  • Emergent Percutaneous Transluminal Coronary Angioplasty in 41 Patients with Acute Myocardial Infarction in Elementary Hospital

    摘要:目的:探讨基层医院开展急诊经皮冠状动脉支架植入术(PCI)治疗急性心肌梗死(AMI)的可行性、安全性。方法:回顾分析2002年11月~2009年4月我院41例AMI患者的急诊PCI资料。结果:41例AMI患者,急诊开通梗死相关动脉(IRA)39例(即时成功率95.1%),开通IRA者中术后死亡2例(死亡率4.9%),总成功率90.2%。结论:在有条件的基层医院开展急诊PCI安全有效。Abstract: Objective: To explore the feasibility and safety of primary percutaneous coronary intervention in patients with acute myocardial infarction in elementary hospital. Methods: The clinical data of 41 AMI patients who underwent emergent PCI from November 2002 to April 2009 were retrospectively analyzed. Results: Among the 41 AMI patients referred to PCI, infarctrelated arteries were recanalized in 39 cases. The immediate success rate was 95.1%. 2 cases of them died. The total success rate was 90.2%.Conclusion: Emergent PCI is safe and effective in the hospitals which could carry out PCI.

  • Correlation Analysis between Rapid Emergency Medicine Score and Therapeutic Intervention Scoring System Score in Critically Wounded Victims in Lushan Earthquake

    ObjectiveTo investigate the correlation between rapid emergency medicine score (REMS) and therapeutic intervention scoring system (TISS-28) score and analyze the feasibility of assessing the nursing workload by REMS score for critically wounded earthquake victims, in order to provide reference for rapid and effective resource allocation for earthquake victims. MethodsA retrospective analysis was carried out on 39 Lushan earthquake victims with their acute plysiology and chronic health evaluationⅡ scores higher than 25, who were directly transferred from the earthquake site to the Emergency Department of West China Hospital between April 20 and 27, 2013. Among them, there were 24 males and 15 females aged between 5 and 90 years old averaging (57.1±19.8) years. REMS score and TISS-28 score were calculated for each victim. The relationship between REMS score and TISS-28 score was analyzed by correlation analysis and curve estimation including linear model, quadratic model, composite model, growth model, logarithm model, cubic model and exponential model. Then, we tried to find out the most suitable description for the relationship between REMS score and TISS-28 score. ResultsThe Spearman correlation coefficient between the two score systems was 0.710 and the most suitable description for the relationship between REMS score and TISS-28 score was logarithmic curve model. The formula was TISS=-5.946+4.467lnREMS. ConclusionREMS score can be applied as a nursing workload predicting tool for critically wounded victims in Lushan earthquake and it provides a guidance for rational allocation of health resources.

  • Evidence-based Chinese medicine for the response to public health emergencies: the Guangzhou declaration

    Traditional Chinese medicine has been used for the treatment of many diseases including acute infections often associated with public health emergencies for thousands of years. However, clinical evidence supporting the use of these treatments is insufficient, and the mechanism for using Chinese medicine therapy in the public health setting has not been fully established. In this report, the Evidence-based Traditional and Integrative Chinese medicine Responding to Public Health Emergencies Working Group proposed five recommendations to facilitate the inclusion of Chinese medicine as part of our responses to public health emergencies. It is expected that the Working Group’s proposals may promote the investigation and practice of Chinese Medicine in public health settings.

  • Disaster vulnerability analysis of a tertiary hospital in southwest China

    Objective To use Kaiser model, three-dimensional risk matrix and Borda ordinal value method comprehensively to analyze the vulnerability of disasters, and identify the key prevention and control risks of the hospital. Methods From June to December 2020, a disaster vulnerability analysis was conducted on a tertiary hospital in southwest China. The risk event evaluation index system was established by referring to the Kaiser model, and the evaluation indexes were classified into three aspects: possibility, controllability and harmfulness. The three-dimensional risk matrix was used to calculate the risk score to determine the risk level. The Borda ordinal value was used to compare the ranking of risk events in the same risk level. Result “Violent medical injuries” “terrorist attacks” and “explosions” were the highest ranked risk events, which needed to be prioritized and targeted for improvement. Conclusions Disaster vulnerability analysis is an important means of emergency management in hospitals. Managers should dynamically assess hospital risks, take effective preventive measures for high-risk events, conduct emergency plan drills, continuously improve risk warning mechanisms, and enhance emergency management capabilities.

  • Application of autoregressive integrated moving average model in prediction of outpatient and emergency visits in a district-level public hospital

    Objective To accurately predict the outpatient and emergency visits of a district-level public hospital based on autoregressive integrated moving average (ARIMA) model, providing important basis for hospital budget planning and operational decisions. Methods The monthly outpatient and emergency visits of a public hospital in Shuangliu District, Chengdu City from January 2012 to November 2023 were collected, and R 4.3.1 software was used to establish an ARIMA model based on the data from January 2012 to December 2022. The outpatient and emergency visits from January to November 2023 were predicted and validated. Results Except for January and March 2023, every monthly number of predicted outpatient and emergency visits for 2023 matched the actual one relatively well. The average absolute percentage error for January to November 2023 was 8.504%. The actual total number of outpatient and emergency visits from January to November 2023 was 1441960, and the predicted value was 1417130 with a relative error of –1.722%. Conclusions ARIMA model can predict the outpatient and emergency visits of district-level hospitals relatively well. However, factors such as the high incidence of COVID-19 may affect the accuracy of short-term prediction.

  • Effect of preoperative jaundice on complications of laparoscopic cholecystectomy combined with intraoperative biliary stone removal in patients with common bile duct stones

    ObjectiveTo explore the effect of preoperative jaundice on the complications of laparoscopic cholecystectomy combined with intraoperative biliary stone removal in patients with common bile duct stones.MethodsA total of 104 patients with choledocholithiasis who underwent laparoscopic cholecystectomy combined with intraoperative biliary stone removal for common bile duct stones in Baishui County Hospital and No.215 Hospital of Shaanxi Nuclear Industry between January 2014 and February 2016 were enrolled and retrospectively analyzed. The patients were divided into the jaundice group (43 cases) and the jaundice-free control group (control group, 61 cases) according to the preoperative serum total bilirubin level. The differences in postoperative complication rates between the two groups were compared and risk factors affecting postoperative complications were explored.ResultsThe ALT and total bilirubin on the first day after operation in the jaundice group were higher than those in the control group (P<0.05). In addition, the hospital stay in the jaundice group was shorter than that of the control group (P<0.001). There was no significant difference in the incidence of total postoperative complication rate and the incidence of complications (included biliary leakage, ballistic hemorrhage, hyperthermia, incision complications, and other complications) between the two groups (P>0.05). There were no significant differences in Clavien-Dindo classification, comprehensive complication index (CCI), and ratio of CCI≥20 (P>0.05). Multivariate analysis showed that male and residual stones were independently associated with postoperative complications (P<0.05), but there was no statistical correlation between preoperative jaundice and postoperative complications (P>0.05).ConclusionPreoperative jaundice does not increase the risk of complications after acute laparoscopic surgery in patients with common bile duct stones.

  • Evidence-Based Evaluation of Earthquake Emergency Plan in China

    ObjectiveTo analyze the earthquake emergency plans in 31 provinces (including autonomous regions, municipalities directly under the Central Government) of China, in order to summarize the relevant characteristics and discuss the existing problems to provide references for the revision and improvement of the earthquake emergency plan for the government and relevant departments. MethodsComprehensive literature search was conducted in the following websites:The Central People's Government of the People's Republic of China, China Earthquake Information Network, China Earthquake Administration, Government Websites and Earthquake Information Networks in 31 provinces (including autonomous regions, municipalities directly under the Central Government) of China. The literature which met the inclusion criteria was collected for descriptive analysis. ResultsEarthquake emergency plans in 31 provinces (including autonomous regions, municipalities directly under the Central Government) played a guiding role in the earthquake emergency and rescue work. However, some existing problems needed to be solved, which included:poor manoeuvrability of earthquake emergency plan, inadequate mechanisms of early warning and prevention, lower security on command platform and emergency shelter, and unclearly defined updating and revising time. ConclusionDifferent provinces (including autonomous regions, municipalities directly under the Central Government) of China should establish sound laws and regulations, improve the mechanisms of early warning and prevention, strengthen security measures of command platform and emergency shelter, and update and revise plans timely combined with its own characteristics, so that earthquake emergency plans could play a greater role in earthquake emergency in the future.

  • Construction of multi-parameter emergency database and preliminary application research

    The analysis of big data in medical field cannot be isolated from the high quality clinical database, and the construction of first aid database in our country is still in the early stage of exploration. This paper introduces the idea and key technology of the construction of multi-parameter first aid database. By combining emergency business flow with information flow, an emergency data integration model was designed with reference to the architecture of the Medical Information Mart for Intensive Care III (MIMIC-III), created by Computational Physiology Laboratory of Massachusetts Institute of Technology (MIT), and a high-quality first-aid database was built. The database currently covers 22 941 medical records for 19 814 different patients from May 2015 to October 2017, including relatively complete information on physiology, biochemistry, treatment, examination, nursing, etc. And based on the database, the first First-Aid Big Data Datathon event, which 13 teams from all over the country participated in, was launched. The First-Aid database provides a reference for the construction and application of clinical database in China. And it could provide powerful data support for scientific research, clinical decision making and the improvement of medical quality, which will further promote secondary analysis of clinical data in our country.

  • Treatment strategy for the obstructive or perforated rectal cancer

    Colorectal cancer is one of the common malignant tumor in the world, and about 57.6% of colorectal cancer surgical cases in our country are rectal cancer patients, which occupies a major proportion. Some patients with rectal cancer may already have emergencies such as intestinal obstruction or limited perforation at the time of consultation, which require immediate relevant treatment measures. Currently, there are multiple surgical and endoscopic treatment strategies available for obstructive and perforated rectal cancer. Surgeons need to perform an accurate and comprehensive assessment of the disease, define the goals of the current treatment, and formulate an appropriate treatment plan based on the patient’s clinical and oncological status in order to optimize the patient’s oncological outcome while minimizing the risk of complications associated with emergency colorectal surgery.
