Purpose To evaluate the surgical effect of idiopathic macular hole (IMH),with optical coherence tomography (OCT). Methods To measure the diameter of holes and hole halos,the thickness of hole in edge,the changes of hole pattern in 19 cases (19 eyes) with IMH using OCT. Results By quantitatively measuring,OCT showed that after surgery the mean hole diameter lessened from (570.95plusmn;264.59 )mu;m to (337.05plusmn;335.95)mu;m.The mean diameter of hole halo lessened from (1043.53plusmn;278.80)mu;m to (695.00plusmn;483.00)mu;m and the mean thickness of the hole edge thinned from (389.78plusmn;60.58)mu;m to (298.78plusmn;109.80)mu;m.In 17 eyes the holes become closed or reduced in size or the hole halo,and thinned in hole edge.The anatomic successful rate of the surgery is 89%. Conclusion OCT can objectively,accurately and effectively judge the anatomic effect of IMH surgery. (Chin J Ocul Fundus Dis,2000,16:71-138)
Objective To investigate the histopathologic charact eristic of the vitreous herniation out of sclerotomy site during vitrectomy. Methods Twenty specimens of tissues herniated at vitrectomy site were collected. The paraffin sections or fresh smears were stained with hematoxylineosin and examined under light microscope. The specimens were collected from the affected eyes with rhegmatogenous retinal detachment (9 cases), traumatic retinal detachment (1 case), miscellaneous vitreous hemorrhage (6 cases) and intraocular foreign body (4 cases). Results The herniated tissues were found to be retina in 4 cases, ciliary tissue in 1 case, retina and ciliary tissue in 1 case, uvea in 1 case, and hyaloid tissue in 13 cases. Conclusion There were not only vitreous, ciliary epithelial cells and pigment containe depithelia, but also ciliary body, retina and uvea in the prolapsed tissues of sclerotomy site, which might be related to the occurence of some clinical complications. OBJECTIVE:To verify the safe dose of cephradine in intravitreal injection. METHODS:After injecting different doses of cephradine(100mu;g,200mu;g,250mu;g,300mu;g,400mu;g)into vitreous cavity of different group of rabbits the activities of the retinal enzymes (SDH,LDH )on different time (Id,3d, 7d ) were determined respectively, and the histological and ultrastructural changes of retinas were also observed simuhaneously. RESULTS:The activity of rellnal SDH and LDH was found to be decreased gradually with tbe icreasing of the dosage of intravitreal cephradine. The activities of SDH and LDH were found in the lowest level on tile 3rd and lsl day,but they recover to normal levels on tile 7th day after intravitreal in}eetion in 100mu;g,200mu;g groups,and still lower tban normal in the other groups. Histologically,retinal edema was found both in 100mu;g and 200mu;g groups,but degradation of retinal cells,and loss of cones and rods were round in the 250mu;g, 300mu;g and 400mu;g groups. Objective To describe cultured human retinal pigment epithelial (RPE) cells transdifferentiation and investigate the effects of human vitreous fluid on the morphologic and cytoskeleton changes of RPE cells in vitro. Methods Cytoskeleton characteristics in the 2nd, 5th, 8th passage of RPE cells in normal culture, which included cytokeratin 18 (CK18) and α-smooth muscle actin (α-SMA) were analyzed by Western blot. RPE cells were cultured in human vitreous-conditioned medium (VCM) at the concentration of 1∶4 for 6 days, morphologic changes were examined by light and electron microscopy, and cytoskeleton characteristics were analyzed by imunocytochemistry and Western blot. Results During culture in vitro, RPE cells lost epithelial characteristics and aquired fibroblast-like phenotype. The expression of CK18 was the highest at the 5th passage, and it decreased in the following passage, but α-SMA increased gradually. The morphologic transdifferentiation from epithelial to fibroblast-like cells of RPE was accelerated by VCM. Ultrastructural changes such as decreased microvilli and gradually increased rough endoplasmic reticulum and Golgi complex were found during the cultivation. CK18 produced by RPE cells decreased in VMC (P<0.05), and α-SMA increased (P<0.01). Conclusion Morphologic changes in epithelialmesenchymal transdifferenetiation of RPE cells are stimulated by VCM and accomplied by the shift of cytoskeleton proteins, The results imply that cells migration may be decreased and contraction may be enhanced in VCM. It may suggest that vitreous accelerates the pathogenesis of PVR and RPE cells play an important role. (Chin J Ocul Fundus Dis, 2002, 18: 289-292)
Objective To prepare a new glycoproteinopticin specific antibody and to explore the distribution of opticin in human eye. Methods Firstly, take the opticin specific antibody to compound a synthetic peptide chain(CLPRLPIGRFT), and then get the opticin antibody. To verify the availability of antibody through the western blot for human vitreous extract, to test the distribution of opticin in human eye by immunohistochemistry. Results Through the western blot for human vitreous extract, we can see a band with wild range at molecular weight 45times;103~50 times;103. We find that opticin exact in retina, vitreous and non-pigmented epithelium of ciliary body which distributes along the collagen fibrils in vitreous. Conclusion The availability of the antibody was confirmed by western blot. Opticin are mainly in retina, vitreous and nonpigmented epithelium of ciliary body. Opticin distributes along the collagen fibrils which may be related to the stability of vitreous. (Chin J Ocul Fundus Dis,2008,24:286-288)
PURPOSE: To investigate the treatment of severe bacterial endophthalmitis. METHODS:The curative effects of vitrectomy after intravitreal antibiotics and steroids (IVAS)for the treatment of 23 patients with bacterial endophthalmitis (group I)and vitrectomy and IVA at the same time for the treatment of 28 patients with bacterial endopbthalmitis (group I)were analyzed retrospectively. RESULTS: The rate of curative effects of two groups were similar,while the marked curative effects in group I (47.8% )was significantly higher than that of the group I (17.9%). The average period of eliminating infiamation of group I was longer than that of group I , and the incidence of postoperative retinal detachment of group Ⅱ was 3 times more than that of group I . CONCLUSION :It was indicated that vitrectomy after IVAS may increase the security of vitrectomy and the curative effects of severe bacterial ndophthalmitis.
ObjectiveTo investigate the relationship between the posterior vitreous detachment (PVD) and the diabetic macular edema (DME).MethodsA total of 169 eyes with DME which were diagnosed by funduscopy,fundus fluorescein angiography (FFA) and optical coherence tomography (OCT) were retrospectively analysed. PVD and partial PVD were detected by OCT and biomicroscopy. The characterizations of OCT images of DME were analysed.ResultsIn 169 eyes, 156 (92.3%) had no PVD with the average thickness of fovea of 297 μm; 11 (6.5%) had complete PVD with the average thickness of fovea of 229 μm; 2 (1.2%) had partial PVD with the average thickness of fovea of 347 μm.ConclusionsOnly a few PVD were found in DME eyes, so traction of PVD may not be the main cause of DME. Objective To investigate the effect of tetrandrine (Tet) on experimental choroidal neovascularization and the effect of Tet on retinal structure and function. Methods Choroidal neovascularization was induced in 20 Brown Norway (BN) rats (40 eyes) by diode laser (wavelength: 810 nm; exposal time: 0.1 second; facular diameter:100 mu;m; energy: 120 mW), and the rats were divided randomly into experimental and control group with 10 rats (20 eyes) in each group. In experimental group, 0.05 ml Tet with the concentration of 3.21 mu;mol/L was injected intravitreously 0 and 3 days after laser photocoagulation; in the control group, the rats underwent an intravitreous injection with the same volume of sodium chloride solution. The incidence of CNV was evaluated by fundus fluorescein angiography (FFA) 14 days after laser photocoagulation. Five right eyes of another Five healthy BN rats underwent intravitreous injection with 0.05 ml Tet with the concentration of 3.21 mu;mol/L, and an intravitreous injection with the same volume of sodium chloride solution was performed on the left eyes. Before injection, 1 hour, and 1 day after the first injection, and 1 hour, 1 day, 7 days, 14 days after e second injection the electroretinography (ERG) was performed on these 5 rats; 14 days after the second injection, the retinae were examined by light microscopy and transmission electron microscopy. Results The incidence of CNV was 23.26% in experimental group,which was obviously lower than that in the control group (63.33%) (Plt;0.01). The ratio of amplitude of b wave of ERG in the rats undergone intravitreous injection with 3.21 mg/ml Tet didnprime;t differ much from which before the injection (Pgt;0.05). There were no structural changes of retinal tissues examined by light and electron microscopy. Conclusion Tet may inhibit choroidal neovascularization in rats; there isnprime;t any significant toxic effect of intravitreous injection with Tet on retina at the dosage of 3.21 mu;mol/L. Objective To review the distribution and shifting trends of cultured bacteria from the aqueous humor and the vitreous body. Methods A retrospective analysis on distribution of Gram′s stain, the distribution and change of isolates was performed in 522 specimens (aqueous humor,261 and vitreous body,261) of patients with suspected endophthalmitis during a 10-year period (1989-1998). Results The positive cultures were 119 (aqueous humor,44 and vitreous body,75) of 522 specimens. The average positive rate was 22.8%. Gram-positive cocci constituting 45.4%(54) of total isolates followed by Gram-negative bacilli,34.5%(41);Gram-positive bacilli, 20.2%(24). In the positive bacterial cultures, enterobacteriaceae was the most common isolate, 18.5%, and the next was micrococcus, 16.0%; coagulase-negative staphylococcus,12.6%; and pseudomonas,10.9%.Comparing the data from 1989 through 1993 with the data from 1994 through 1998, the frequency of Gram-positive cocci had no significant change, while the frequency of Gram-positive bacilli was decreased and the percentage of Gram′s-negative bacilli was increased. Conclusions Gram-positive cocci and Gram-negative bacilli are the predominant pathogens of bacterial endophthalmitis. The percentage of Gram′s-negative bacilli has increased for 5 years. It is very important to comprehend the distribution and shifting trends of these pathogenic bacteria for diagnosis, prevention and treatment of bacterial endophthalmitis. Vitreous is composed of transparent gel-like tissue with complex molecular structure, accounting for 80% of the volume of the eyeball. In addition to support the eye structure, vitreous has important functions such as regulating eye development, refraction and cellular barriers, participating in intraocular oxygen metabolism and the development of retinal diseases. Age, myopia and systemic diseases and other factors have an impact on vitreous function. Intra-vitreous injection and vitreous surgery may interfere with vitreous function, damage the intraocular metabolic balance. To better maintain intraocular metabolic balance and visual function, it is important to study vitreous metabolic function carefully and intervene it cautiously.
