  • Investigation and analysis of humanistic care on improving the experience of inpatients

    Objective To understand the effect and influencing factors of humanistic care on improving the experience of inpatients. Methods Patients were collected from a third grade class A women’s and children’s hospital in June 2015 and June 2016, and their satisfaction was investigated by a third party. The service items of Inpatients Satisfaction Item Score Table in 2015 were analyzed. Appropriate intervention measures were taken to low-score items, such as humanistic knowledge training to all medical staff, improvement health guidelines, implementation of recycling process, carrying out high quality nursing interventions, and so on. The patients satisfaction survey results in 2016 were compared with those of 2015. Results In 2016, the total satisfaction rate (89.94%), and the average score of items ranked the top three (94.64±0.14), including the level of medical technology, medical ethics and the overall evaluation of doctor’s professional ehtics, medical communication and service attitude, were higher than those of 2015 (85.25, 90.86±1.53). The average score of items ranked the last three (89.25±9.21), including hospital ward, hospital environment (clean, quiet and safe), hospital meals and room service, and hospital food quality, was higher than that of 2015 (78.64±2.40). However, compared with the same period in the last year, the rank of hospital environment fell by two places. Conclusions Hardware conditions like physical environment have an important impact on the experience of hospital patients. However,  humanistic care is the key factor to improve the patients’ inpatient experience and satisfaction.

  • Application and practice of multi-disciplinary team service model in West China Hospital of Sichuan University

    Multi-disciplinary team (MDT) is a leading diagnosis and treatment model widely respected in modern international medical field, which plays an important role in clarifying disease diagnosis, determining treatment direction, and reducing patient time and labor cost. In recent years, China has issued relevant policies to encourage hospitals to vigorously develop MDT. West China Hospital of Sichuan University established a MDT clinic in 2013. Through continuous innovation and bold breakthrough in terms of experience accumulation, clinical resource integration, disciplinary collaboration and other aspects, the hospital breaks the barriers of MDT, timely helps complex severe diseases patients to solve the diagnosis and treatment needs, provides patients with the best personalized treatment plan, and continuously improves the medical experience of patients. Therefore, this article introduces and summarizes the implementation process and experience of MDT outpatient clinic in West China Hospital of Sichuan University.

  • Status quo of epilepsy management in Chinese community and international experience

    To understand the current situation of community epilepsy management in China, summarize the experience of international community epilepsy management, and provide reference for strengthening community epilepsy management in China. Summarize the current situation of epilepsy development in China, summarize and analyze the international experience of community epilepsy management in the United States, Australia, Britain and other countries, as well as the reference significance for domestic community epilepsy management. According to the evaluation criteria of community epilepsy management, it is suggested that the Chinese government should increase its support, formulate various strategic objectives, strengthen publicity and health education, improve patients' self-management support, explore the prevention and control mode of promoting epilepsy management in urban and rural communities, strengthen the training of medical service teams and design a reasonable referral system.

  • Enlightenments and reflections from Guangdong’s experience for the diagnosis-intervention packet national pilot cities

    In 2021, China launched a national pilot project of diagnosis-intervention packet (DIP) in 71 cities, which is an important measure of medical insurance payment system reform. DIP was originated in Guangdong. This paper summarizes the reform experience and effectiveness of Guangzhou and Shenzhen, discusses how to promote the reform of DIP in the national pilot cities from the medical insurance agencies aspect and the hospitals aspect, so as to provide a reference for the national pilot cities.

  • Research on the application effect of information construction of the hospital admission service center based on improving patients’ medical experience

    Objective To construct an information hospital service system and discuss the application effect of information construction in the hospital service center. Methods Patients admitted to West China Hospital of Sichuan University between June 2022 and January 2023 were selected. We innovatively practiced intelligent safety gate, self-appointment admission registration, pre-hospital examination and advance migration, pre-hospital health education, an age-appropriate transformation of information service, and other information service measures to investigate the medical experience of patients, and compared patients’ satisfaction with medical treatment under four admission management methods (Huayitong APP, WeChat, self-service machine, and manual management). Results A total of 1452 patients were surveyed. The overall satisfaction score for medical treatment of patients was (4.98±0.04) points. Among them, Huayitong APP was (4.99±0.03) points, WeChat was (4.98±0.13) points, self-service machine was (4.97±0.05) points, and manual treatment was (4.92±0.11) points. There was a statistically significant difference between groups in overall satisfaction with different admission procedures (F=68.582, P<0.001). Since the information construction of the hospital admission service center was carried out, the average time of admission was (12.4±2.3) minutes, and 89.4% (1 298/1 452) of patients thought the time of admission was ideal. Conclusions The information construction of a hospital admission service center can effectively improve patients’ medical experience and enhance patient satisfaction. In the future, it is necessary to explore the influencing factors of patients’ satisfaction with information construction, and constantly improve and upgrade the information construction of hospital admission service centers.

  • The interpretation for the application of "evidence as a core, consensus as a supplement, and experience as a reference" in clinical practice guidelines for traditional Chinese medicine or integration of traditional Chinese medicine and Western medicine

    Clinical practice guidelines need to be based on evidence, but traditional Chinese medicine, especially integration of traditional Chinese and Western medicine guidelines, inevitably need to combine clinical research evidence, ancient literature, and expert experience and consensus. In the process of formulating, there are deviations in understanding and application of the expert consensus method and expert experience, resulting in opaque of the recommendations, unclear of the details of the diagnosis and treatment strategy, less prominent of the advantages and value of traditional Chinese medicine, which affects the scientificity, transparency, applicability, promotion and application of the guidelines of traditional Chinese medicine or integration of traditional Chinese and Western medicine. This study discusses the guiding principle of "evidence as a core, consensus as a supplement, and experience as a reference" that was generally followed and puts forward detailed methodological suggestions to the formulation of guidelines for traditional Chinese medicine and integration of traditional Chinese and Western medicine.

  • Construction and application of a new follow-up visit model based on the Internet hospital consultation

    Objective To explore the construction and application of a new follow-up visit model in the context of Internet hospital consultation, aiming to create a novel follow-up visit model that integrates precise identification of follow-up patients, messages of follow-up reminders, online free follow-up visits, and promotional activities. Methods Satisfaction surveys were conducted among outpatient patients and doctors at Jintang County First People’s Hospital from July 2023 to June 2024. Patients and doctors were divided into two groups based on whether the online free follow-up visit program had been implemented: the pre-implementation group (July to December 2023) and the post-implementation group (January to June 2024). The satisfaction levels of patients and doctors before and after the implementation were compared and analyzed. Results A total of 17 831 patient visits and 801 doctor visits were included. Since its launch, WeChat messages had been pushed to all outpatient patients, and both WeChat and SMS messages had been pushed to patients in surgical departments. The average waiting time for outpatient visits in January-June 2024 was shortened by 2 minutes compared with the same period last year (January-June 2023). The hospital’s Case Mix Index increased by 3.7%, and the surgical volume increased by 7.5%. After the launch of the Internet hospital, both patient and doctor satisfaction improved. Conclusion The new follow-up visit model of the Internet hospital represents an important initiative in the digital transformation of hospitals and holds value and significance for promotion in more county-level medical institutions.

  • Research on coronavirus disease 2019 field disinfection technology for prevention and control

    The coronavirus disease 2019 (COVID-19) epidemic has had a serious impact in the world. In the absence of vaccines and therapeutic drugs, disinfection has become an important technical means to block the spread of the virus. By analyzing the characteristics of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), we studied a series of disinfection technologies for COVID-19. During the outbreak of COVID-19, Jinan Second Center for Disease Control and Prevention disinfected the houses of the cases to be investigated in a community. The effectiveness of the disinfection technology was verified through the process of disinfection preparation, sampling before disinfection, field disinfection, sampling after disinfection and evaluation of disinfection effect. Compared the data before and after disinfection, the killing rate of the total bacterial colonies in the air and on the surface of the object was more than or equal to 90%, and no SARS-CoV-2 was detected after disinfection. The results show that the disinfection effect of the disinfection technology meets the standard. Finally, through the analysis of the wrong way of disinfection and the harm of over disinfection, the importance of scientific disinfection and precise disinfection are emphasized, and the research has a good guiding value for prevention and control of the epidemic.

  • Hip preservation experience of avascular necrosis of femoral head according to China-Japan Friendship Hospital classification

    ObjectiveTo sum up staging treatment experiences of hip preservation for avascular necrosis of the femoral head (ANFH) according to China-Japan Friendship Hospital Classification (CJFH Classification).MethodsThe literature about hip preservation of ANFH was extensively reviewed, and a staging treatment method for ANFH was set up base on CJFH Classification and the 28 years of clinical experience in this research group.ResultsAccording to CJFH Classification, the ANFH can be rated as types M, C, and L. And the type L is rated as L1, L2, and L3 subtypes. The staging treatment method for ANFH based on CJFH Classification is set up. Conservative treatment was selected for CJFH-M patients because the necrotic area is small and in the medial non-weight-bearing area. Minimally invasive sequestrum clearance, impacting bone graft, and supporting allogenic fibular graft is selected for CJFH-C patients because the necrotic area is also small and in the lateral non-weight-bearing area. If patients are in CJFH-L1, normal bone area is less than 1/3 on hip abduction radiograph, the sequestrum clearance and impacting bone graft via surgical hip dislocation approach can be selected. If patients are in CJFH-L1, normal bone area is more than 1/3 on hip abduction radiograph, the transtrochanteric curved varus osteotomy can be selected. The rotational osteotomy on the base of femoral neck via surgical hip dislocation approach is for CJFH-L2 patients. Total hip arthroplasty via direct anterior approach is for CJFH-L3 patients.ConclusionThe staging treatment method for ANFH according to CJFH Classification has good short-term effectiveness. But the long-term effectiveness needs further follow-up.

  • Qualitative research of the actual experience in video electroencephalogram examination among epileptic patients

    Objective To explore the actual experience of epileptic patients in video electroencephalogram (VEEG) examination, and to provide reference basis for formulating corresponding nursing strategies and coping methods. MethodsIn this descriptive analysis study, 18 patients (11 males and 7 females, average age 37.78±18.7 years) receiving VEEG from January to April 2022 in the Second Affiliated Hospital of Guangzhou Medical University, underwent a semi-structural interview. Information obtained from the interview was analyzed using the Colaizzi 7-step method. ResultsThe actual experience of epileptic patients in video EEG examination can be summarized into two aspects: the medical experience and the need for nursing care. The medical experience includes positive and negative experience. The positive experience includes good service attitude, professional medical services, good endurance, and being hopeful. The negative experience includes a weird feeling in the head, insomnia, inconvenience in life, eye discomfort, psychological pressure, and pain. The need for nursing care includes needs for knowledge, strong needs for communicating with doctors, needs for humanistic care and female needs for female implementing the equipment. Conclusion Epileptic patients suffer from different degrees of discomfort and psychological pressure during VEEG examination. Both negative and positive experience exist. Medical staff should improve the content of nursing services according to the nursing needs of patients and provide professional VEEG examination services to patients.
