  • REPAIR OF LARGE BONE DEFECT DUE TO EXCISION OF BONE TUMOR WITH BONE ALLOGRAFT

    OBJECTIVE: To study the reparative and reconstructive methods for the large bone defect due to the excision of bone tumor. METHODS: According to the size and shape of the bone defect, we selected the proper bone and joint or manipulated bone segment of the profound hypothermia freezing allograft and gave locked intramedullary nails or steel plate and screws for stable internal fixation. RESULTS: In the 22 cases, 20 survived without tumor and 2 died. One patient treated with the allograft of semi-knee joint was found rejection. Then the wound did not heal. After the skin flap grafting was performed, the wound still did not heal, so the patient accepted amputation(4.5%). In the other 21 cases, the X-ray and 99mTc SPECT showed some callus or concentration of nuclein which implied bone union. According to Markin bone graft criterion, the excellent rate of function recovery was 81.8%. CONCLUSION: Allografting of bone and joint is a good and workable method in repairing and reconstructing the bone defect due to the excision of bone tumor. It should be further studied and be applied.

  • TREATMENT OF COMPLEX ACETABULAR FRACTURES BY A MODIFIED EXTENDED ILIOFEMORAL APPROACH

    Objective To evaluate the effect of the modified extended iliofemoral approach on treatingcomplex acetabular fractures. Methods Thirty-six cases of complex acetabular fractures were treated by the open reposition and internal fixation by a modified extended iliofemoral approach. Results Thirty-sixcases were followed up for 7-46 months, with an average of 23.8 months. According to the Matta standard, anatomical reposition was performed in 24 cases, perfect reposition in 8 cases, and unsatisfactory reposition in 4 cases. By the modified d’Aubignepostel score, among the 36 cases, 22 had an excellent result, 9 had a good result, and 5 had a poor result. Conclusion Themodified extended iliofemoral approach facilitates the operative exposure of the anterior and posterior walls and both columns of the acetabulum in the surgically-treated acetabular fractures by the open reposition and internal fixation.Because of the reconstruction, the functions of the abductor muscle mass managed by the lagscrew-fixed osteotomies of the iliac crest, and greater trochanter, the patients can achieve a rapid rehabilitation of the joint.

  • REBUILDING OF SCROTUM IN GANGRENE

    Objective To study diagnosis, treatment and rebuilding of scrotum gangrene. Methods From January 1992 to September 2004, 15 patients with scrotum gangrene were treated and their clinical data were analysed.Their ages ranged from 23 to80 years. The results of bacterial culture were positive for wound sample in 14 cases and for blood in 1 case. All the patients underwent surgical treatment including incision,aggressive debridement,drainage,irrigation and antibiotic therapy. Two patients accepted hyperbaric oxygen therapy. All the patients received scrotum rebuilding by transfer of skin flap, skin grafting and suturation and orchectomy was given in 2 elder patients. Results All the patients received healing by first intention after 21 to 34 days.There was no death. Aftera follow-up of 1 to 3 years, the appearance of scotum was satisfactory and no orchiatrophy occurred. Thirteen patients with testicles had normal sexual function. Conclusion After a definite diagnosis,early aggressive debridement,broad-spectrum antibiotics and sufficient local drainage should be used. If available,hyperbaric oxygen therapy may be used to promote healing of tissue wound. Scrotum should be rebuilt based on different conditions.

  • REPAIR AND RECONSTUCTION OF ORAL AND MAXILLOFACIAL DEFECT——CLINICAL ANALYSIS OF 1 973CASES

    Objective To compare the reconstructive method of oral and maxillofacial defect with free tissue flaps. Methods The clinical materials were collected from 1 973 reconstructive cases between January 2000 and June 2004 and analyzed in terms of the distribution of age, gender,disease type, defect location, reconstructive method and the incidence of vascular crisis of free flaps as well as success rate of free flap respectively. SAS 6.12 was adopted for statistical analysis. Results A total of 1 973reconstructive cases includded 764 in middle age (>45 years to ≤60 years, 38.72%), 527 in old age (>60 years, 26.71%), 450 young adults (>28 years to ≤45 years, 2281%), 187 in young age (>14 years to ≤28 years, 9.48%) and 45 children(≤14 years, 2.28%). The ratio of male to female was 1.5∶1. The ratio of benign to malignancy lesion was 1∶1.94. The tongue defect accounted for 20.63%, followed by mandibular defect(1738%), parotid defect(13.74%),buccal defect(12.72%), maxillary defect(8.16%), oral pharynx defect (7.60%), floor of mouth defect(5.68%) and others (14.09%). Vascular free flap transfers accounted for 45.82%(90.4), followed by axial flap(38.17%,753), random flap(10.19%,201), avascularizedbone graft (1.52%, 30) and others(4.30%, 85). The most frequently used flap was the forearm flap(594 cases), followed by the fibula free flap(143 cases) and the pedicled pectoralis major myocutaneous flap(369 cases); these three flaps accounted for 56.06% (1 106/1 973).In 47 free tissue flaps(5.20%) having vascular crisis, 30 were saved (63.83%). The success rate of total free tissue flaps was 98.19%(923/940). Conclusion The majority of reconstructive cases of oral and maxillofacial defects is the middle aged andthe old aged male patients with malignancy. The tongue defect accounts for about one fifth of all the cases. The vascularized free flap has a high success rate, so it is a main method for reconstruction of oral and maxillofacial defects. The forearm flap, the fibular free flap and the pedicled pectoralis major myocutaneous flap are the main management for repairing oral and maxillofacial defects.

  • CLINICAL OBSERVATION OF TRAUMATICALLYDAMAGED JOINT AFTER ITS REPAIR WITH TRANSPLANTATION OF ALLOGENIC JOINT

    Objective To evaluate the long-term function of the traumaticallydamaged joint after its repair with transplantation of a fresh or a frozen allogenic joint. Methods From March 1977 to September 1993, 13 patients (9 males, 4females; age, 17-55 years) with traumatically-damaged joints underwent transplantation of the fresh or the frozen allogenic joints. Five patients had 5 damagedmetacarpophalangeal joints, 6 patients had 9 damaged interphalangeal joints, and 2 patients had 2 damaged elbow joints. So, the traumatic damage involved 13 patients and 16 joints. All the metacarpophalangeal joints and the interphalangeal joints were injured by machines and the 2 elbow joints were injured by road accidents. The patients were randomly divided into 2 groups: Group A (n=7) andGroup B (n=6). The 7patients with 8 joints in Group A underwent transplantation of fresh allogenic joints; the 6 patients with 8 joints in Group B underwent transplantation of frozen allogenic joints. The allogenic joint transplants were performed in the period from immediately after the injuries to 6 months after the injuries. The motion ranges of the transplanted joints and the X-ray films were examined after operation, and the immunological examination was performed at 8 weeksafter operation. Results The time for synostosis was 5-8 months in Group A, but4-6 months in Group B. In Group A, at 2 years after operation the metacarpophalangeal flexion was 30-40° and the interphalangeal flexion was 20-30°; however,at 6 or 7 years after operation the interphalangeal flexion was only 10-20°. The patients undergoing the transplantation with fresh elbow joints had the elbowflexion of 60° and the elbow extension of 0°, and had the forearm pronation of 30°and the forearm supination of 30°. But in Group B, at 2 years after operation the metacarpophalangeal flexion was 6070° and the interphalangeal flexionwas 40-50°; at 6 or 7 years after operation the interphalangeal flexion was still 40-50°. However, the patients undergoing the transplantation with frozen elbow joints had the elbow flexion of 90° and the elbow extension of 0°, and hadthe forearm pronation of 45° and a forearm supination of 45°. The joint motion ranges, the Xray findings, and the immunological results in the patients undergoing the transplantation of the frozen allogenic joints were significantly better than those in the patients undergoing the transplantation of fresh allogenicjoints. There was a significant difference in the immunological examination between Group A and Group B (IL2, 21.64±3.99;CD4/CD8,3.88±0.82 vs.IL-2,16.63±3.11;CD4/CD8, 2.53±0.23, P<0.01). Conclusion Repairing the traumatically-damaged joints with frozen allogenic joints is a better method of regaining the contour, movement, and complex motion of the hands. 

  • HYPOPHARYNX RECONSTRUCTION OF DEFECTS AFTER OPERATION ON ADVANCED PYRIFORM SINUSCANCER WITH REMAINING LARYNGEAL MUCOSA FLAP AND PECTORALIS MAJOR MYOCUTANEOUS FLAP

    Objective To explore the outcome of hypopharynx reconstruction by using remaining laryngeal mucosa flap and pectoralis major myocutaneous flap in advanced pyriform sinus cancer.Methods Twelve patients with pyriform sinus cancers underwent hemilaryngectomy and partial pharyngectomy, two patients underwentcervical esophagectomy at the same time. The defects were reconstructed by remaining laryngeal mucosa flap. Four cases were involved in the bilateral larynx, received total laryngectomy and were repaired by pectoralis major myocutaneous flap. Results There was no operative fatal case and all flaps survived. Only one suffered from postoperative pharyngocutaneous fistulas, whose defect was reconstructed by remaining laryngeal mucosa flap and had radiotherapy. All patients could swallow ordinary food and had no benign esophagostenosis and pharyngostenosis after operation. Out of 16 pateints, 1 case died of general metastasis;3 cases died of local tumor relapse, tumor relapse of cervical lymphonode and lung metastasis respectively within 1 year after operation; the other 12 casessurvived over 2 years.Conclusion The advantage of hypopharynx reconstruction with remaining laryngeal mucosa flap is simple and convenient with less trauma and complication. The reconstruction should be completed by using the pectoralis major myocutaneous flap when the bilateral larynx are involved in.

  • OPERATIVE TREATMENT OF MONTEGGIAS FRACTURE IN CHILDREN

    From 1980 to 1992, 62 cases of Monteggias fracture in children were treated byoperation, in which were 18 cases were fresh, 44 old. The following procedure performed separately: the reduction of the head of radius; the use of internal fixation following anatomical reduction of the ulna or the osteotomy of the ulna and elongation; the repair or reconstruction of annular ligament of the superior radioular joint. From the last follwup, 93 percent was excellent in the clinical result. The operative method was introduced briefly. The importance of earlydiagnosis and prompt effective treatment of Monteggias fracture in children was discussed. The key to prevent redislocation of the head of radius was that rigid fixation of ulna and repair or reconstruction of the annular ligament of radius were important. It is also important to improve curative effect that early functional movement after operation was mandatory for good operative results.

  • RECONSTRUCTION OF METACARPOPHALANGEAL JOINT DEFECT BY THE SECOND TOE PROXIMAL INTERPHALANGEAL JOINT WITH SKIN FLAPS

    Objective To invest igate the operat ive method and cl inical ef f icacy of reconstruct ing metacarpophalangeal joint defect by the second toe proximal interphalangeal joint with skin flaps. Methods From March 2003 to January 2008, 26 cases (26 fingers) with metacarpophalangeal joint defect were treated, including 19 males and 7 females aged 18-36 years old (average 27 years old). Among them, 23 cases were caused by mechanical injury and the time from injury to operation was 1-6 hours; while 3 cases suffered from secondary injury due to trauma and the time from injury to operation was 3-12 months. Four thumbs, 10 index fingers, 8 middle fingers, 3 ring fingers and 1 l ittle finger were injured.The metacarpophalangeal joint defects were 2 cm × 1 cm-4 cm × 2 cm in size, and 22 cases were combined with skin and soft tissue defect (1.5 cm × 1.5 cm - 6.0 cm × 5.0 cm). During operation, the second toe proximal interphalangeal joint with skin flaps was transplanted to reconstruct those defects, 20 fingers received whole-joint transplantation and 6 fingers received halfjoint transplantation. The skin flaps ranging from 2.0 cm × 1.5 cm to 6.5 cm × 6.0 cm in size were adopted. The donor site of 21 cases received toe amputation, and the rest 5 cases received joint fusion. Results The transplanted joints and skin flaps of all the 26 fingers survived. All incisions and donor sites healed by first intention. All patients were followed up for 6-20 months (average 12 months). The union of transplanted joints was achieved in all the cases at 6-12 weeks after operation, no bone nonunion and refracture occurred. The flexion range of transplanted metacarpophalangeal joints was 30-75° (average 45°). Joint activity was evaluated according to the total active movement/total passive movement assessment criteria, 8 fingers were excellent, 13 good, 3 fair, 2 poor, and the excellent and good rate was 80.77%. The foot donor-site abil ity to walk was unaffected. Conclusion Applying second toe proximal interphalangeal joint with skin flaps is an effective approach to reconstruct the metacarpophalangeal joint defect, and the function recovery of the injured joints is satisfying.

  • REPAIR AND RECONSTRUCTION OF POST-TRAUMATIC INSTABILITY OF THE ANKLE

    OBJECTIVE: To investigate the reparative and reconstructive method of post-traumatic lateral instability of the ankle. METHODS: From January 1992 to June 2000, 7 cases of male patients with ankle injury (aged 25-43 years) underwent Wetson-Jones modification. A bone tunnel was drilled through 2.5 cm upside the lateral malleolus tip and talus, and short peroneal tendon was cut to pass the tunnel to fix twining. RESULTS: All patients were followed up 2-10 years, 5 cases mineworkers changed to work on the ground, 2 patients returned to the original work, no traumatic arthritis occurred. According to Baird ankle joint scoring standard, the ankle function was excellent in all cases. CONCLUSION: Reformed Wetson-Jones modification can repair and reconstruct perfectly the traumatic instability of the ankle.

  • PROGRESS in surgical MANAGEMENT OF PROGRESSIVE HEMIFACIAL ATROPHY

    ObjectiveTo summarize recent progress in surgical management of progressive hemifacial atrophy (PHA), to analyse the key features of various methods of treatment, and to define subjects worthy of further researches. MethodsThe publications concerning the etiology and surgical management of PHA were reviewed, analyzed, and summarized. ResultsSurgical management serves as the primary treatment, including flap transposition, tissue flap reconstruction, free tissue grafting, prosthetic implants, and other surgical treatments. Each method has its own advantages and limitations. At present, comprehensive treatment are considered to be the most commonly method for PHA. ConclusionThe combined use of various surgical methods is the trend of surgical management of PHA; effective treatments specific to the etiology and minimally invasive surgical methods are still to be developed.
