目的 目前对青壮年有移位新鲜股骨颈骨折患者的治疗仍存在许多争议,被认为尚未完全解决难题之一,股骨颈骨折内固定术后常导致骨折不愈合股骨头缺血性坏死。 方法 2003年1月-2008年6月,收治青壮年新鲜移位股骨颈骨折患者12例,年龄17~55岁,平均36.3 岁。所有患者在受伤后1周内采用牵引下闭合复位空心钉内固定加股方肌骨瓣移植术治疗。术后随访1~5年,平均3.5年。 结果 12例患者均达骨性愈合,随访期内未出现股骨头缺血性坏死;术后采用Harris 髋关节评分标准进行评估,获优良10例,一般1例,差1例。 结论 青壮年新鲜有移位股骨颈骨折患者,采用牵引下闭合复位空心钉内固定加股方肌骨瓣转移术治疗效果良好,均能达到骨折愈合,且无股骨头缺血性坏死发生。
Since 1984, the authors have used the iliac bone graft with ascending branch of lateral circomflex femoral vessel for treatment of bone defect, nonunion, and avascular necrosis of femoral head in 88 cases. Seventy-two patients were followed from one to six years. Seventy cases were successful with 2 failure. The applied anatomy and clinical application of the iliac bone graft, and the operative principles were discused in this article.
Objective To study the method and effect of transferring the pedicled second metatarsal base for repairing bone defect of lateral malleolus. Methods Thirty lower limb specimens were anatomized to observe the morphology, structure and blood supply of the second metatarsal bone . Then transferring of thepedicled second metatarsal base was designed and used in 6 patients clinically.All cases were male, aged from 24 to 48 years old, and the area of bone defect was 3-4 cm. Results Followed up for 3-11 months, all patients healed primarily both in donor and recipient sites. There were excellent results in 4 cases and good results in 2 cases . The morphology and function of the malleoli were satisfactory. Conclusion Transferring of the pedicled second metatarsal base for repairing bone defect of lateral malleolus is an effective and reliable operative method.
In order to study the curative effect of vascularized bone graft in the treatment of avascular necrosis of talus, 24 patients were treated with vascularized bone grafts, in which 9 cases had received 1st cuneiform bone graft with a malleolaris anteriomedialis, 4 cases with the 1st cuneiform bone graft with the medial tarsal artery and 11 cases with vascularized cuboid bone graft with the lateral tarsal artery. All of the patients were followed up for 3-5.5 years. The clinical observation and X-ray examination showed that function of the ankle joint was completely or almost normal in 16 cases, and the bone repair was excellent. There was slight pain in the ankle joint in 4 cases. The efficiency rate of the treatment was 83.3%. It could be concluded that vascularized bone graft might be an effective method in the treatment of avascular necrosis of talus.
In order to explore a new method to treat the ischemic necrosis of the head of 2nd metatarsal bone in adult, transposition of reversed vascularized metatarsal bone graft was adopted in the treatment of 4 cases, 2 of which were followed up for one and a half years. The postoperative X-ray showed the round head of the 2nd metatarsal bone became round and even density. The clinical observation showed that there was excellent function of metatarsophalangeal joint. It was concluded that the reversed vascularized metatarsal bone graft could provide a reliable blood supply and various osteogenic factors to the avascularized head of the metatarsal bone, and the operation could also debride the joint cavity and carry out decompression.
OBJECTIVE To study the anatomical basis of vascularized spina scapular bone flap, which was used in mandibular reconstruction. METHODS Fifteen adult cadavers were adopted in this study. The two common carotid arteries of each cadaver were intubed and perfused with red emulsion respectively. Then the course and distribution of the transverse cervical artery(TCA) and its spina scapular branches were observed on 30 sides. RESULTS The TCA was divided into two segments: the cervical segment originated from the origin of the artery to the superior margin of the trapezius muscle, and the dorsal segment originated from the superior margin of the trapezius muscle to the site where the TCA bifurcated into the superficial and deep branches. The average length and original caliber of the cervical segment were(4.7 +/- 0.1) cm and (4.0 +/- 0.1) mm. The average length and original caliber of the dorsal segment were (5.88 +/- 0.63) cm and (3.30 +/- 0.35) mm. 86.7% spina scapular branches originated from the superficial branch of TCA and 13.3% from TCA. The length of the spina scapular branch was (4.97 +/- 1.68) cm and its external diameter was (2.08 +/- 0.27) mm. It constantly sent 4-8 periosteal branches to spina with 0.20-1.25 mm in caliber. CONCLUSION The spina scapular branch of TCA is one of the main blood supplier to the spina scapular area. The spina scapular flap pedicled with spina scapular branch of TCA may provide a new operation for mandibular reconstruction, whose circumpoint locates at the origin of the dorsal segment and the average length of the pedicle is 10.85 cm which enough to transposite to mandibular area.
Objective To compare the effects and indications of total hip replacement(THR) and transplantation of vascularized bone graft in treating late ischemic necrosis of the femoral head.Methods From March 1986 to March 1993, 81 patients with late ischemic necrosis underwent treatment. Of 81 patients, 59 patients who suffered in unilateral hip were divided into 2 groups: 26 underwent total hip replacement and 33 underwent transplantation of vascularized bone graft. There was no significant difference in the Harris-scores of the two groups before operations (Pgt;0.05). The Harris hip scores were compared between different ages(<30 years, 31.50 years, and >51 years) in two groups. Results The follow-up ranged from 10 years and 3 months to 16 years and 5 months (15 years and two months on average). The changes of Harrisscores of the two groups after operations had significant difference in initial stage(Plt;0.05),but had no significant difference in the long term(Pgt;0.05). The change of Harris-scores of patients who underwent total hip replacement had no significant difference between different ages (Pgt;0.05),but that of patients who underwent transplantation of vascularized bone graft had significant difference between different ages(Plt;0.05). Both groupshad satisfactory effects in treating late ischemic necrosis of the femoral head. Conclusion Both methods are effective in treating late ischemic necrosis of the femoral head. Total hip replacement can be more effective insenior patients. Transplantation of vascularized bone can be more effective in young patients.
