  • Estimation of the number of patients with mitral regurgitation in China

    Mitral regurgitation (MR) is the most common heart valve disease. In recent years, the rise of interventional therapy has expanded the indications of interventional treatment for patients with MR, but the epidemiological characteristics of MR (especially the number of patients with MR requiring treatment) in China are still unclear. In this paper, we analyzed and estimated the number of MR patients in China based on three epidemiological studies in Europe and America, and referring to the patients population surveys from Zhongshan Hospital Affiliated to Fudan University, the Second Affiliated Hospital Zhejiang University School of Medicine and Beijing Fuwai Hospital. Our analysis estimated that about 7.5 million patients with MR need intervention in China, including about 5.5 million patients with severe MR.

  • Mechanism of immune checkpoint inhibitors related adverse events

    Most immune-related adverse event (irAE) associated with immune checkpoint inhibitors (ICIs) resulted from excessive immune response against normal organs. The severity, timing, and organs affected by these events were often unpredictable. Adverse reactions could cause treatment delays or interruptions, in rare cases, pose a life-threatening risk. The mechanisms underlying irAE involved immune cell dysregulation, imbalances in inflammatory factor expression, alterations in autoantibodies and complement activation, even dysbiosis of intestinal microorganisms. However, the mechanisms of irAE occurrence might differ slightly among organs due to variations in their structures and the functions of resident immune cells. Future research should focus on the development of targeted drugs for the prevention or treatment of irAE based on the mechanisms by which irAE occurs in different organs. A deeper understanding of the mechanisms underlying irAE occurrence would aid clinicians in effectively utilizing ICIs and provide valuable guidance for their clinical application.

  • Screening results of retinopathy of prematurity in preterm infants in 3 hospital s in Shenzhen

    Objective To learn the screening results of retinopathy o f prematur ity (ROP) of the preterm infants in three hospitals in Shenzhen. Metho ds From Jan. 2004 to Jan. 2007, 1372 preterm infants (2744 eyes) with birth weight lt;200 0 g or but the ones having severe systemic disease in Shenzhen People's Hospita l, Shenzhen Maternity and Child Healthcare Hospital and Shenzhen Eye Hospital we re screened for ROP with binocular indirect ophthalmoscope and (or) widefield digital pediatric retinal imaging system (RetCamII). Cryotherapy or laser photoco agulation was performed if threshold or pre-threshold type I ROP was found. All preterm infants were followed up until retina is completely vascularized or the disease regressed. Results In all the infants, 218 cases (436 eyes) (15.9%) developed ROP, including 190 eyes (6.9%) suffering from threshold or pre-threshold type 1 ROP, 16 eyes (0.6%) from stage 4 or stage 5, and 230 eyes (8.4%) from stages below threshold or pre-threshold type 1 ROP. There were 435 infants ( 870 eyes) (31.7%) with BW of 1500g or less, in which 236 eyes (27.1%) developed ROP, including 126 eyes (14.5%) suffering from threshold or pre-threshold type 1 ROP, 10 eyes (1.1%) from stage 4 or stage 5, and 100 eyes (11.5%) from stages below threshold or pre-threshold type 1 ROP. There were 137 infants 274 eyes (10%) with BW of 1250g or less, in which 108 eyes (39.4%) developed ROP, including 60 eyes (21.9%) suffering from th reshold or pre-threshold type 1 ROP, 4 eyes (1.4%) from stage 4 or stage 5, and 44 eyes (16%) from stages below threshold or pre-threshold type 1 ROP. Th eincidence of ROP(chi;2=60.43,Plt;0.001), the incidence of threshold or pre-threshold type 1 ROP(chi;2=46.82,Plt;0.001)and the incidence of below threshold or pre-threshold type 1 ROP (chi;2=10.71,P=0.005)among the total group, BWle;1500g group and BWle;1250g group had statistical differences. Conclusions The incidence of ROP in the three hospitals in Shenzhen was lower. However, the incidence of severe ROP (threshold or pre-threshold type 1 ROP) was higher. Birth weight is an important factor to affect ROP incidence.

  • Clinical Analysis of 205 Coats Disease

    Objective To analyze and summarize the clinical regulation and ocular fundus characteristics of the Coats diseases paients. Methods The clinical data of 205 Coats diseases (211 eyes) which including sex, age, which-eye and visual distribution were retrospectively analyzed. To analyze the region,scope, and degree of the disease according to the ophthalmoscopy, ocular fundus photography, and fundus fluorescein angiography results, in additon to invistigate the distribution and development rule of lesions. Results In 205 cases, Coats disease was diagnosed at a median age of 28 years old. There were 54.2% patients over 20 years old. It occurred in 76.1% males,and was unilateral in 97.1% patients. In 211 eyes, the visual acuity was below 0.3 in 67.3% eyes. The retinal telangiectasia occurred in all eyes, with yellow white lipid exudation in 90.5% eyes; located in temporal fundus in 90.1% eyes, in front of midperipheral 73.9% eyes; spread over one sector in 72.5% eyes. Conclusions Coats disease occurs mostly in males; it can occurrs in any age; it almost occurrs in unilateral. The essential characteritics are abnormal expansion of microvascular, mostly located in temporal fundus and in front of midperipheral, with yellow white lipid exudation, spread macular and damage visual function seriously.   • Epidemiological investigation of COVID-19 infection among young and middle-aged people in Lanzhou City of Gansu province under the new control policy

    Objective To discuss the epidemiological characteristics of young and middle-aged people infected with COVID-19 in Gansu province under the new epidemic policy. Methods A total of 1800 people were collected from two tertiary hospitals in Gansu province from November 8, 2022 to January 28, 2023. The vaccination status, nucleic acid antigen detection, the specific time of infection, main symptoms and severity of the disease were investigated. Results Among 1800 participants, 1685 (93.6%) were vaccinated and 1565 (86.9%) were infected with COVID-19. Among the 1565 infected persons, 523 (33.4%) completed both nucleic acid and antigen testing, 382 (24.4%) completed nucleic acid testing, 490 (31.3%) completed antigen testing, 170 (10.9%) received IgG testing. 1490 (95.2%) were slight ill, 75 (4.8%) were critical ill, and 96 (6.1%) were hospitalized, and no one died. In 2022, 92 cases (5.9%) were infected in the first half of November, 141 cases (9.1%) in the second half of November, 630 cases (40.3%) in the first half of December, and 553 cases (35.4%) in the second half of December. 109 cases (7.0%) were infected in the first half of January, 38 cases (2.2%) in the second half of January, and 2 cases (0.1%) in the first half of February of 2023. and no cases in the second half of February. Among the 1565 infected persons, 825 (52.7%) had respiratory symptoms, 293 (18.7%) had gastrointestinal symptoms, 257 (16.4%) had autonomic disorders, 140 (8.9%) had other symptoms such as decreased smell and taste, and 48 (3.3%) had no symptoms after infection. Conclusions The vaccination rate of young and middle-aged people in Lanzhou city of Gansu Province is high. Since the new policy, the infection rate of the novel coronavirus among young and middle-aged people is high, the number of antigen tests is more than nucleic acid tests, most of the infected patients are slight, with fewer critical patients, and the hospitalization rate is low. The peak of infection occurred in early December 2022, and the infection rate was basically zero by February 2023. The main symptoms of COVID-19 infection are mainly respiratory tract, followed by digestive tract and autonomic nervous system disorders, and few patients are completely asymptomatic.

  • The prevalence of age-related macular degeneration in the rural area of Qidong County, Jangsu Province

    ObjectiveTo study the prevalence of age-related macular degeneration (AMD) in the population aged 50 and over in Qidong County of Jangsu Province. Methods3644 individuals from 4 villages were randomly selected by clustering sampling method, according to the household registration information and door to door visits. Visual acuity was measured by modified Bailey-Lovie E logMAR chart. The examination of eyelids, cornea, lens and fundus were also carried out. The diagnosis of AMD was made according to the clinical hierarchy system by Age-Related Eye Diseases Study. χ2 test was used to analyze the prevalence of AMD and its related factors. ResultsAmong 3644 selected individuals, 2985 individuals received examination with a participating rate of 81.92%. In total 97 patients (136 eyes) had AMD with a prevalence rate of 3.25%. Among them, 71 patients (105 eyes) had early stage of AMD (2.38%); 26 patients (31 eyes) had late stage of AMD (0.87%). In these late stage patients, there were 9 patients (13 eyes) of exudative lesions (0.30%). There were 32 male (3.11%) and 65 female (3.32%) patients. There was no statistically significant difference between male and female prevalence (χ2=0.29, P > 0.05). Correlation analysis results showed that the long-term smoking (χ2=15.19) and heart cerebrovascular disease (χ2=81.50) was associated with AMD (P < 0.05). ConclusionsThe prevalence rate of AMD is 3.25% in the residents aged 50 and above in the rural area of Qidong County, Jangsu Province. Long-term smoking, high blood pressure and cardiovascular disease are the risk factors of AMD.

  • Epidemiological investigation of type 2 diabetic patients with diabetic retinopathy in Culai Town of Tai'an City in Shandong Province

    Objective To investigate the prevalence and related factors of diabetic retinopathy (DR) among residents with type 2 diabetes mellitus (T2DM) in Culai Town of Tai'an City in Shandong Province. Methods According to the DM management file database for community, 785 patients with T2DM were randomly selected by cluster sampling method. The questionnaires, routine general examinations, visual and fundus-free fluoroscopy were performed on all the patients. DR diagnosis and classification was according to the guidelines for clinical diagnosis and treatment of DR in China (2014). Both monocular and binocular DR were selected as DR patients, and the worse eye for binocular DR were treated as the DR classification of the patient. The patients were grouped by presence or absence of DR. GraphPad Prism 6, SigmaPlot 12.5, SPSS 20.0 and Excel were used to achieve data analysis. Also, SPSS 20.0 was used for multi-factor logistic regression analysis. Results A total of 699 patients (89.04%) were actually recorded. There were 122 eyes of 63 patients (9.01%) with DR (DR group), 1272 eyes of 636 patients (90.99%) without DR (NDR group). Among the 122 eyes of DR, there were 19 (15.57%), 17 (13.93%), 70 (57.38%), 10 (8.20%), 6 (4.92%) eyes in stage Ⅰ, Ⅱ, Ⅲ, Ⅳ, Ⅴ, respectively. The differences of mean age (t=15 290, P=0.002), DM duration (t=9075, P<0.000) and diastolic blood pressure (t=15 810, P=0.006) between the two groups were statistically significant. There were 23 (36.51%) and 394 (61.95%) patients with hypertension history in the DR group and the NDR group, with the significant difference ( χ2=15.42, OR=0.35, 95%CI 0.21-0.60). There were 57 (90.48%) and 500 (78.62%) patients with fasting blood glucose larger than 6.11 mmol/L in the DR group and the NDR group, with the significant difference (OR=2.51, 95%CI 1.06-5.95, P=0.031). Logistic regression analysis showed that the age, fasting blood glucose and DM duration were influencing factors for DR (OR=1.039, 0.864, 0.898; P=0.021, <0.000, <0.000). Conclusion The prevalence of DR in patients with T2DM in Culai Town of Tai'an City is 9.01%. Age, DM duration, fasting blood glucose are associated to DR. Those with a history of hypertension may have a lower risk of DR than those without a history of hypertension.

  • Global and China-US epidemiology and treatment status of valvular heart disease

    Objective To systematically analyze the disease burden, long-term trends, and age-sex distribution of major valvular heart disease (VHD) subtypes—rheumatic heart disease (RHD), non-rheumatic valvular disease (NRVD), and non-rheumatic calcific aortic valve disease (CAVD)—in global, Chinese, and US populations from 1990 to 2021, providing evidence for public health strategies and clinical resource allocation. Methods Based on publicly available data from the Global Burden of Disease (GBD) Study 2021, we extracted incidence, mortality, and disability-adjusted life years (DALYs) for VHD from 1990 to 2021. Age-standardized rates (ASRs) were calculated using the GBD 2021 global standard population, and the estimated annual percentage change (EAPC) with its 95% uncertainty interval (UI) was computed for the period. Data from the US Healthcare Cost and Utilization Project (HCUP), the European Society of Cardiology (ESC)/Eurostat surveys, and Chinese national registries were used for trend triangulation and contextual background. Results From 1990 to 2021, the ASR and disease burden of RHD significantly decreased globally and in China (EAPC for DALYs in China: -2.15%, 95% UI: -2.28% to -2.02%). In contrast, the burden of NRVD and CAVD has been steadily increasing in aging populations like those in China and the US, with a higher burden observed in older adults and males. In 2021, the incidence of NRVD and CAVD peaked in individuals aged ≥65 years, with rates being significantly higher in men than in women. RHD burden was concentrated in low Socio-demographic Index (SDI) regions, whereas NRVD/CAVD burden was strongly associated with high-SDI regions. ConclusionThe global VHD epidemiological landscape is transitioning from an RHD-dominant to an NRVD/CAVD-dominant pattern. China faces a dual challenge of a residual RHD burden and a rapidly growing burden of degenerative valvular diseases. Developing tailored screening, prevention, and treatment strategies for different disease subtypes and populations is crucial.

  • The differences and similarities between polypoidal choroidal vasculopathy and age related macular degeneration is worthy for further investigation

    Polypoidal choroidal vasculopathy (PCV) is one of the exudative maculopathy, which is characterized by retinal pigment epithelium detachment, subretinal hemorrhages and sensory retinal detachment. The prevalence of polypoidal choroidal vasculopathy (PCV) reached 33.5% in neovascular age related macular degeneration (AMD) for Chinese population. Indocyanine green angiography showed a single or multiple focal nodular areas of hyperfluorescence arising from the choroidal circulation and currently is recognized a gold standard for diagnosis of PCV. The histopathologic findings indicated that hyalinization of choroidal vessels, like arteriosclerosis. Up to now there is no reliable evidence to demonstrate the difference in genetic study. The study of environment factor showed hypertension is associated with PCV closely than with AMD. PCV and AMD is different genotype or different phenotype as well as difference in pathogenesis need further studies.

  • Investigation and improvement of prevention and control strategies for gastric cancer: epidemiology, pathogens, and risk factors

    ObjectiveTo summary the study results of the Sichuan Gastric Cancer Early Detection and Screening Research Program (SIGES). MethodsIn the past 10 years, SIGES program conducted a series of clinical and clinical epidemiological studies on the current situation and strategies of gastric cancer prevention and control in China, which provided evidence for supply-side structural reform to improve the gastric cancer prevention and control. We collected related studies and made a review. ResultsSIGES program systematically and specifically studied carcinogenic pathogens, risk factors, screening strategies, as well as critical technique and system construction of tertiary prevention for gastric cancer. The main results supported that Helicobacter pylori was highly correlated with precancerous atrophic gastritis and gastric cancer. Oncoviruses, such as Epstein-Barr virus, hepatitis B virus, human cytomegalovirus, human papillomavirus, John Cunningham virus were correlated with the risk of gastric cancer, and should be paid further attention in the prevention and screening strategy of gastric cancer. Ethnicity, domicile, and family history of gastric cancer were also correlated with gastric cancer and gastric cancer-related risk factors, so it was suggested to include in the personalized evaluation criteria system of high-risk groups for gastric cancer. ConclusionsAt present, the prevention and control of gastric cancer in China has achieved substantial achievements, but the burden of gastric cancer keeps heavy. It is still necessary to conduct further scientific research on critical technique or issues in designing prevention and control strategies.
