  • The expression of hypoxia inducible factor -1αin embryonic and postnatal rats′retinae

    Objective To investigate the expression of hypoxia inducible factor 1(HIF1alpha;) in ratsprime; retinae during the embryonic and earlier postnatal period. Methods The retinal expression patterns of HIF-1alpha; protein and mRNA of embryonic day 12 (E12), E16, E20, and postnatal day 1(P1) and P5 rats were determined by immunohistochemical staining and reverse transcriptionpolymerase chain reaction (RT-PCR). Results HIF-1alpha; protein was detected in the neural epithelial layer and the pigment epithelial layer at all those 5 timepoints, with higher expression in the ganglion cell layer and the inner plexiform layer, and seems limited to the ganglion cell layer when re tina became more mature. Embryonic rat retina had higher expression of HIF-1alpha; protein and mRNA than postnatal retina, the difference was significant (P<0.01). Conclusion The expression of HIF1alpha; in ratsprime;retina e differs from embryonic to earlier postnatal stages. 

  • The Value of Selecting Appropriate Delay Time in Computed Tomography Urography Based on the Type of Delayed Imaging

    ObjectiveTo analyze the relationship between image development types and delay time in computed tomography urography (CTU) examination and to investigate the value of choosing reasonable delay time to get the best images. MethodWe collected the data of 62 patients who underwent multi-slice CTU examination without fully developed images within 8 minutes from January 2010 to May 2014. There were 35 unilateral and 27 bilateral delayed imaging patients with a total of 89 cases. According to the degree of imaging development after the delayed 8 minutes, we divided it into 4 levels as follows:level 0 (no development), levelⅠ (light development), levelⅡ (partial development) and levelⅢ (full development). LevelⅢ was not included in this study. For level 0 to levelⅡ, we delayed scan for 15 to 1 440 minutes. ResultsAs the imaging development degree decreased, the delay time obviously extended. LevelⅡ could be fully displayed after 15 to 150 minutes of delayed scan. LevelⅠ could be fully displayed after 150 to 720 minutes. Level 0 still had no imaging development after 120 minutes, so there was no need to continue scanning. ConclusionsSelecting appropriate delay time of CTU based on the type of delayed imaging development can avoid unnecessary repeated delay scans, get the best development images, and estimate the kidney secretion functions, which is most helpful for accurate diagnosis of urinary system related diseases.

  • Morphological development of premature retina: a study with an indirect ophthalmoscopelinked imaging system

    Objective To observe the characteristics of morphological development of premature retina at 33-46 weeks of gestational corrected age (GCA). Methods A total of 268 premature infants were divided into 7 groups according to the GCA (33-34,35-36,37-38,39-40,41-42,43-44 and 45-46 weeks). The ocular fundus of those infants were recorded and analyzed by an indirect ophthalmoscopelinked imaging system. Results As GCA increases, noticeable macular morphological changes occurred and recorded in 96% of infant at 45-46 weeks of GCA. Retinas were gradually vascularized at 41-42 weeks (nasal retina) or 43-44 weeks (area Ⅲ,temporal retina), and pigmented in 84% of infant at 45-46 weeks of GCA. Conclusion Macular morphological patterns, retinal blood vessels and pigments continue to develop in postnatal premature infants.

  • SWOT model analysis and innovative strategies for high quality development of pediatric day-care surgery in comprehensive hospitals

    The developing status of pediatric day-care surgery is comprehensively analyzed based on SWOT model from the following four dimensions: strength (S), weakness (W), opportunities (O) and threats (T). Day-care surgery strategy is in line with the characteristics of pediatric diseases, which can improve medical efficiency and reduce medical costs. However, it is limited by both the particularity of pediatric patients and the complexity of postoperative management. In terms of external environment at present, there are many opportunities for its development of day-care surgery such as national policy support, medical technology innovation, and regional medical center construction. Nevertheless, some threats cannot yet be ignored such as medical safety risks, unfair resource allocation, traditional habit of medical treatment and medical insurance payment policies. In order to promote the high-quality development of pediatric day-care surgery in the future, it is recommended to innovate medical service models, improve medical quality management systems, train medical qualified personnel, and enhance national policy support.

  • A study of total hip arthroplasty with subtrochanteric osteotomy in Crowe type Ⅳ developmental dysplasia of hip

    ObjectiveTo evaluate the effectiveness of total hip arthroplasty (THA) combined with subtrochanteric osteotomy in the treatment of Crowe type Ⅳdevelopmental dysplasia of the hip (DDH).MethodsBetween April 2008 and June 2016, 71 patients with unilateral Crowe type Ⅳ DDH were treated with THA. Of 71 cases, 44 were performed with subtrochanteric osteotomy (osteotomy group) and 27 were performed without subtrochanteric osteotomy (non-osteotomy group). There was no significant difference in gender, age, body mass, height, body mass index, affected side, and preoperative Harris score between 2 groups (P>0.05). The complications were recorded and the effectiveness was assessed by Harris score. Besides, the femoral dislocation height and the settling depth of sleeve were measured in the pelvic anteroposterior X-ray film pre- and post-operatively.ResultsOsteotomy group was followed up 12-90 months (mean. 34.77 months), and non-osteotomy group was followed up 12-79 months (mean, 34.33 months). There was no significant difference in follow-up time between 2 groups (t=–0.088, P=0.930). There was 11 cases of intraoperative or postoperative complications in osteotomy group, and 3 cases of postoperative complications in non-osteotomy group. Among the osteotomy group, 1 case had nonunion due to infection and received revision after 20 months. No loosening or dislocation of the implant occurred in both 2 groups. Significant differences were found in femoral dislocation height and settling depth of sleeve between 2 groups (t=–8.452, P=0.000; t=6.783, P=0.000). Moreover, the osteotomy length was not correlated with the settling depth of sleeve (r=–0.038, P=0.806). At last follow-up, there was no significant difference in Harris score between 2 groups (t=–1.160, P=0.254).ConclusionTHA combined with subtrochanteric osteotomy can provide a favorable outcome for treating Crowe type Ⅳ DDH. Furthermore, patients with higher femoral dislocation and severely narrow femoral proximal canals are prone to be peformed with subtrochanteric osteotomy.

  • Action guideline on promoting the high-quality development of rehabilitation services in public hospitals

    According to a guideline on promoting the high-quality development of public hospitals issued by the General Office of the State Council and a guideline on accelerating the development of medical rehabilitation, a high-quality and efficient rehabilitation system should be established, and efforts should be made to promote the high-quality development of rehabilitation services in public hospitals. We propose an action guideline for promoting the high-quality development of rehabilitation services in public hospitals, which is based on a Delphi method questionnaire on the opinions of front-line clinical rehabilitation experts, and integrated with relevant literature and evidences. This guideline introduces 31 specific action plans from 7 dimensions, including standardization of basic rehabilitation settings, standardization of clinical rehabilitation techniques, improvement of the tertiary rehabilitation system, standardization of emergency rehabilitation, standardization of rehabilitation education, standardization of rehabilitation management, and promotion of scientific and technological innovation in rehabilitation medicine.

  • A STUDY ON APOPTOSIS OF NEURONES WITHIN THE RETINA OF THE HUMAN FETUSES

    PURPOSES:To investigate the time of neuronie apoptosis in the retinas of Imman fetuses,and its relations with neuronie proliferation and differentiation, METHODS:The retinas of 27 human fetuses from 8th to 38th week of R,~til- ization age and 3 adults were studied by TdT-mediated dUTP nick end labelling(TUNEL) method. RESULTS:Tbe nuctei of labeled apoptotic cells were charaeterised by nuclear marginization,ehromatln condensation and cleseent shape,and some apoptotie bodies were visible in the specimens. The apoptosis of neuroepithelium of fetal rclina took place during 8th to 18th week, Apoptosis of ganglion cells were observed from 1256 to 18th week. The apoptos[s of pholorec, plors were formd from 14th to 2Ist week ,while thai of bipolar neurones and M~ller cells were found from ldth to 28th week. No apoptosb of ocstones were observed in the retinas after 28th week of fertilization age and within the retinas of adults. CONCLUSION:The proliferating cells of neuroepithelium and Ihe neurones which just differetiated from fetal retina might partly undergo apoptosis. The time of apoptosls of differentiated neurones was consistent with the time of the synapses formation between neurones and their targel cells. (Chin J Ocul Fundus Dis,1997,13:67 -69 )

  • Research on the development of restricted medical technology management in medical institutions from the perspective of high-quality development

    Under the background of high-quality development of public hospitals and from the perspective of medical technology management, this paper discusses the high-quality development path of medical technology management in West China Hospital, Sichuan University. By sorting out the development context of restricted medical technology management in this medical institution, it concludes that there are four problems in restricted medical technology management at present: lack of standardized management system, non-unified surgery grading management, technology management information level to be improved, and the after-effect evaluation and transformation mechanism of medical technology being not perfect. It is proposed that establishing standardized management system, unifying surgical grading catalogue, constructing intelligent information system and establishing achievement transformation mechanism are important ways to strengthen medical technology management.
