ObjectiveTo summarize the clinical application and future application prospects of organoid model in pancreatic cancer. MethodThe domestic and foreign literature related on the application of organoid model in pancreatic cancer was reviewed. ResultsIn recent years, the organoid model of pancreatic cancer was constructed mainly using patient-derived tissues, fine-needle aspiration samples, and human pluripotent stem cells. The biomarkers of pancreatic cancer were screened according to the histological and structural heterogeneities of the primary tumor retained in organoid model, such as microRNA, glypican-1, annexin A6 and protein biomarkers cytokeratin 7 and 20, cell tumor antigen p53, Claudin-4, carbohydrate antigen 19-9, etc.in the extracellular vesicles. The results of organoid model could maintain the original tumor characteristics and the higher correlation between the organoid model drug sensitivity data and the clinical results of pancreatic cancer patients suggested that, the drug sensitivity data of organoid model could be used to avoid ineffective chemotherapy, so as to improve the treatment response rate and reduce the toxicity of chemical drug treatment, and reasonably select individualized treatment plans for pancreatic cancer patients in future. ConclusionsOrganoid model has many research in screening biomarkers of pancreatic cancer, individualized drug screening, and drug sensitivity test. It can simulate the complex pathophysiological characteristics of pancreatic cancer in vitro, and retain the physiological characteristics and gene phenotype of original tumor cells. It is expected to become a new platform for selecting biomarkers of pancreatic cancer, testing drug sensitivity, and formulating individualized treatment methods for pancreatic cancer, which might further accelerate the research progress of pancreatic cancer.
OBJECTIVE:To investigate the value of psychophysical testing for the macular function in the diegnosis of diabetic retinopathy(DR). METHODS:To compare the testing results of macular light sensitivity and pattern visual evoked potential(P-VEP)of 30 eyes of 15 normal person with those of 82 eyes of 41 diabetic patients(27 eyes without DR,55 eyes with simple type DR ). RESULTS:The macular light sensitivity of diabetic patients is much lower than that of normal Control group(plt;0.05). In the diabetic group, 62.19% is abnormal in macular light sensitivity, 69.51% in P-VEP. CONCLUSION: Testing of macular light sensitivit y is helpful in finding of diabetic retinopathy and early deterioration of macular visual function in diabetics. ObjectiveTo evaluate the predictive value of the high-sensitivity cardiac troponin I (hs-cTnI) in patients with acute pulmonary embolism (APE). MethodsIn a retrospective cohort study,272 consecutive patients with APE were reviewed and the 30-days death and in-hospital adverse events were evaluated. The patients were classified according to hs-cTnI value into a high hs-cTnI group and a low hs-cTnI group. The simple pulmonary embolism severity index (sPESI) was used for clinical risk determination. The adverse event was defined as intravenous thrombolytic therapy,noninvasive ventilator support to maintain oxygen saturation >90% and suffered with severe complications. The correlations of hs-cTnI with sPESI score,30-days adverse events and mortality were analyzed. The Kaplan-Meier curves and the log-rank test were used to compare time-to-event survival. Stepwise multivariate logistic regression analysis models were used to determine the incremental prognostic value of sPESI score and hs-cTnI. ResultsThe incidence of 30-day death (6.1%),renal failure (14.6%),bleeding (13.4%) and thrombolytic therapy (7.9%) were higher in the high hs-cTnI group than those in the low hs-cTnI group (P values were 0.009,<0.001,0.018 and 0.003,respectively). The patients with sPESI ≥1 and low hs-cTnI had greater free adverse events survival (P=0.005). hs-cTnI provided incremental predictive value for in-hospital adverse events,beyond the sPESI score (P<0.001). Conclusionhs-cTnI has excellent negative predictive value of APE prognosis,especially when used combined with sPESI score.
In the development of radio frequency (RF) coils for better quality of the mini-type permanent magnetic resonance imager for using in the small animal imaging, the solenoid RF coil has a special advantage for permanent magnetic system based on analyses of various types of RF coils. However, it is not satisfied for imaging if the RF coils are directly used. By theoretical analyses of the magnetic field properties produced from the solenoid coil, the research direction was determined by careful studies to raise further the uniformity of the magnetic field coil, receiving coil sensitivity for signals and signal-to-noise ratio (SNR). The method had certain advantages and avoided some shortcomings of the other different coil types, such as, birdcage coil, saddle shaped coil and phased array coil by using the alloy materials (from our own patent). The RF coils were designed, developed and made for keeled applicable to permanent magnet-type magnetic resonance imager, multi-coil combination-type, single-channel overall RF receiving coil, and applied for a patent. Mounted on three instruments (25 mm aperture, with main magnetic field strength of 0.5 T or 1.5 T, and 50 mm aperture, with main magnetic field strength of 0.48 T), we performed experiments with mice, rats, and nude mice bearing tumors. The experimental results indicated that the RF receiving coil was fully applicable to the permanent magnet-type imaging system.
ObjectiveTo observe the contrast sensitivity (CS) of patients with branch retinal vein occlusion (BRVO) without involving the macular region. Methods92 BRVO patients (93 eyes) and 56 cases (112 eyes) without eye diseases (control group) were included in the study. According to different region, BRVO patients were divided into the nasal BRVO group (31 eyes) and temporal BRVO (62 eyes) group, and the average corrected visual acuity was 1.02±0.13 and 0.98±0.12 respectively. According to the type of ischemia, BRVO patients were divided into the nonischemic BRVO group (58 eyes) and ischemic BRVO group (35 eyes), the average corrected visual acuity was 1.01±0.14 and 0.99±0.12 respectively. The average corrected visual acuity of the control group was 1.03±0.11. There was no statistically significant difference of the average corrected visual acuity between nasal BRVO group, temporal BRVO group and control group (F=3.03, P=0.06), and between nonischemic BRVO group, ischemic BRVO group and control group (F=1.60,P=0.20). Contrast sensitivity (CS) was measured by OPTEC 6500 vision tester (Stereo Company, USA) under the standard lighting conditions and different spatial frequencies including low (1.5 and 3.0 c/d), medium (6.0 c/d) and high frequencies (12.0 and 18.0 c/d). ResultsThe CS under each spatial frequency of the nasal BRVO group was the same as the control group (t=4.25, 9.48, 3.08, 5.86, 0.94; P>0.05), but the CS under each spatial frequency of the temporal BRVO group was different from the control group (t=8.59, 19.11, 10.38, 17.28, 6.01; P<0.05). The CS under high spatial frequency of the temporal and nasal BRVO group was statistically different (t=11.42, 6.95; P<0.05). The CS under each spatial frequency of the ischemic BRVO group was different from the control group (t=8.88, 10.56, 11.64, 19.06, 6.67; P<0.05).The CS under 6, 12 and 18 c/d spatial frequency of the nonischemic BRVO group was statistically different with the control group (t=10.14, 11.54, 2.82; P<0.05). The CS under 12 and 18 c/d spatial frequency of the nonischemic BRVO group was statistically different with the ischemic BRVO (t=7.52, 3.84; P<0.05). ConclusionsThe CS of the temporal BRVO and ischemic BRVO decreased more significantly under each spatial frequency. CS is a better indicator to evaluate the visual function than the visual acuity in BRVO without involving the macular region.
Objective To predict clinical chemotherapy sensitivity of primary non-small cell lung cancer(NSCLC) by methylthiazal (MTT) tumor chemosensitivity assay method in vitro and detection of multidrug resistance gene1 (MDR1), and provide reference for clinical individualized treatment. Methods We selected 80 fresh primary NSCLC samples from NSCLC patients who underwent surgical resection in Zibo Central Hospital Affiliated to Binzhou Medical College between January 2009 and December 2011. There were 46 male patients and 34 female patients with their median age of 54 (29 to 81)years. Viable NSCLC cells obtained from malignant tissue were tested for their sensitivity to cisplatin (DDP), gemcitabine (GEM), docetaxe (DOC), etoposide (VP-16) ,and vinorelbine (NVB) using MTT assay in vitro. Fluorescent quantitative reverse transcriptase-polymerase chain reaction (RT-PCR) was used to analysis the expression level of multidrug resistance gene1 (MDR1). Results After exposure to antitumor drugs, morphologic changes, decrease of metabolic activity, and apoptosis were detected in NSCLC cells. MTT results showed that different individual cancer cells had different chemosensitivity to antitumor drugs, and cancer cells also had different chemosensitivity to different antitumor drugs. Inhibitory rates of cancer cells exposed to DOC, GEM, and VP-16 were significantly higher than those of cancer cells exposed to DDP and NVB (42.5%±9.5%, 40.5%±6.5%, 38.4%±7.6% versus 31.5%±8.5%,32.5%±7.8%, P<0.05).The positive rate of MDR1 in tumor tissues was 40.0% (32/80). The expression of MDR1 was not associated with tumor histological type, degree of differentiation, lymph node metastasis and TNM stage. The expression of MDR1 was associated with resistance to NVB (χ2=5.209,P=0.022),GEM (χ2=4.769,P=0.029),VP-16 (χ2=4.596,P=0.032),and DDP(χ2=6.086,P=0.014), but not associated with resistance to DOC(χ2=0.430,P=0.512). Conclusion MTT chemosensitivity assay can effectively predict clinical chemotherapy sensitivity. Detection of MDR1, together with MTT chemosensitivity assay, can more accurately predict NSCLC chemosensitivity and be a guide for individualized chemotherapy of NSCLC.
Objective To investigate the clinical features of hypersensitivity pneumonitis and misdiagnosis causes. Methods The morbidity, misdiagnosis, progression, treatment, recovery, relapse and experience of treatment of a case with hypersensitivity pneumonitis was retrospectively analyzed. Results Patients with hypersensitivity pneumonitis usually have a contact history of organic dust, and clinical manifestations are nonspecific. Chest radiography shows wandering pattern of multiple reticular or patchy infiltration shadows. Lung function tests showrestricted ventilation and impaired diffusion. Blood eosinophil is usually normal. Lymphocyte and sedimentation antibody in serum and bronchoalveolar lavage fluid may still be normal. Pathohistology is complicated, with a progression from acute inflammation to chronic fibrosis. Systemic steroid should be prescribed as soon as possible. Antigen avoidance should be emphasized. Conclusions Clinical symptoms of hypersensitivity pneumonitis is atypical, which may be easily misdiagnosed. The contact history, clinical features, chest radiography, laboratory examination and pathological changes should all be considered in such cases.
A multiple-stimuli-responsive drug-conjugated cross-linked micelles was prepared by radical copolymerization. The chemical structure, morphology, and size of the cross-linked micelles were characterized, and the drug loading of the micelle was calculated. The experimental results indicated that the hydrodynamic size of the drug-loaded micelles were about 100 nm, and the as prepared micelles could be degraded and swelled in presence of reducing glutathione (GSH). The low critical solution temperature (LCST) of the micelle was around 39.4℃. According to the experimental results, the micelles will shrink at temperature above the LCST. Subsequently, the accumulative drug release rate was up to 91.78% under acidic (pH 5.0), reductive (GSH 10 mmol/L) and high temperature (42.0℃) conditions mimicking the tumor microenvironment, while a relatively low release rate of 1.12% was observed without stimulation. The drug-conjugated cross-linked micelles showed a strong cell uptake behavior. In the cytotoxicity assay, the micelles exhibited effective anti-cancer activity and excellent biocompatibility. In brief, the experimental results show that the as-prepared drug-conjugated cross-linked micelle exhibits multiple stimuli-responsiveness, which holds great promise for anti-cancer drug delivery.
摘要:目的: 金黄色葡萄球菌(金葡菌)的感染近年来已成为医院内的主要致病菌,而其耐药性也呈逐渐升高的趋势,为了解该菌在我院的感染和耐药情况,为临床合理使用抗生素提供科学依据。 方法 : 用经典生理生化鉴定方法,对各种临床标本主要来源于痰液和各种伤口脓液标本分离到的102株金葡菌进行生物学特性及药敏试验。 结果 : 从我们医院2007年5月至2009年8月所分离出来的102株金葡菌中青霉素耐药性8923%,氨苄青霉素耐药率为9385%,没有发现万古霉素耐药菌。 结论 : 除万古霉素外,耐药率较低的依次是利福平、苯唑青霉素、环丙沙星、呋喃妥因、阿米卡星、磺胺甲基异恶唑、红霉素,而青霉素G、氨苄青霉素、四环素耐药性情况非常严重,并且多重耐药,耐药性强,应引起临床的高度重视。Abstract: Objective: To analyze the bionomics and antimicrobial susceptibility of staphylococcus aureus, which was the main pathogenic bacterium with high drug tolerance in our hospital, in order to provide the rational use of antibiotics. Methods : Samples of one hundred and two staphylococcus aureus cases from sputamentum and pus were evaluated by classic physiology and biochemistry methods to test the bionomics and antimicrobial susceptibility. Results : The drug resistance rate to penicillin, penbritin and vancomycin was 8923%, 9385% and 0, separately. Conclusion : Besides vancomycin, the drug resistance rate of rifampicin, oxazocilline, ciprofloxacin, furadantin, amikacin, sulfamethoxazole and sulfamethoxazole increased one by one. The resistance to penicillin G, penbritin and tetracycline was serious, including multidrug resistant, which should be paid highly attention.
The cold chain safety of vaccines is a global issue. The electronic vaccine vial monitor (eVVM) label can monitor the temperature of vaccines in real time and provide “early warning” prompts. In order to comprehensively evaluate the monitoring efficiency of eVVM, this study selected 75 eVVM labels and distributed them with a total of 600 vaccine vial monitor (VVM) labels of four different types in different experimental environment (2−8℃, −20℃ and 40℃), and used a temperature recorder as “gold standard”. The results showed that the accuracy of the eVVM labels and VVM labels in high temperature environment was as same as that of the temperature recorder (P = 0.195). The accuracy of low temperature anomalies report and high temperature anomalies report of eVVM labels was 100%, which was better than those reported by VVM labels. Therefore, eVVM labels have high monitoring accuracy, which is suitable not only for ordinary environments, but also for severe temperature environments. It should be helpful for the improvement of the efficiency and accuracy of cold chain monitoring.
