  • Interpretation of guidelines for hypertension in chronic kidney disease

    Chronic kidney disease (CKD) and hypertension are very common chronic diseases. Active and standardized treatment of hypertension in patients with CKD can not only delay the progress of renal disease, but also reduce the risk of cardiovascular events. In recent years, although the guidelines for hypertension have put forward detailed suggestions for the management of hypertension in CKD patients, there are differences in the recommendation of blood pressure target value for CKD patients. Combined with the latest guidelines, this review interprets the blood pressure measurement methods, diagnostic criteria, antihypertensive targets and drug therapy in patients with CKD.

  • Effect of Combination of Antihypertensive and Lipid Lowering Therapy on Arterial Stiffness in Elderly Patients with Mild to Moderate Essential Hypertension

    Objective To observe the effect of combination of antihypertensive and lipid lowering therapy on arterial stiffness in elderly patients with mild to moderate essential hypertension. Methods A total of 216 elderly patients with mild to moderate essential hypertension were enrolled and treated by hydrochlorothiazide as the basic therapy for two weeks. Then the patients were randomly divided into four groups. Namely, the intensified antihypertensive and lipid lowering therapy group (hydrochlorothiazide 25 mg/d, Candesartan 8 mg/d, Rosuvastatin 10 mg/d, n=54), the intensified antihypertensive treatment group (hydrochlorothiazide 25 mg/d, Candesartan 8 mg/d, n=54), the antihypertensive and lipid lowering therapy group (hydrochlorothiazide 25 mg/d, Rosuvastatin 10 mg/d, n=54), and the control group (hydrochlorothiazide 25 mg/d, n=54). After 12-month treatment, the blood pressure, blood lipid and carotid-radial pulse wave velocity (crPWV) of each group were recorded. Results Twelve months later, the SBP, DBP, PP and crPWV of each group were significantly lower than before (Plt;0.05). There was interactive effect of antihypertensive and lipid lowering therapy in lowering SBP, DBP, PP and crPWV (F=40.765, 4.869, 24.829, and 53.149, respectively, all Рlt;0.05). Conclusion The combination of antihypertensive and lipid lowering therapy can significantly lower the crPWV of elderly patients with hypertension and improve the arterial stiffness; it is superior to single treatment of either antihypertensive or lipid lowering.

  • Factors Influencing Early Prognosis after Surgical Treatment of Supratentorial Hypertensive Intracerebral Hemorrhage

    目的:探讨幕上高血压脑出血手术治疗近期预后的影响因素。方法:回顾经手术治疗的73例幕上高血压脑出血患者的临床资料,分析患者性别、年龄、术前GCS评分、出血部位、术前瞳孔有无散大、出血是否破入脑室、出血量、手术时机8个因素对患者预后的影响。评定患者日常生活能力(ADL)分级,将ADL 1-3级划为预后良好组,ADL 4-6级划为预后差组。结果:预后良好组和预后差组在术前GCS评分、出血量、出血是否破入脑室、术前瞳孔有无散大及手术时机上在统计学上有显著性差异(Plt;0.05),而患者的年龄、性别及出血部位则在统计学上未见显著性差异(Pgt;0.05)。Logistic回归分析,术前GCS评分、出血量、出血是否破入脑室与预后ADL分级相关(Plt;0.05)。其相关系数分别为:-0.456、0.124、0.341。结论:术前GCS评分、出血量、出血是否破入脑室是估计患者手术治疗近期预后有意义的指标,术前瞳孔有无散大、手术时机对评价患者预后有一定的参考价值。了解影响高血压脑出血预后的因素可以更有效的制定治疗方案,估计预后。

  • Analysis of Micro-trauma Technique Cooperated with Liquefaction Drainage Treating 120 Patients with Hypertensive Putamen Hemorrhage

    目的:应用微创液化引流术治疗高血压壳核出血的疗效。方法:对120例高血压壳核出血患者,在CT引导下,依据血肿大小、形态及患者病情,选择适宜的穿刺点、方向,行CT平面导向下微创液化引流术,清除血肿。结果:120例患者出院94例,死亡26例,病死率为21.6%。对存活65例患者随访6个月至2年,以日常生活能力(ADL)评估患者神经功能,ADL128例(43%),ADL220例(30.7%)、ADL310例(15.3%)、ADL45例(7.6%)、ADL52例(3.0%)。结论:应用微创液化引流治疗壳核出血,能最大限度地清除血肿,避免或减轻并发症,具有较大优越性。

  • 健康教育对原发性高血压患者生活方式及疗效的影响

    【摘要】 目的 探讨健康教育对原发性高血压患者生活方式及疗效的影响。方法 2008年6月—2009年5月,将原发性高血压患者随机分为实验组80例和对照组78例,实验组在降压药物治疗的基础上,配合健康教育指导,观察服药依从性、合理饮食、戒烟限酒、控制体重、适量运动等衡量指标;对照组单纯采用药物治疗。随访1年。〖HTH〗结果〖HTSS〗 实验组患者生活方式的改善及血压控制与对照组比较,差异有统计学意义(P<005)。结论 原发性高血压患者在常规药物治疗的基础上配合健康教育指导,可明显改善患者不良的生活方式,提高降压效果,减少用药量,提高患者的生活质量,减少或延缓并发症的发生。

  • Polymorphism of TGF on Genetic Susceptibility to Hypertension: A Meta-analysis

    Objective To explore whether the polymorphism of transforming growth factor β1 (TGF β1) gene at 869T/C and 915G/C loci contributes to the genetic susceptibility to hypertension. Methods Assessed under the same criteria, all case control studies on relationship between the polymorphism of TGF β1 gene and hypertension were searched in both English and Chinese databases. All articles retrieved were screened and evaluated, and meta-analyses were conducted with RevMan 5.1 software. Results A total of 14 case control studies were included. The results of meta-analyses showed TGF β1 gene C allele was related to hypertension (OR=1.37, 95%CI 1.21 to 1.54). It was noted that individuals with CC genotype and TT genotype had a significant increased risk of hypertension (OR=1.43, 95%CI 1.27 to 1.60; OR=0.64, 95%CI 0.53 to 0.78, respectively). And there was no b evidence showing that TGF β1 915G/C genetic polymorphism was related to hypertension. The results from meta-analyses of the studies based on Chinese population on the two loci were in consistent with the outcomes of overall meta-analyses. Sensitivity analyses indicated the results were stable. And publication bias was not present, reflected by P values from Egger’s regression asymmetry test and Begg’s adjusted rank correction test. Conclusions 869T/C polymorphism of TGF β1 gene is associated with hypertension. C allele is potentially one of the genetic risk factors for hypertension. Present studies do not support a direct relationship between 915G/C polymorphism TGF β1 gene and hypertension.

  • The Relationship between Microalbuminuria and Atherosclerosis in Aged Patients with Essential Hypertension

    目的:探讨老年原发性高血压(EH)患者早期肾损害指标微量白蛋白尿(MAU)和动脉粥样硬化的早期征象颈动脉内中膜厚度(C-IMT)之间的关系。方法:99名老年EH患者按尿白蛋白/肌酐比值(ACR)分为异常ACR组和正常ACR组,对两组的C-IMT、斑块发生率、ACR水平及血中尿素氮(Bun)、肌酐(Cr)、总胆固醇(TC)、甘油三酯(TG)、血糖(Glu)、体重指数(BMI)等进行分析和比较。结果:异常ACR组的年龄、尿酸、ACR、C-IMT、24小时平均收缩压,平均动脉压及脉压均高于正常ACR组(Plt;0.05);且C-IMT与ACR水平呈正相关(Plt;0.05)。结论:微量白蛋白尿和颈动脉IMT密切相关,提示微量白蛋白尿不仅和老年高血压肾脏病变有关,也是亚临床期动脉粥样硬化的早期标志.

  • Evidence-Based Treatment for A Patient with Hypertensive Cerebral Hemorrhage

    Objective To explore an approach of evidence-based treatment for acute hypertensive cerebral hemorrhage. Methods (1) Thoroughly evaluating the patient’s condition. (2) Formulating clinical problems. (3) We searched The Cochrane Library (Issue 1, 2006), Ovid EBM REVIEWS (2001 to 2006), MEDLINE (1980 to 2006) and CNKI (1994 to 2006) for evidence. (4) Assessing evidence. (5) Applying evidence. Results Total 13 relevant studies were retrieved. Results indicated that all kinds of interventions for the treatment of hypertensive cerebral hemorrhage showed different degrees of efficacy, including the Stroke Unit, blood pressure control, mannitol, hemostyptic and neuroprotective agents, promotion of blood circulation and resolving of blood stasis, stereotactic aspiration etc. However, the effects of other interventions need further validation except Stroke Units which had higher quality evidence. Based on the patient’ specific conditions, we recommended the following evidence-based treatment plan: immediate transfer to the Stroke Unit; sequential application of reptilase (within 6 h), citicoline (within 24 h), mannitol and Compound Danshen Injection (after 24 h); temporary withhold of enalapril meleate; monitoring of blood pressure, ECG, renal function and electrolytes; and if intracranial hemorrhage occurs again during the treatment, stereotactic aspiration should be applied. Conclusions Through evidence-based method, an individualized treatment plan could obviously improve the treatment effectiveness and reduce the incidence of adverse effects in patients with hypertensive cerebral hemorrhage.
