  • Current status of low vision in a population with opportunistic diabetes based on a physical examination center

ObjectiveTo investigate the current status of visual disability in people with opportunistic diabetes based on the physical examination center, and explore its related factors. MethodsPeople who went to West China Hospital of Sichuan University (West China Hospital district and Wenjiang hospital district) for physical examination between January 2019 and March 2020 were selected. The subjects were those who had a history of diabetes or fasting blood glucose≥7 mmol/L or glycosylated hemoglobin≥6.5%. They were divided into two groups according to visual acuity. The physical examinees with low vision were the observation group, and the physical examinees with normal vision were the control group (the number of cases was twice that of the observation group). The relevant data of the two groups were observed and compared, and the risk factors of low vision were analyzed by logistic regression. ResultsA total of 1 636 physical examinees with diabetes were included. There were 158 cases in the observation group and 316 cases in the control group. 158 cases (203 eyes) had low vision, and the incidence was 6.20% (203/3272). The main diseases leading to low vision were cataract (92 cases, 58.23%), high myopia (32 cases, 20.25%) and diabetes retinopathy (20 cases, 12.66%). Logistic regression analysis showed that the independent risk factors for low vision were age of diabetes patients, diabetes retinopathy, systolic blood pressure and glycosylated hemoglobin. ConclusionsThe incidence of low vision in diabetes population based on physical examination centers in Chengdu is low. Visual acuity examination should be strengthened for diabetes patients, especially the elderly, with diabetes retinopathy, high systolic blood pressure and glycosylated hemoglobin. Early effective prevention and treatment can reduce the damage to vision caused by diabetes.

  • Etiologies and vision outcomes of 367 inpatients with no light perception

Objective To observe the etiologies and vision outcomes of inpatients with no light perception (NLP). Methods A total of 367 inpatients (430 eyes) with NLP in Zhongshan Ophthalmic Center were enrolled in this study. The visual acuity examination followed the international standard methods. NLP was detected by torch light in a dark room and the pupil light reflection state was also considered. The patients included 208 males (235 eyes) and 159 females (195 eyes). Sixtythree patients (126 eyes) were bilateral and 304 patients (304 eyes) were unilateral cases including 159 right eyes and 145 left eyes. The patients' ages ranged from 2.5 to 86.0 years, with a mean age of (40.85plusmn;18.03) years. All the patients were treated according to their diseases. The ratio of different eye disease and visual outcome were recorded and analyzed. Results Among 430 eyes, there were 157 eyes (36.5%) with optic neuritis, 68 eyes (15.8%) with uveitis, 54 eyes (12.6%) with retinal vascular disease, 35 eyes (8.1%) with ischemic optic neuropathy, 29 eyes (6.7%) with traumatic optic neuropathy, 28 eyes (6.5%) with optic atrophy, 18 eyes (4.2%) with trauma, 17 eyes (4.0%) with radiation optic neuropathy, 10 eyes (23%) with glaucoma, five eyes (1.2%) with retinal detachment, four eyes (0.9%) with compressive optic neuropathy, two eyes (0.5%) with orbital apex syndrome, two eyes (0.5%) with hysteria, and one eye (0.2%) with orbital cellulitis. After active treatment, 269 eyes (62.6%) remained NLP, 161 eyes (37.4%) got improved visual acuity, including light perception- 0.02 in 74 eyes (17.2%), ge;0.02-<0.05 in 25 eyes (5.8%), ge;0.05 -<0.1 in 14 eyes (3.3%), ge;0.1 -<0.3 in 11 eyes (2.6%) and ge;0.3 in 37 eyes (8.6%). Conclusions The main causes of nonsurgical and non-trauma NLP are retinal disease and optic neuropathy. Some patients with NLP may restore useful vision if they received prompt referral and active intervention.

  • Visual improvement of therapeutic plasma exchange for refractory optic neuritis patients in acute phase

ObjectiveTo evaluate the visual improvement of therapeutic plasma exchange (TPE) for refractory optic neuritis (ON) patients in acute phase.MethodsSeventy-five affected eyes from 44 refractory ON patients with severe visual defect or resistance to high-dose intravenous methylprednisolone (IVMP) therapy, who were admitted to The Chinese PLA General Hospital between January 2015 and August 2016, were recruited and received TPE therapy. Among these patients, 11 were male and 33 were female; the average age was 39.1±13.9; 31 patients had two affected eyes, 13 patients had one affected eye. The course of the disease on the group of patients were more than 2 weeks, and the visual acuity worsened for more than 10 days and continued to deteriorate. TPE treatment was performed on all of the patients. BCVA was recorded before and 24 h after treatment, and the visual function was scored using visual outcome scale (VOS). At the same time, the adverse reactions of TPE treatment were observed. The paired t-test was used to compare the VOS before and after treatment. The correlation between VOS before and after treatment was analyzed by Linear-by-Linear correlation analysis.ResultsAmong 75 affected eyes, the post-therapy VOS 3.89±2.13 was significantly improved from pre-therapy VOS 5.56±1.69 (t=6.77, P<0.001). Forty-eight of 75 eyes were improved at lease 1 score of VOS, the overall rate of visual improvement was 64.0%. Especially among the eyes with initial vision of light perception, an improved rate of 82.4% was presented. 75.0% in those eyes with initial vision of count fingers and 67.7% in no light perception. Linear-by-Linear correlation analysis showed a significant linear correlation between the scores of VOS before and after TPE treatment (r=0.398, P=0.01). During the course of TPE treatment, 5 patients had mild adverse reactions such as low calcium reaction and allergic reaction and were well controlled after treatment.ConclusionUsing TPE to treat refractory ON in acute phased can improve the visual function of patients.

  • Analysis of the causes of visual loss after silicone oil removal

Objective To investigate the causes of visual loss and failure of treatment after intraocular silicone oil removal. Methods Retrospective clinical analysis of the causes of loss of visual acuity of 15 eyes after silicone oil removal in patients with complicated retinal detachment which were successfully treated with vitreous and retinal microsurgery. Results Among the 15 eyes, retina failed to reattach in 11 eyes, secondary glaucoma occurred in 2 eyes and corneal opacity appeared in another two eyes. All of them were resulted in total loss of vision. Conclusion Retinal redetachment was the leading cause of final visual loss in the failing 15 cases(15 eyes) after intraocular silicone oil removal, and secondary glaucoma and corneal decompensation may also be the causes of visual loss. (Chin J Ocul Fundus Dis, 1999, 15: 230-231)

  • The main effective factors for the visual outcome of macular edema secondary to central retinal vein occlusion

Macular edema (ME) secondary to central retinal vein occlusion(CRVO) often cause severe visual impairment. Intravitreal anti-vascular endothelial growth factor agents and steroids can effectively eliminate ME and improve visual function, but the visual outcome is affected by multiple factors. Retinal blood flow, especially the macular microcirculation, has significant correlation with visual outcome. Ischemic CRVO, especially patients with severe damage in the deep and superficial vascular layer of the macular zone, usually have poor visual outcome. In addition, the integrity of the multi-layer retinal structure closely correlates with the visual outcome. Patients with intact ellipsoid zone, external limit membrane beneath the fovea have good visual recovery. Additionally, good baseline visual acuity, positive response to treatment in early phase, young age and timely treatment usually brings about better visual outcome.
