  • 电视胸腔镜下胸腺切除术治疗重症肌无力

    目的探讨电视胸腔镜手术(VATS)下行胸腺切除治疗重症肌无力的可行性和治疗效果。方法19例重症肌无力患者,按照Osserman临床分型标准,Ⅰ型10例,Ⅱa型5例,Ⅱb型3例,Ⅲ型1例,均在VATS下行胸腺扩大切除术。结果19例患者均顺利完成手术,无手术死亡,平均手术时间120min,术中出血量均小于100ml,术后发生重症肌无力危象3例,经及时治疗治愈。术后对所有患者均进行了随访,随访时间〉6个月,重症肌无力病情完全缓解9例,好转6例,无变化4例,总有效率79%(15/19)。结论在VATS下行胸腺扩大切除术治疗重症肌无力是可行的,且创伤小、疼痛轻,以期望减少术后并发症。

  • Clinical application of subxiphoid uni-portal thoracoscopic thymectomy: A propensity score matching study

    ObjectiveTo investigate the safety, feasibility and advantages of subxiphoid uni-portal thoracoscopic thymectomy.MethodsClinical data of 65 patients undergoing subxiphoid uni-portal thoracoscopic thymectomy in our hospital from September 2018 to March 2019 were retrospectively analyzed. They were treated as a subxiphoid surgery group, including 36 males and 29 females, aged 49.5 (29-71) years. The incision with the length of about 3 cm was located approximately 1 cm under the xiphoid process. From January 2016 to December 2017, 65 patients received intercostal uni-portal thoracoscopic thymectomy, who were treated as a control group, including 38 males and 27 females, aged 48.9 (33-67) years. All patients who were clinically diagnosed with thymic tumor before surgery were treated with total thymectomy. After surgery, expectoration and analgesia were used.ResultsThere was no statistically significant difference in general clinical data, lesion size, intraoperative blood loss, postoperative catheterization time, postoperative hospital stay and postoperative pathology between the two groups. All operations were successfully completed, and the patients in both groups recovered uneventfully after surgery. Visual analogue scale scores on the 1st, 3rd, 7th and 30th day after surgery in the subxiphoid surgery group were lower than those in the control group.ConclusionThe subxiphoid uni-portal thoracoscopic approach can achieve total thymectomy with less trauma and faster postoperative recovery.

  • 小儿重症肌无力的外科治疗

    目的 探讨小儿重症肌无力(MG)的手术适应证,围手术期处理及影响疗效的有关因素.方法 按Osserman临床分型分为Ⅰ型(单纯眼肌型)11例,ⅡA型(轻度全身型)6例,ⅡB型(中度全身型)2例.胸腺病理检查:增生12例,正常组织学表现7例,无合并胸腺瘤者.12例胸腺标本进行了免疫组织化学观察.结果 全组无手术死亡.术后发生肌无力危象1例,总有效率89.5%.全身型术后缓解率75%,较单纯眼肌型36.3%高.常规病理分型与疗效无关,根据免疫组织化学观察,12例胸腺分为上皮细胞密集型6例和上皮细胞松散型6例,两者术后疗效相差显著.结论 小儿全身型MG应行手术治疗,而对单纯眼肌型MG的手术适应证应从严掌握,对学龄前儿童尽可能采用药物治疗.使用人工呼吸器是治疗术后肌无力危象的有效措施.胸腺免疫组织化学分型能较好地评估手术后的疗效.

  • Technical points of modular operation and standard procedure for three-port anterior mediastinal thymic disease surgery via subxiphoid approach: Experience of Tangdu Hospital

    Surgery is an important treatment for the anterior mediastinal disease. With the rapid development of minimally invasive techniques, complete resection of the lesion in most patients with thymic disease can be achieved through thoracoscopic surgery. Practice has proved that the three-port resection of anterior mediastinal thymus disease via the subxiphoid approach is an ideal surgical method for the treatment of anterior mediastinal thymic tumors at present, which has strong popularization and popularity and can benefit the patients. The procedure focuses primarily on the anterior and upper mediastinum and can thoroughly expose the anatomy of the mediastinum and both sides, with minimal intraoperative bleeding, high safety, minimal trauma and postoperative pain, and a short hospital stay. It has clear advantages over conventional thoracic open-heart surgery and transversal resection. However, the surgical approach and field of view, and intraoperative precautions of this procedure are completely different from those of previous thoracoscopic procedures, and from the subxiphoid single-port approach adopted by other centers. Based on 10 years of surgical experience at our center, a modular mode of surgical operation has been developed and its procedure has been standardized. This paper will share and discuss relevant operational points and experiences.

  • Perioperative safety of thymectomy in myasthenia gravis patients with oral high-dose glucocorticoids

    ObjectiveTo investigate the perioperative safety of patients with myasthenia gravis who take high doses of oral corticosteroids. MethodsA retrospective analysis was conducted on the clinical data of patients with myasthenia gravis who received oral corticosteroids and underwent thoracoscopic thymectomy at the Department of Thoracic Surgery, the University of Hong Kong-Shenzhen Hospital from April 2013 to October 2019. Patients were divided into a high-dose steroid group and a medium-to-low dose steroid group based on the dosage of oral steroids, and the clinical data of the two groups were compared. ResultsA total of 102 patients were included, including 19 (18.62%) males and 83 (81.37%) females, with an average age of (32.25±9.83) years. All patients in both groups successfully completed the surgery without major intraoperative bleeding, conversion to open chest surgery, delayed extubation, severe infection, or perioperative death. The daily oral steroid dose for the high-dose steroid group was (35.81±4.29) mg, and for the medium-to-low dose steroid group it was (15.29±2.17) mg. There was no statistical difference in the operation time [(124.69±23.51) min vs. (117.89±21.46) min, P=0.172] and intraoperative blood loss [(21.19±3.48) mL vs. (20.56±3.41) mL, P=0.419] between the two groups. Postoperatively, 12 (11.76%) patients developed complications: one patient of myasthenic crisis (medium-to-low dose steroid group), which was improved after short-term respiratory support and intravenous immunoglobulin treatment; 11 patients of respiratory/swallowing difficulties (9 in the low-dose steroid group and 2 in the high-dose steroid group), which were improved after anticholinergic treatment to reduce oral secretions and sputum suction, and the patients were discharged smoothly. There was no statistical difference in the incidence of postoperative complications between the two groups (P=0.637). ConclusionOn the basis of good perioperative management, it is safe and feasible for patients with myasthenia gravis who take high dose of oral steroids to undergo thymectomy, and they have the same perioperative safety as patients with medium-to-low dose steroids.

  • The Application of Noninvasive Ventilation in Patients with Myasthenic Crisis after Thymectomy

    ObjectiveTo investigate the effect of noninvasive ventilation (NIV) in patients with myasthenic crisis after thymectomy. Methods31 myasthenic crisis patients after thymectomy who initially used NIV,admitted in the First Affiliated Hospital of Guangzhou Medical University between January 2011 and June 2013,were analyzed retrospectively.They were assigned to two groups according to the successful application of NIV or not,with 13 patients in the NIV success group and 18 patients in the NIV failure group.The related factors including gender,age,APACHEⅡ score when admitted to ICU,the results of blood gas analysis before NIV,thymoma or not,the history of myasthenic crisis,the history of chronic lung disease,and minute ventilation accounted for the largest percentage of predicted value (MVV%pred)were analyzed. ResultsThere were no significant differences in age,gender,or APACHEⅡ score between two groups (P>0.05).The PaCO2 in the NIV success group was lower than that in the NIV failure group.The preoperative MVV%pred in the NIV success group was higher than that in the NIV failure group.There were no significant differences between two groups in pH,PO2,thymoma or not,the history of myasthenic crisis,or the history of chronic lung disease (P>0.05).If using the 45 mm Hg as the cut-off value of PaCO2 and 60% as the cut-off value of MVV%pred,the incidence of PaCO2<45 mm Hg and the incidence of MVV%pred>60% were higher in the NIV success group than those in the NIV failure group (84.6% vs.33.3%, P<0.05;100% vs. 55.6%,P<0.05).Logistic regression analysis revealed that PaCO2<45 mm Hg was an independent influence factor for successful application of NIV in patients with myasthenic crisis after thymectomy. ConclusionPaCO2<45 mm Hg can be a predictor of successful application of NIV in patients with myasthenic crisis after thymectomy.For the patients underwent NIV whose PaCO2<45 mm Hg or MVV%pred<60%,the clinician should predict the possibility of failure and prepared for intubation.

  • 女性重症肌无力患者的外科治疗与围术期处理

    摘要: 目的 总结女性重症肌无力(MG)患者的外科治疗及围术期处理经验。 方法 回顾性分析1979年1月至2007年12月收治的186例(包括川北医学院附属医院胸心血管外科41例,南方医科大学附属南方医院胸心血管外科145例)MG女性患者的临床资料,年龄5~64岁,平均年龄43.9岁。按Osserman临床分型,小儿MG45例,成人MG141例,其中眼肌型25例,轻度全身型63例,中度全身型29例,急性暴发型19例,晚期严重型5例。合并胸腺瘤166例,按Masaoka分期 I期40例,Ⅱ期69例,Ⅲ期44例,Ⅳ期13例。152例患者经胸骨正中切口径路手术,行胸腺切除,纵隔淋巴结清扫术;34例胸腺无明显增生经颈部横切口行单纯胸腺切除术。 结果 全组无手术死亡。围术期发生MG危象22例,经相应的处理治愈。术后随访12个月以上165例,失访21例。痊愈30例(18.18%)、基本痊愈28例(16.97%)、显效60例(36.36%)、好转25例(15.15%)、无效22例(13.33%)。Ⅰ型、Ⅱa 型、Ⅱb 型、Ⅲ型和Ⅳ型有效率分别为100.00%、93.10%、90.00%、77.27%和53.85%。 结论 女性MG患者经胸腺切除术治疗效果良好;加强围术期处理,合理使用抗胆碱酯酶药物和激素,可减少各种MG危象的发生。

  • Subxiphoid uniportal approach using double sternum retractors versus subxiphoid and subcostal arch three-portal approach of video-assisted thoracoscopic surgery thymectomy for thymoma treatment: A retrospective cohort study

    ObjectiveTo compare the efficacy and safety of video-assisted thoracoscopic surgery (VATS) thymectomy for the treatment of thymoma through subxiphoid uniportal approach using double sternum retractors, and subxiphoid and subcostal arch approach. Methods We retrospectively analyzed the clinical data of the patients diagnosed with thymoma who underwent VATS thymectomy from June 2023 to June 2024 in West China Hospital. Patients were categorized based on the surgical approach into two groups: a subxiphoid uniportal VATS thymectomy (SUVT) group and a subxiphoid and subcostal arch VATS thymectomy (SASAT) group. Comparisons were made between the two groups regarding surgical duration, intraoperative blood loss, postoperative drainage, thymoma size and location, and postoperative pain assessed using the visual analogue scale (VAS). ResultsThe SUVT group consisted of 20 patients, including 11 males and 9 females, with an average age of (51.5±14.3) years. The SASAT group comprised 40 patients, including 26 males and 14 females, with an average age of (50.0±13.0) years. Compared to the SASAT group, the SUVT group had significantly larger thymomas [ (5.9±2.7) cm vs. (4.2±2.1) cm, P=0.010] and a higher proportion of neoplasms located in the superior mediastinum (30.0% vs. 2.5%, P=0.007). Additionally, the VAS pain scores on postoperative days 3, 7, and 30 were significantly lower in the SUVT group compared to the SASAT group (P<0.05). There were no statistical differences between the two groups in demographic characteristics, operative time, intraoperative blood loss, duration and volume of postoperative drainage, length of postoperative hospital stay, or the VAS pain score on the first postoperative day. Conclusion SUVT using double sternum retractors significantly reduces postoperative pain and provides superior efficacy in the resection of larger thymomas or those situated in the superior mediastinum.

  • Comparison of different types of thymectomy for the treatment of thymoma with myasthenia gravis

    Objective To compare the different surgical treatment methods of thymoma combined with myasthenia gravis (MG), and to discuss the clinical effectiveness of thoracoscopic combined mediastinoscopic extended thymectomy. Methods We retrospectively analyzed the clinical data of 58 patients of thymoma combined with myasthenia gravis in Northern Jiangsu People's Hospital between 2011 and 2016 year. According to the operation method, the patients were divided into three groups including a group A for thoracoscopic thymectomy (n=32), a group B for thoracoscopic combined mediastinoscopic thymectomy (n=15), and a group C for transsternal thymectomy (n=11). The clinical effects were observed and compared. Results In the group A and the group B, the bleeding volume, postoperative hospital stay and other complications were significantly lower than those in the group C with statistical differences (P<0.05). The incidence of myasthenic crisis in the group B (6.7%) was less than that in the group C (36.4 %), but the difference was not statistically different (P=0.058). The operation time of the three groups was 122.0 ± 39.4 min, 130.3 ± 42.5 min, and 142.3 ± 40.8 min respectively with no statistical difference between the two groups (P>0.05). The rate of dissection grade in the group B (grade 1, 12 patients, 80%) was significantly greater than that in the group A (grade 1, 14 patients, 43.8%,P<0.05). The effective rate of the group A, the group B, the group C was 84.4%, 93.3% and 90.9%, respectively with no statistical difference between groups (P>0.05). Conclusion The thoracoscopy combined mediastinoscopic thymectomy not only has the advantages of less trauma, quicker recovery and fewer complications, but also can more thoroughly clean the thymus and adipose tissue, which can achieve the same therapeutic effect as the transsternal thymectomy.

  • Progress in Thymectomy for the Treatment of Non-thymomatous Myasthenia Gravis

    Thymectomy is a major surgical procedure for patients with non-thymomatous myasthenia gravis,and can enhance their symptomatic remission rate and cure rate. There is still much controversy about appropriate surgical approach and extent of resection of thymectomy. The majority of thoracic surgeons believe that the completeness of thymectomy is closely associated with clinical symptom improvement,and perform complete resection of encapsulated thymus and surroun-ding fat tissues via mid-sternotomy. But minimally invasive thymectomies are often more acceptable by patients. On the contrary,in view of common existence of ectopic thymus tissue,some thoracic surgeons advocate a combination of cervical incision and sternotomy in order to further completely remove all thymus tissue.
