 1 case with 5-10 mm. The incidence of femoral neck shortening was 16.7%. The Harris score of hip joint was 73-97, with an average of 93.5; among them, 12 cases were rated as excellent, 3 cases as good, and 3 cases as fair, with an excellent and good rate of 83.3%. ConclusionFor the treatment of unstable femoral neck fracture with posteromedial comminutations, cannulated screws and medial bracing plate combined with bone allograft are dramatically effective due to earlier weight bearing, faster fracture healing, and better hip function recovery.

  • TREATMENT OF FRACTURE OF FEMORAL NECK BY TRANSPLANTATION OF ILIAC BONE GRAFT PEDICIED WITH DEEP CIRCUMFLEX ILIAC VESSEL

    Thirty-nine cases of fracturc of the femoral neck were treated by transplantation of iliac bone graft pedicled with the deep circumflex iliac vessel. The average period of the follow up was 89.4 months. The results showed that Harris Hip Functional Evaluation was 88.2±15.6.   • FREE VASCULARIZED FIBULAR ASSOCIATED WITH ILIAC GRAFT TO TREAT OLD FEMORAL NECK FRACTURE

    【Abstract】 Objective To evaluate the cl inical outcome of free vascularized fibular associated with il iac graft intreatment of old femoral neck fracture. Methods From January 1994 to January 1997, 76 cases of old femoral neck fracture were treated with free vascularized fibular associated with il iac graft, including 54 males and 22 females, aging from 24 to 48 years with an average of 31.5 years. All of these fractures resulted from injury. There were 20 cases of Garden II, 41 of Garden III and 15 of Garden IV. Based on the location of fracture, there was 26 cases of subcapital, 42 cases of transcervical and 8 cases of basal. Sixty-five cases were treated with internal fixation, 7 cases with skin traction and 4 just with staying in bed. Their Harris score were from 52 to 72 with an average of 65.6. The time from injury to operation was 2-24 months. The size of free vascularized fibular was 6-8 cm and il iac graft was 3.0 cm×2.0 cm×1.5 cm. Results In 76 cases, 68 were followed up and all fractures healed within 4 to 6 months with an average of 5.2 months. The increased density in femoral head was observed 1 year after operation. After 10 years of operation, normal hip function was achieved in 63 cases(followed up 10.1 -12.4 years with theaverage as 10.5 years); the Harris score was 87.5 (84 to 94). The structure of femoral head was normal and the grafted fibular and il iac bone healed with the femoral, no elapse or cyst occurred. Five cases had been compl ied with total hip replacement for femoral head necrosis or other. Conclusion Free vascularized fibular associated with il iac graft is a good method to treat old femoral neck fracture.

  • TREATMENT OF FEMORAL NECK FRACTURE WITH CANNULATED SCREW FIXATION IN YOUNG ADULTS

    Objective To evaluate the clinical effect of cannulatedscrew on treatment of femoral neck fracture(FNF). Methods Forty-two FNFpatients were treated by using cannulated screw from January 2001 to December 2005.There were 22males and 20 females with an average age of 41 years (19-59 years). Fracture was caused by traffic accident in 21 cases, by falling from height in 14 cases and by bruise in 7 cases. All cases were fresh fracture. According to Garden criterion for typing, 15 cases were classified as type Ⅱ, 16 cases as type Ⅲ and 11 cases as type Ⅳ . It was 7 hours to 15 days from injury to operation. Results Thepatients were followed up for 1-6 years with an average of 2.5 years. The average fracture union time was 6.5 months. Three patients had ischemic necrosis of femoral head, andloosening and breakage of screw and rob was observed in 1 case. According to Brumback criterion for hip joint function, the result was excellent in 18 cases, good in 20 cases and bad in 4 cases, and the excellent and good rate was 90.4%. Conclusion Cannulated screw fixation is a good method to treat FNF in young adults. It can improve the rate of fracture union and reduce the rate of avascular necrosis of femoral head.
