  • Exploration of multi-type outpatient multi-disciplinary team models

    The outpatient multi-disciplinary team (MDT) model is an important measure to improve the quality of medical services and enhance patients’ medical experience. The Second Affiliated Hospital of Zhejiang University School of Medicine has explored three types of outpatient MDT models, namely specialized disease model, self-service model, and professor team model, in order to improve the accessibility and coverage of outpatient MDT. Through practice, it has been found that the implementation of multi-type outpatient MDT models can further leverage the advantages of MDT in society, hospitals, and patients. This article will share the experience of building multi-type outpatient multi-disciplinary team models mentioned above.

  • The Experience of Diagnosis and Treatment to 22 Cases of Acute Traumatic Diaphragmatic Hernia

    目的:探讨急性创伤性膈疝的诊疗体会。方法:对2000年1月至2008年12月我院收治的22例急性创伤性膈疝的创伤原因、临床表现、辅助检查以及治疗方法进行回顾性分析。结果:术前确诊20例,术中确诊2例,手术治愈21例,死亡1例,平均住院16~25天。结论:急性创伤性膈疝极易发生绞窄,且容易被合并伤所掩盖,所以早期的诊断和治疗就尤为重要。

  • Development and prospects of multi-disciplinary team in digital era

    The national policy on high-quality development of hospitals proposes to strengthen information technology support and actively promote the multi-disciplinary team (MDT) model. How to use the “Internet Plus” technology and operation mode to promote MDT communication and improve the efficiency of diagnosis and treatment in the digital and intelligent information age is a direction worthy of attention and research. This paper systematically reviews the current development status of MDT informatization construction at home and abroad. Based on the current challenges and opportunities, it makes prospects for the future development of MDT informatization construction from the aspects of strengthening the digital and intelligent support of MDT operation, connecting MDT “information silos”, and deepening the construction of MDT supervision and effect evaluation system, etc.

  • Expert consensus on diagnosis and management of neurofibromatosis type 1 (2021 edition)

    Neurofibromatosis type 1 (NF1) is an autosomal dominant neoplastic disease caused by mutations in the NF1 gene and one of the most challenging diseases to treat. Patients have a characteristic phenotype with neurofibromas as the main features in different forms, including numerous cutaneous neurofibromas, plexiform neurofibromas involving the primary nerves, or malignant peripheral nerve sheath tumors with a very short survival period after malignant transformation. NF1 patients also suffer from multi-system involvement, with a high rate of deformity and disability, making complete surgical resection more difficult. Currently, there is no consensus on the diagnosis and treatment of NF1 in China, and different disciplines have different understandings of NF1. Multidisciplinary systematic evaluations and cooperative treatments are the keys to improve the treatment, quality of life, and prognosis of NF1 patients. In 2020, the Department of Plastic Surgery of the Ninth People’s Hospital of Shanghai Jiaotong University School of Medicine led the establishment of the first multi-center collaboration group for NF1 in China. Furthermore, the group had worked with renowned experts from the various departments including surgical oncology, medical oncology, dermatology, reproductive medicine, et al. in China to formulate the “Expert consensus on diagnosis and management of neurofibromatosis type 1 (2021 edition)”, aiming to promote standardized and homogeneous treatment covering the whole life cycle of NF1 patients and improve the treatment level and outcome of NF1 patients in China.

  • Integrating radiomics into multi-omics research: unveiling new perspectives on precision oncology

    Cancer presents a significant global public health challenge, impacting human health on a broad scale. In recent years, the rapid advancement of big data-based bioinformatics has unveiled crucial potential in precision oncology through various omics research methods. The advent of radiomics has notably expanded the application scope of medical imaging in the field. However, due to the multi-level and multifactorial nature of tumor initiation and progression, a single omics information remains insufficient to meet the demands for advancing precision oncology strategies. Multi-omics research has become an emerging trend. The research paradigm integrating radiomics with other omics offers a novel perspective for personalized decision-making in oncology. Nevertheless, there persists a need to introduce more integrated new technologies and theories to expedite the progress of this field.

  • 双心护理在冠状动脉介入诊疗术患者中的应用

    目的探讨双心护理对于行冠状动脉介入诊疗患者的作用。 方法选取2013年3月-11月在心内科住院并行介入诊疗的冠状动脉粥样硬化性心脏病患者100例,按入院单双日将患者分为对照组和干预组;对照组给予常规护理;干预组在给予常规护理基础上实施心理护理。利用焦虑评估值、心率、术后并发症评价两组患者护理效果。 结果干预组实施双心护理后,其焦虑评估值、心率、术后并发症发生率均低于对照组,满意度高于对照组,两组比较差异有统计学意义(P<0.01)。 结论双心护理有利于患者术前、术中、术后的配合,减少并发症的发生,提高患者满意度。

  • Changes of disease spectrum and main therapeutic interventions in respiratory intensive care unit

    ObjectiveTo analyze the trend of disease spectrum and main diagnosis and therapeutic technologies in respiratory intensive care unit (RICU) in recent years, and find out the trend of change of patient’s characteristics and commonly used interventions in order to provide evidence for planning discipline development and improving personnel training program.MethodsPatients information and main diagnosis and therapeutic technologies of 1503 inpatients in RICU of Shenzhen People's Hospital from January, 2017 to December, 2020 were collected. The changes of disease spectrum and diagnosis and treatment technologies in different years were compared and analyzed.ResultsAmong all the patients, 66.3% were directly admitted into RICU, 12.1% were transferred from respiratory department, and 21.6% were transferred from other departments. The proportion of patients with non-respiratory diseases as principal diagnosis had an increasing trend, from 18.8% in 2017 to 37.3% in 2020 (P<0.05). The diseases with most obvious increasing trend were sepsis, nervous system diseases, circulatory system diseases and extra-pulmonary malignancies (P<0.05). The use of respiratory related diagnosis and therapeutic technologies was gradually increasing, meanwhile, the use of non-traditional respiratory related technologies, especially continuous renal replacement therapy, was also increasing. There was no significant difference in fatality rate among different years (P>0.05).ConclusionsThe number of patients with extra-pulmonary diseases and the use of non-traditional respiratory related diagnosis and therapeutic technologies in RICU were increasing. The development of RICU and the allocation of technical personnel needed to be improved accordingly.

  • Application of multi-disciplinary team clinic in thyroid eye disease

    Objective To analyze the implementation of multi-disciplinary team (MDT) clinic for thyroid eye disease (TED) and explore the significance of TED MDT clinic. Methods A retrospective analysis was conducted on the medical records of patients who visited TED MDT clinic at West China Hospital of Sichuan University between December 1, 2022 and November 30, 2024. Patient condition indicators were collected and MDT outpatient operation indicators were statistically analyzed. Results During the study period, a total of 696 patients were admitted to the TED MDT clinic, with an average age of (51.14±12.00) years. All patients were diagnosed with TED and showed symptoms of extraocular muscle involvement and restricted eye movement. According to the NOSPECS clinical grading system, all patients were classified as grade 4 or above. Among them, 693 patients’ orbital enhanced MRI showed that the extraocular muscles were in an inflammatory active phase, and 690 patients received periarbital injection of triamcinolone acetonide, precise orbital radiation therapy, and thyroid function regulation. During the research period, TED MDT clinic received a total of 90 visits, with an average of 7.73 patients per visit. The patient satisfaction rate was 99.67%, and there were no cases of experts being late, absent, or doctors with insufficient qualifications to participate in discussions. ConclusionThe TED MDT clinic has been well developed, and the experts within the team attach great importance to it, making it valuable for continued large-scale promotion.

  • Construction of a panoramic diagnosis and treatment service system for difficult and severe diseases in outpatient services

    Under the policy background of deepening the reform of the hierarchical diagnosis and treatment system in the country, large public hospitals, as centers for the diagnosis and treatment of difficult and severe diseases, face multiple challenges such as difficulty in patient appointment, difficulty in precise diagnosis and treatment, and difficulty in continuous medical care. To overcome this series of severe challenges, West China Hospital of Sichuan University has established a “panoramic” outpatient diagnosis and treatment service system for difficult and severe diseases based on the pain points of patients seeking medical treatment. This system optimizes the entire diagnosis and treatment process through innovative measures such as pre-diagnosis “three convergences”, during-diagnosis “three guarantees”, and post-diagnosis “three connections”. It not only significantly improves the efficiency and quality of diagnosis and treatment, but also provides patients with a high-quality and convenient medical service experience, offering a practical example for large public hospitals to address the diagnosis and treatment challenges of difficult and severe diseases.
