    目的 总结高频探头超声在副乳腺诊断中的应用价值。 方法 回顾分析2009年1月-2010年12月临床怀疑副乳腺患者38例的高频探头超声声像图表现,同时用超声对副乳腺进行术前定位。 结果 高频探头超声对38例(50处)副乳腺成功定位,并手术切除,其病理组织学检查结果均与超声诊断一致。 结论 高频探头超声在副乳腺诊断中具有操作简便、无创伤、价廉、诊断符合率高等优点,是诊断副乳腺的首选辅助检查方法之一。

  • 高频彩超定性诊断乳腺占位病变的应用价值

    【摘要】目的 评价高频彩超对乳腺占位病变定性诊断的应用价值。方法 总结2004年1月-2009年9月收治的200例经手术病理证实的乳腺占位病变的高频彩超影像特征,并与病理学结果相比较。结果 高频彩超初步定性诊断与病理诊断的符合率为94%(188/200),误诊率为6.0%(12/200)。结论 高频彩超检查有助于定性诊断乳腺占位病变。

  • 综合干预法对减轻胃镜下手术中的反应

    【摘要】 目的 观察运用音乐疗法配合穴位按压的综合干预方法对减轻胃镜下行高频电凝电切术患者术中反应的作用。 方法 2008年10月-2009年12月,将行胃镜下高频电凝电切术的96例患者随机分为两组,对照组给予胃镜下高频电凝电切术常规护理,观察组在常规护理的基础上给予音乐疗法配合手法按压合谷、内关穴。 结果 观察组在术中反应程度及紧张恐惧程度上明显优于对照组(Plt;0.01); 结论 音乐疗法配合穴位按压的综合干预法能减轻胃镜下高频电凝电切术中患者反应程度及紧张恐惧程度。

  • Preliminary study on microdissection needle-assisted ear cartilage reshaping in vivo rabbit models

    ObjectiveTo preliminarily investigate morghological changes of rabbits reshaping ear cartilage assisted by microdissection needle and explore feasibility of new therapy for ear deformity.MethodsThe bilateral ears of 5 male New Zealand rabbits (aged, 5-6 months) were fixed maintaining the curvature and randomly divided into 2 groups (5 ears in each group). The ears were stimulated by microdissection needle in experimental group and were not treated with stimulation in control group. The skin reaction in the experimental group was observed immediately and at 4 weeks after stimulation. Then, the fixtures were removed at 4 weeks, and the shapes of the ears were observed. The cartilages were harvested from the ears to examined morphological changes after HE staining, and measured the chondrocyte layer thickness.ResultsAll rabbits survived until the end of the experiment. The skin has healed completely after 4 weeks in experimental group. After removing fixtures, the ears in the two groups all maintained certain forms momentarily; while 24 hours later, the ears in the control group mostly recovered original form, and the ears in the experimental group still maintained certain molding form until 8 weeks. HE staining showed there were smooth cartilage and uniform distribution of cells in the control group; the matrix staining was basically consistent; and the skin was normal appearance with epidermis, dermis, and cartilage of normal aspect. But the proliferation of chondrocyte with more layers of cells were observed in the experimental group. In addition, there were degeneration and injury of cartilage cells and connective tissue with necrotic cells and inflammatory cells at needle insertion sites. The chondrocyte layer thickness was (385.714±2.027) μm in the control group and (1 594.732±1.872) μm in the experimental group, there was significant difference between the two groups (t=–759.059, P=0.000).ConclusionRabbit ear cartilage can be effectively reshaped by microdissection needle. Proliferation of chondrocyte and changes in matrix can be found during the reshaping process.

  • Recognition of high-frequency steady-state visual evoked potential for brain-computer interface

    Coding with high-frequency stimuli could alleviate the visual fatigue of users generated by the brain-computer interface (BCI) based on steady-state visual evoked potential (SSVEP). It would improve the comfort and safety of the system and has promising applications. However, most of the current advanced SSVEP decoding algorithms were compared and verified on low-frequency SSVEP datasets, and their recognition performance on high-frequency SSVEPs was still unknown. To address the aforementioned issue, electroencephalogram (EEG) data from 20 subjects were collected utilizing a high-frequency SSVEP paradigm. Then, the state-of-the-art SSVEP algorithms were compared, including 2 canonical correlation analysis algorithms, 3 task-related component analysis algorithms, and 1 task discriminant component analysis algorithm. The results indicated that they all could effectively decode high-frequency SSVEPs. Besides, there were differences in the classification performance and algorithms' speed under different conditions. This paper provides a basis for the selection of algorithms for high-frequency SSVEP-BCI, demonstrating its potential utility in developing user-friendly BCI.

  • Clinical application of cone beam CT guided technique in diagnosis of pulmonary nodules

    ObjectiveTo explore the clinical application of the comprehensive guidance technologies, such as cone beam computed tomography (CBCT), virtual bronchoscopic navigation (VBN), and superimposed high-frequency jet ventilator for respiratory control in the biopsy of peripheral pulmonary nodules (PPNs). MethodsThe clinical information of 3 patients with PPNs diagnosed by CBCT combined with VBN and superimposed high frequency superposition jet ventilator in Shanghai Changhai Hospital were retrospectively analyzed. Results Clinical data of 3 patients were collected. The average diameter of PPNs was (25.3±0.3) mm with various locations in left and right lung. The first nodule was located in the apex of the left upper lung, and the biopsy was benign without malignant cells. The lesion was not enlarged during the 5-year follow-up. The second one was located in the left lingual lung, and the postoperative pathology was confirmed as mucosa-associated lymphoma. The third one was located in the anterior segment of the right upper lung. After the failure of endobronchial procedure, percutaneous PPNs biopsy under CBCT combined with VBN was performed, and the pathological diagnosis was confirmed as primary lung adenocarcinoma. Postoperative pneumothorax complication occurred in the third patient with right lung compression rate approximately 20%. ConclusionsThe application of CBCT, combined with VBN and the superimposed high frequency jet ventilator for respiratory control can potentially improve the accuracy and safety in the diagnosis of PPNs. Multi-center clinical trials are needed to verify its further clinical application.

  • Endobronchial Electrocautery Treatment of Tracheobronchial Obstructive Lesions in Inoperable Tracheobronchial Squamous Cell Carcinoma

    Obstractive To observe the clinical effects and safety of endobronchial electrocautery treatment for tracheobronchial obstructive lesions in inoperable tracheobronchial squamous cell carcinoma.Methods Ninety-five patients with advanced and inoperable tracheobronchial squamous cell carcinoma were included. Thirty-four patients with central airway obstruction were treated with endobronchial electrocautery plus chemotherapy ( group A) and 61 patients without central airway obstruction were treated with chemotherapy alone ( group B) . The chemotherapy consisted of cisplatin or carboplatin, plus another thirdgeneration chemotherapy agent. Results In groug A, there were mean improvements in FEV1 of 41. 1% and in peak expiratory flow( PEF) of 65. 6% . There was no significant difference in the survival rates of the patients with and without central airway obstruction. Median survival time of group A was 11. 3 months and those of group B was 11. 6 months. 3, 6, and 12-month survival rates in group A were 87% , 68% and 39% respectively, and those in group B were 93% , 76% , and 45% respectively. Conclusion Endobronchial electrocautery is an effective and safe approach for inoperable tracheobronchial obstructive malignancies with few complications.

  • 心外膜高频刺激心脏植物神经节丛的时间累积和疲劳效应

    目的 观察术中经心外膜对心包内植物神经节丛行高频刺激(HFS)时出现的时间累积效应和疲劳效应,初步探讨其产生的原因和临床意义。 方法 对江苏昆山宗仁卿纪念医院收治的16例行心脏手术患者同期行同步HFS心脏植物神经节丛,刺激时程从50 ms逐渐延长至100 ms及200 ms,观察P-R间期延长和房室传导阻滞的发生情况。 结果 16例患者中2例患者被排除,14例患者均在同步HFS时程为50 ms时出现P-R间期延长,100 ms时出现P-R间期进一步延长,200 ms时出现房室传导阻滞的时间累积效应;4例患者发生迷走效应出现后又再次消失的疲劳现象。 结论 术中经心外膜探查心脏植物神经节丛需重视迷走效应的时间累积和疲劳效应,提高神经节的检出率,并提高心房颤动消融的疗效。

  • Bibliometric Analysis of Literatures on Evidence-Based Medicine in 2002

    Objective To investigate the hot topics of research on evidence-based medicine in 2002. Method To search MEDLINE for papers on evidence-based medicine published in 2002, identify high-frequency subject headings related to research on evidence-based medicine, and cluster the high-frequency subject headings according to rates they appeared in common papers. Results 545 papers, 1 554 subject headings, 30 high-frequency subject headings on evidence-based medicine, and nine clustering categories of high-frequency subject headings were identified through search. Conclusions Both high-frequency subject headings and their clustering categories suggested that “evidence-based practice guidelines and their innovation”, “evidence-based health research and health policy”, “methodology on systematic reviews and randomized clinical trials”, “method of evidence-based decision making and its application in various subjects”, were the hot topics of evidence-based medicine. They provided useful references for Chinese medical professionals to practice evidence-based medicine.
