  • A multicenter survey on the current status of delirium knowledge among geriatric nurses in Sichuan province

    Methods To explore the level of delirium knowledge of geriatric nurses in Sichuan province and analyze the factors, so as to provide the basis for systematic and targeted knowledge training on delirium and clinical management. Methods Using the self-designed “the Questionnaire of Elderly Delirium Knowledge”, geriatric nurses from 22 hospitals in Sichuan province were investigated through a convenient sampling method from September 2018 to February 2019. Results A total of 475 geriatric nurses were investigated. The average delirium knowledge score of the 475 geriatric nurses was 69.51±12.42. Multiple linear regression analysis showed that the main factors affecting the score of delirium-related knowledge were the education of nurses (P=0.037), technical title (P<0.001), years of working in the geriatric department (P=0.001), and the level of working hospital (P=0.001). Conclusions The level of delirium knowledge of geriatric nurses is low and can not meet the needs of clinical work. Nursing managers should carry out delirium knowledge training according to the different characteristics of nurses.

  • The risk factors of delirium in mechanical ventilation patients with chronic obstructive pulmonary disease

    ObjectiveTo investigate the risk factors of delirium in mechanical ventilation patients with chronic obstructive pulmonary disease (COPD).MethodsA total of 97 mechanically ventilated non-hypertensive patients with COPD who were admitted to this department from January 2018 to October 2018 were selected as subjects. The patients were divided into 49 cases with delirium and 48 cases non-delirium according to the Consciousness Assessment Method for the Intensive Care Uint. The examined data were collected in the patients such as pH, arterial partial pressure of oxygen (PaO2), arterial partial pressure of carbon dioxide (PaCO2), neuron-specific enolase (NSE), and Acute Physiology and Chronic Health EvaluationⅡ (APACHEⅡ) scores were calculated in the pre-mechanical (d0) and mechanically ventilated 3rd (d3), 5th (d5) days. The mechanical ventilation days were recorded in the two groups. Logistic regression analysis was used to screen the risk factors influencing delirium of patients.ResultsThe PaCO2, NSE, APACHEⅡ scores and mechanical ventilation days were higher in the delirium group than in the non-delirium group [(88.1±7.5) vs. (85.3±6.2) mm Hg; (28.4±5.8) vs. (26.1±3.3) μg/L; (23.7±3.9) vs. (21.7±2.6); (7.5±1.3) d vs. (6.6±1.2) d] and PaO2 were lower than non-delirium group [(54.9±5.5) vs. (57.2±3.1) mm Hg], the differences were statistically significant (P<0.05). Multivariate logistic regression analysis showed that PaO2, NSE, APACHEⅡ scores and mechanical ventilation days were risk factors for delirium in mechanically ventilated patients with COPD (regression coefficients were –0.177, 0.163, 0.203, 0.597 respectively, P<0.05). The PaO2 and APACHEⅡ scores of mechanical ventilation on the 3rd and 5th day of the two groups [d3 (88.3±5.3) vs. (89.1±6.9) mm Hg; d5 (90.3±9.0) vs. (91.3±6.4) mm Hg; d3 (21.7±3.0) vs. (21.4±2.2); d5 (20.9±2.8) vs. (20.7±2.1)] were not statistically significant (P>0.05).The NSE changes on the 3rd and 5th day of mechanical ventilation [d3 (30.0±5.3) vs. (26.8±3.6) μg/L; d5 (27.3±4.3) vs. (25.7±2.6) μg/L] were statistically significant (P<0.05).ConclusionPaO2, NSE, APACHEⅡ score and mechanical ventilation days are risk factors for delirium in COPD patients with mechanical ventilation and NSE is one of the more important risk factors.

  • Analysis of related factors of postoperative delirium in elderly colon cancer patients undergoing radical surgery

    ObjectiveTo explore the relevant risk factors for postoperative delirium (POD) in elderly patients undergoing radical colon cancer surgery, and provide a basis for formulating postoperative prevention and treatment measures for POD. MethodsA total of 128 elderly patients diagnosed with colon cancer and underwent radical colon cancer surgery at Xindu District People’s Hospital in Chengdu from January 2018 to December 2021 were included as the study subjects. Patients were divided into two groups according to the score of Delirium Assessment Scale (4AT Scale). The basic data, main perioperative clinical data and laboratory indicators of the two groups were collected, and univariate and logistic regression analysis were carried out to determine the potential risk factors of POD in elderly patients with colon cancer after radical operation. ResultsAccording to the results of the 4AT scale score, a total score of ≥4 points was used as the threshold for determining patient POD. Among 128 patients, there were 29 patients (22.66%) with POD and 99 patients (77.34%) without POD. ① General data comparison: There was no significant difference between the two groups in gender, body mass index, years of education, hypertension, diabetes, smoking history and drinking history (P>0.05), but there was significant difference in age, preoperative mini-mental state examination (MMSE) score and American Society of Anesthesiologists (ASA) grade (P<0.05). ② Comparison of main clinical data during the perioperative period: There was no statistically significant difference between the two groups of patients in ICU treatment, nonsteroidal anti-inflammatory drug treatment, visual analogue scale, and intraoperative hypotension (P>0.05), but there was a statistically significant difference in operative time, anesthesia time, intraoperative blood loss, and dexmedetomidine treatment (P<0.05). ③ Comparison of preoperative laboratory indicators: There was no statistically significant difference between the two groups of patients in terms of hemoglobin, serum albumin, white blood cell count, prognostic nutritional index, neutrophil/lymphocyte ratio, D-dimer, and albumin to fibrinogen ratio (P>0.05). ④ The results of logistic regression analysis showed that low preoperative MMSE score [OR=0.397, 95%CI (0.234, 0.673)], long surgical time [OR=1.159, 95%CI (1.059, 1.267) ], and long anesthesia time [OR=1.138, 95%CI (1.057, 1.226) ] were independent risk factors for the occurrence of POD in elderly colon cancer patients undergoing radical surgery. ConclusionPreoperative MMSE score, operative time, and anesthesia time are closely related to the occurrence of POD in elderly colon cancer radical surgery, worth implementing key perioperative management in clinical practice to prevent and manage POD.

  • Research progress on the role mechanism of gut flora in postoperative delirium and its prevention strategy

    Postoperative delirium (POD) is a common postoperative complication. Dysregulation of gut flora is involved in POD through mechanisms such as neuroinflammation, oxidative stress, deposition of β-amyloid, and aberrant production of metabolites of gut flora. Therefore, interventions to regulate gut flora, such as probiotics, prebiotics, and faecal microbiota transplantation, can alleviate cognitive dysfunction. This article reviews the mechanisms of gut flora in POD and its prevention and treatment strategies, with the aim of providing new ideas for the clinical prevention and treatment of POD.

  • Efficacy and Safety of Antipsychotics in the Treatment of Delirium: A Systematic Review

    ObjectivesTo systematically review the clinical efficacy and safety of antipsychotics for delirium. MethodsDatabases including The Cochrane Library (Issue 5, 2015), PubMed, MEDLINE, EMbase, CNKI, VIP and WanFang Data were electronically searched for randomized controlled trials (RCTs) about antipsychotics compared with placebo/blank for delirium from inception to May 2015. We also hand-searched related conference proceedings and references of included studies for additional studies. Two reviewers independently screened literature, extracted data, and assessed the risk of bias of included studies. Then, meta-analysis was conducted by using RevMen 5.3 software. ResultsA total of 7 RCTs involving 712 patients were included. The results of meta-analysis showed that there were no significant differences between the antipsychotics group and the placebo/blank group in mortality (RR=1.00, 95%CI 0.90 to 1.10, P=0.99), duration of delirium (MD=-1.53, 95%CI -4.95 to 1.89, P=0.38), length of stay (MD=-0.89, 95%CI -7.69 to 5.90, P=0.80), and ICU stay time (MD=-3.70, 95%CI -15.83 to 8.43, P=0.55). Compared with the placebo/ blank group, the antipsychotics could reduce the severity of delirium (SMD=-1.62, 95%CI -2.32 to -0.93, P<0.000 01). ConclusionCurrent evidence shows that the efficacy of antipsychotics in the treatment of delirium is not clear. Due to the limited quantity and quality of the included studies, the above conclusion needs to be further verified by more high quality studies.

  • Delirium Syndrome after Spinal Surgery

    目的:分析脊柱外科患者术后并发谵妄综合征的原因,总结其诊断、预防、治疗。方法:回顾性分析我科2008年10月至2009年4月脊柱手术167例,其中11例患者术后发生谵妄综合征。结果:11例患者均给予氟哌啶醇5mg im bid治疗,平均使用5.6天,症状缓解;并获3~6月随访,无一例复发谵妄综合征。结论:谵妄综合征是脊柱外科患者术后常见并发症,其发生与年龄,性别,低血糖等有关,目前治疗首选氟哌啶醇。

  • 基于加速康复外科的综合护理模式对腰椎间盘突出症老年患者术后谵妄发生的效果研究

    目的 探究基于加速康复外科(enhanced recovery after surgery,ERAS)的综合护理模式对腰椎间盘突出症老年患者术后谵妄(postoperative delirium,POD)发生的效果研究。 方法 采取便利抽样的方法选取 2016 年 3 月—2017 年 2 月因腰椎间盘突出症行经皮内镜微创手术患者 80 例,将 2016 年 3 月—9 月的 40 例患者作为对照组,2016 年 10 月—2017 年 2 月的 40 例患者作为观察组。对照组采取常规围手术期护理措施,观察组采取基于 ERAS 的综合护理模式。对两组患者的 POD 发生率、术后住院时间、住院时间、患者满意度进行比较。 结果 观察组无 POD 发生,低于对照组[15%(6/40)],差异有统计学意义(χ2=4.505,P=0.034)。观察组患者术后住院时间[(1.0±0.5)d]和住院时间[(5.1±1.6)d]均低于对照组[(3.6±2.3)、(10.1±4.9)d],患者满意度[(99.8±0.5)分]高于对照组[(99.2±1.0)分],差异有统计学意义(P<0.05)。 结论 对腰椎间盘突出症老年患者实施基于 ERAS 的综合护理模式,降低了其 POD 的发生率,同时有利于缩短老年患者的术后住院时间、住院时间,提高患者满意度。

  • Clinical Features and Treatment Strategies of Patients with Delirium in Emergency Intensive Care Unit

    Objective To investigate the clinical features, etiology and treatment strategies of patients with delirium in emergency intensive care unit ( EICU) . Methods Patients with delirium during hospitalization between January 2010 and January 2012 were recruited from respiratory group of EICU of Beijing Anzhen Hospital. Over the same period, same amount of patients without delirium were randomly collected as control. The clinical datawere retrospectively analyzed and compared. Results The incidence of delirium was 7.5% ( 42/563) . All delirium patients had more than three kinds of diseases including lung infections, hypertension, coronary heart disease, respiratory failure, heart failure, renal failure, hyponatremia, etc. 50% of delirium patients received mechanical ventilation ( invasive/noninvasive) . The mortality of both the delirium patients and the control patients was 11.9% ( 5 /42) . However, the patients with delirium exhibited longer hospital stay [ 14(11) d vs. 12(11) d, P gt;0. 05] and higher hospitalization cost [ 28, 389 ( 58,999) vs. 19, 373( 21, 457) , P lt;0.05] when compared with the control group. 52.4% ( 22/42) of delirium patients were associated with primary disease. 9. 5% ( 4/42) were associated with medication. 38. 1% (16/42) were associated with ICU environment and other factors. Conclusions Our data suggest that the causes of delirium in ICU are complex. Comprehensive treatment such as removal of the relevant aggravating factors, treating underlying diseases, enhancing patient communication, and providing counseling can shorten their hospital stay, reduce hospitalization costs, and promote rehabilitation.

  • Association of anesthetics with postoperative delirium in patients undergoing cardiac surgery

    ObjectiveTo determine whether there was a clinical relevant association between anesthetic regimen (propofol or inhalational anesthetics) and the occurrence of postoperative delirium (POD) in patients undergoing cardiac surgery.MethodsThis retrospective study was conducted on patients with elective cardiac surgery under cardiopulmonary bypass (CPB) at West China Hospital of Sichuan University between October 2018 and March 2019. The patients were divided into a propofol group or an inhalational anesthetics group according to anesthetic regimen (including CPB). The primary outcome was the occurrence of POD during first 3 days after surgery. Logistic regression analysis was used to determine the relationship between anesthetic regimen and the occurrence of POD.ResultsA total of 197 patients who met the inclusion criteria were included, with an average age of 53 years, and 51.8% (102/197) were females. POD occurred in 21.3% (42/197) patients. The incidence of POD was 21.4% in the propofol group and 21.2% in the inhalational anesthetics group; there was no significant difference between the two groups (RR=1.01, 95%CI 0.51-2.00, P=0.970). Logistic regression analysis did not find that anesthetic regimen was a risk factor for delirium after cardiac surgery after adjusting risk factors (OR=1.05, 95%CI 0.48-2.32, P=0.900).ConclusionAnesthetic regimen (propofol or inhalational anesthetics) is not associated with an increased risk for POD in adult patients undergoing elective cardiac surgery under CPB.

  • Predictive value of varying degrees of delirium on the occurrence of perioperative myocardial infarction following off-pump coronary aortic bypass grafting

    Objective To explore the predictive value of different degrees of delirium after off-pump coronary aortic bypass grafting (OPCABG) for perioperative myocardial infarction (MI). Methods A retrospective analysis was conducted on the clinical data of patients who underwent OPCABG in the First Department of Cardiac Surgery at the First Hospital of Hebei Medical University between April 2018 and March 2024. Patients were divided into a mild delirium group, a moderate delirium group, and a severe delirium group based on the degree of delirium, and into a MI group and a non-MI group based on the occurrence of perioperative MI. Binary logistic regression analysis was used to investigate the predictive factors for secondary MI during OPCABG. The predictive value of different degrees of postoperative delirium for secondary perioperative MI was assessed using receiver operating characteristic (ROC) curves. Results A total of 436 patients were included, with 211 males and 225 females, and a median age of 51 (44.0, 57.75) years. Delirium occurred in 139 patients, with 52 in the mild delirium group, 29 in the moderate delirium group, and 58 in the severe delirium group. MI occurred in 101 patients, with 101 in the MI group and 335 in the non-MI group. Binary logistic regression analysis showed that severe delirium was an independent predictor of secondary MI during OPCABG [OR=23.979, 95% CI (11.572, 49.691), P=0.000]. ROC curve analysis revealed that the area under the ROC curve for predicting perioperative MI by severe postoperative delirium was 0.709, with a sensitivity of 0.546 and a specificity of 0.964. Conclusion Severe postoperative delirium can be used as an indicator to predict secondary MI during OPCABG.
