  • The key elements of technological achievements “production-transformation” in China: a systematic review

    ObjectiveTo systematically review studies on evaluating the quality evaluation index and the transformation application effect evaluation index of domestic related scientific and technological achievements, to analyze and identify the key elements of evaluating the " production-transformation” of scientific and technological achievements, and to provide the methodological support for the establishment of evaluation system for the transformation and application of domestic scientific and technological achievements.MethodsWe searched CNKI, WanFang Data, SinoMed and VIP databases from inception to December 2016. In addition, manual retrieval and tracing references were also used to collect researches. Two reviewers screened literatures, extracted data, and descriptive analysis was performed to assess the results.ResultsA total of 18 studies on the production quality of the technological achievements and 12 studies on the transformation and application effect of the technological achievements were included. The results of systematic reviews showed that: the evaluation of production quality of technological achievements were mainly established from the benefits, characteristics, value, effect and source, and input of technological achievements; the evaluation of transformation and application of technological achievements were mainly established from researching and development ability, transforming ability and transformation effect of technological achievements.ConclusionsThe evaluation of technological achievements covers 2 key links: the production quality, transformation and application effect of technological achievements that are mutually complementary to each other. At the same time, the value elements are found in the production quality, transformation and application of technological achievements, namely scientific value, discipline value, economic value and social value. Therefore it is necessary to establish the evaluation of production quality, transformation and application effect of technological achievements based on the value.

  • Risk factors associated with prediabetes in Chinese: a meta-analysis

    ObjectivesTo systematically review the risk factors associated with prediabetes, and to provide reference for prevention and treatment of diabetes mellitus and health education.MethodsPubMed, EMbase, SinoMed, CNKI, WanFang Data and VIP databases were electronically searched to collect case-control studies on the risk factors associated with prediabetes from inception to February 20th, 2018. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies, then, meta-analysis was performed by using RevMan 5.3 software.ResultsA total of 13 studies involving 49 168 patients were included. The results of meta-analysis showed that: drinking (OR=1.31, 95%CI 1.04 to 1.65, P=0.02), hypertension (OR=1.75, 95%CI 1.11 to 2.76, P=0.02), abdominal obesity (OR=1.26, 95%CI 1.12 to 1.42, P=0.000 1), and waist to hip ratio (OR=1.77, 95%CI 1.27 to 2.45, P=0.000 6) might be the risk factors for prediabetes. However, dyslipidemia, smoking, physical activity, family history of diabetes, body mass index and waist circumference might not be risk factors for prediabetes.ConclusionsCurrent evidence shows that drinking, hypertension, abdominal obesity and waist-hip ratio may be the influencing factors of pre-diabetes, while other related risk factors are required to be further studied. Due to limited quality and quantity of the included studies, more high quality studies are required to verify above conclusions.

  • Effectiveness of free fascia lata flap assisted by indocyanine green angiography in treatment of Myerson type Ⅱ and Ⅲ chronic Achilles tendon ruptures

    Objective To investigate of effectiveness of free fascia lata flap assisted by indocyanine green angiography (ICGA) in treatment of Myerson type Ⅱ and Ⅲ chronic Achilles tendon ruptures. Methods A clinical data of 14 patients with Myerson type Ⅱ and Ⅲ chronic Achilles tendon ruptures between March 2020 and June 2024 was retrospectively analyzed. There were 12 males and 2 females with an average age of 45.4 years (range, 26-71 years). The causes of Achilles tendon rupture included sports injury in 10 cases, Achilles tendon-related tendinopathy in 3 cases, and glass laceration injury in 1 case. The time from Achilles tendon rupture to operation was 4-40 weeks (median, 4.5 weeks). Preoperative MRI examination showed that the defect length of the Achilles tendon was 2-5 cm (mean, 3.2 cm). The operation time and intraoperative blood loss were recorded. The color Doppler ultrasound (CDU) and MRI were taken to observe the foot blood vessels and the tendon healing. The visual analogue scale (VAS) score, American Orthopaedic Foot and Ankle Society (AOFAS) score, Achilles Tendon rupture score (ATRS), and range of motion of the ankle joint were used to estimate the ankle joint fuction. Results All operations of the 14 patients were successfully completed. The operation time ranged from 3.00 to 4.50 hours (mean, 3.60 hours). The intraoperative blood loss ranged from 10 to 50 mL (mean, 36.4 mL). After operation, 1 patient had exudation at the recipient site, which healed after dressing change; the other incisions healed by first intention. All incisions at the donor sites healed by first intention. All patients were followed up 6-36 months (mean, 11.4 months). The CDU of the foot at 1 month after operation showed that the blood flow signal of the perforating vessels of the fascia lata flap was clear. The ankle MRI at 2 months after operation showed that the good continuity of the Achilles tendon. No complications such as the Achilles tendon re-rupture, ankle stiffness, or scar contracture occurred during follow-up. Compared with preoperative score, the AOFAS score, ATRS score, and plantar flexion range of motion significantly increased at 1, 3, and 6 months after operation (P<0.05), while the pain VAS score and dorsiflexion range of motion significantly decreased (P<0.05). The AOFAS score, ATRS score, and VAS score at 3 and 6 months were further improved compared with those at 1 month (P<0.05); however, there was no significant difference in the range of motion of the ankle joint (P>0.05). There was no significant difference in above indicators between 3 and 6 months after operation (P>0.05). Conclusion The treatment of Myerson type Ⅱ and Ⅲ chronic Achilles tendon ruptures with free fascia lata flaps under the guidance of ICGA has the advantages of precise design, fast healing, and a wide range of adaptability.

  • The status of registrations, ethical reviews and informed consent forms in RCTs of high impact factor Chinese medical journals

    ObjectivesTo investigate the status of clinical trial registrations (CTRs), ethical reviews (ERs) and informed consent forms (ICFs) of randomized controlled trials (RCTs), which were published in the top 20 journals included by Chinese Science Citation Database (CSCD), and evaluate the methodological quality of RCTs.MethodsWe selected top 20 high impact factor Chinese medical journals indexed by CSCD, the RCTs published between 2016 and 2017 were searched. The CTR, ER and ICFs for all RCTs were analyzed.ResultsA total of 395 RCTs were included for data analysis. Nineteen RCTs (4.8%) reported the information of registration. For the 4 traditional Chinese medicine journals, only 4 (1.9%) of 207 RCTs reported the registered information. For the 16 western medical journals, only 15 (8%) of 188 RCTs reported the registered information. There were 185 RCTs (46.8%) which reported the information of ethical review. Among them, 66 RCTs (31.9%) were published in traditional Chinese medical journals and 119 RCTs (63.3%) were published in western medical journals. There were 253 RCTs (64.1%) which reported the information of informed consent. Among them, 154 RCTs (74.4%) were published in Chinese medical journals and 199 RCTs (52.7%) were in western medical journals. In terms of methodology, approximately 299 RCTs (75.7%) reported the type of randomization where 60 RCTs (15.2%) reported the information of allocation concealment and 38 RCTs (59.6%) reported blinding.ConclusionsThe proportion of RCTs registration, ethical review and informed consent is still low and the methodological quality of the studies require to be improved. Therefore, it is necessary to strengthen the training of clinical trial methodology.

  • Current status of health economics reports on clinical practice guidelines and expert consensus in China from 2021 to 2023

    ObjectiveTo systematically investigate the current status of reporting health economics evidence in clinical practice guidelines and expert consensuses published in China from 2021 to 2023, providing references for the formulation and revision of guidelines and consensuses in our country. MethodsComputer searches were conducted in the CNKI, CBM, WanFang Data, China Academic Journals Full-text Database, PubMed, and Web of Science to collect clinical practice guidelines and expert consensuses published in China from 2021 to 2023. Two researchers independently screened the literature, extracted information on the inclusion of economic evidence in guidelines and consensuses, and then used quantitative analysis methods for description. ResultsA total of 4 236 relevant articles were included, of which 1 066 (25.17%) reported health economics evidence; 120 (11.26%) reported health economics evidence in the formation of recommendation opinions; 109 (10.23%) reported health economics evidence in the grading of evidence quality; 832 (78.05%) reported health economics evidence in the interpretation and explanation of recommendation opinions. ConclusionThe reporting rate of health economics evidence in clinical practice guidelines and expert consensuses published in China is not high. The reporting rate of health economics evidence in consensuses is lower than that in guidelines. It is recommended that during the formulation process of guidelines and consensuses, the application of health economics evidence should be further strengthened in aspects such as the formation of recommendation opinions, the grading of evidence quality, and the interpretation and explanation of recommendation opinions, in order to improve the scientific, rigorous, and applicability of clinical practice guidelines and expert consensuses, and to play the role of guidelines and consensuses in optimizing the allocation of health resources, improving clinical diagnosis and treatment effects, and enhancing the quality of medical care.
